Mucopolisacaridosis tipo III C, un reto diagnóstico. Revisión de un caso clínico

dc.descriptionAntecedentes: Las mucopolisacaridosis son un conjunto de enfermedades raras de baja incidencia. Se caracterizan por la acumulación intra y extracelular de glicosaminoglicanos. En Ecuador, no existen cifras oficiales sobre la prevalencia de pacientes con estas enfermedades. Hasta el momento se han descrito 7 tipos y 13 subtipos de la enfermedad a nivel mundial. De estas, la MPS tipo III o Síndrome de Sanfilippo abarca 4 subtipos, de los cuales la MPS tipo III C es la menos común. Esta condición se hereda de forma autosómica recesiva. Clínicamente se caracteriza por una degeneración progresiva del sistema nervioso central, que provoca discapacidad intelectual grave, regresión del desarrollo y otras manifestaciones neurológicas. Su diagnóstico es clínico y se confirma con pruebas de laboratorio y moleculares. Además, no existe un tratamiento definitivo para sus manifestaciones primarias. Objetivos: Describir la presentación clínica de la mucopolisacaridosis tipo III C, detallar el procedimiento para establecer un diagnóstico preciso y precoz y revisar las opciones terapéuticas disponibles en la actualidad. Métodos: Recolección de datos a través de la anamnesis, examen físico, revisión de exámenes complementarios y la historia clínica de la paciente. Además, de una revisión bibliográfica complementaria para el análisis del caso. Resultados: Se describe un caso clínico de MPS III C en una paciente de 22 años que presentó dismorfias faciales y deterioro neurológico progresivo con epilepsia, pérdida de habilidades cognitivas, motoras y del lenguaje. El diagnóstico definitivo se realizó de forma tardía con análisis genético.
dc.description.abstractBackground: Mucopolysaccharidoses are a group of rare diseases with low incidence, characterized by the accumulation of intra and extracellular glycosaminoglycans. In Ecuador, there are no official statistics regarding the prevalence of patients affected by these conditions. So far, 7 types and 13 subtypes of the disease have been described worldwide. Among these, MPS type III, or Sanfilippo syndrome, is classified into 4 subtypes, of which the MPS type III C is rare. This condition is inherited in an autosomal recessive pattern. Clinically, it is characterized by a progressive degeneration of the central nervous system, leading to severe intellectual disability, developmental regression and other neurological manifestations. The diagnosis is clinical and it is confirmed with laboratory tests and molecular tests. Moreover, there is no definitive treatment for its primary manifestations. Objectives: Describe the clinical presentation of the mucopolysaccharidosis type III C, detail the procedure to establish an accurate and early diagnosis and review the currently available therapeutic options. Methods: Data collection through the anamnesis, physical examination, review of the patient’s complementary exams. Moreover, a complementary bibliographic review for the analysis of the case. Results: A clinical case of MPS III C is described in a 22-year-old female patient who presented with facial dysmorphisms and progressive neurological deterioration with epilepsy, and loss of cognitive, motor, and language skills. Definitive diagnosis was established late through genetic testing.
