| dc.description | Objetivo: Evaluar la exactitud diagnóstica de los modelos de inteligencia artificial para detectar o clasificar caries interproximal en radiografías bitewing utilizando los criterios radiográficos del ICCMS como método de referencia para el etiquetado diagnóstico. Materiales y métodos: Se realizó una revisión sistemática de estudios de exactitud diagnóstica siguiendo las directrices PRISMA-DTA y PRISMA 2020. Se incluyeron estudios originales que evaluaron modelos de IA aplicados a radiografías bitewing para detección o clasificación de caries interproximal mediante criterios del ICCMS. La búsqueda se efectuó el 2 de marzo de 2026 en PubMed, Scopus y ScienceDirect. La selección, extracción de datos y evaluación metodológica fueron realizadas por dos revisores independientes. El riesgo de sesgo y la aplicabilidad se evaluaron mediante QUADAS-2. Debido a la heterogeneidad metodológica, se realizó una síntesis descriptiva. Resultados: Se incluyeron cinco estudios publicados entre 2022 y 2025. La mayoría evaluó arquitecturas basadas en YOLO y un estudio evaluó un software preentrenado, con un total de 6384 radiografías bitewing antes del aumento de datos. Los modelos mostraron desempeño diagnóstico variable y reportaron métricas clínicas y computacionales, como sensibilidad, especificidad, exactitud, AUC, precisión, puntuación F1, exhaustivida (recall), precisión promedio (AP) y precisión promedio media (mAP). No fue posible realizar metaanálisis debido a diferencias en diseños, modelos, unidades de análisis y métricas reportadas. Conclusión: La IA debe considerarse una herramienta de apoyo a la interpretación radiográfica, no un método autónomo de diagnóstico. La evidencia disponible es limitada por la heterogeneidad metodológica, la ausencia de validación externa y el reporte incompleto de resultados. | |
| dc.description.abstract | Objective: To evaluate the diagnostic accuracy of artificial intelligence models for detecting or classifying interproximal caries on bitewing radiographs using ICCMS radiographic criteria as the reference method for diagnostic labeling. Materials and methods: A systematic review of diagnostic accuracy studies was conducted following the PRISMADTA and PRISMA 2020 guidelines. Original studies evaluating AI models applied to bitewing radiographs for the detection or classification of interproximal caries using ICCMS criteria were included. The search was performed on March 2, 2026, in PubMed, Scopus, and ScienceDirect. Study selection, data extraction, and methodological assessment were carried out by two independent reviewers. Risk of bias and applicability were assessed using QUADAS-2. Due to methodological heterogeneity, a descriptive synthesis was performed. Results: Five studies published between 2022 and 2025 were included. Most evaluated YOLO-based architectures, and one study assessed pretrained software, with a total of 6,384 bitewing radiographs before data augmentation. The models showed variable diagnostic performance and reported clinical and computational metrics, including sensitivity, specificity, accuracy, AUC, precision, F1-score, recall, average precision (AP), and mean average precision (mAP). A meta-analysis could not be performed because of differences in study designs, models, units of analysis, and reported metrics. Conclusion: AI should be considered a support tool for radiographic interpretation, not an autonomous diagnostic method. The available evidence is limited by methodological heterogeneity, lack of external validation, and incomplete reporting of results. | |
