    Actinic cheilitis: clinical, pathological and therapeutic considerations

    Actinic cheilitis (AC) is a chronic inflammation that affects more frequently to the lower lip and is considered as a potentially malignant disorder that could develop in to squamous cell carcinoma (SCC). AC is caused by excessive exposure to ultraviolet (UV) radiation, mainly sunlight radiation. Fair-skinned male patients, older than 50 years, who have worked outdoors or have had long-term sunlight exposure are more susceptible to develop this lesion. Their clinical manifestations are very varied; nevertheless, they are not related to the histopathological alterations that can be found. Histopathological diagnosis is the most important predictive factor of malignant transformation, depending on the severity of epithelial dysplasia. The characteristics of the AC are reviewed, so the clinician can provide optimal care to the patient and prevent lip cancer. The literature review was performed analyzing articles from PubMed, Science Direct, Google Scholar and Cochrane databases.

    Oral Candidiasis and Nutritional Status in Hospitalized Aged Patients: A Cross-sectional Study in Cuenca-Ecuador

    Background: Oral candidiasis (OC) is an opportunistic fungal disease related to various local and systemic factors. Hospitalized aged patients are considered susceptible hosts with several adverse clinical outcomes, including OC and undernutrition. Aims: The aim of this study was to determine the prevalence of oral candidiasis among hospitalized aged patients who attended on the inpatient units of Vicente Corral Moscoso Hospital and Jose Carrasco Arteaga Hospital of Cuenca-Ecuador. And also, to evaluate the association between OC and nutritional status in hospitalized aged patients. Methods and materials: This study was performed with two hundred and twenty subjects; convenience sampling was applied for seven months. Demographic data were collected by a questionnaire. OC diagnosis was accomplished by oral examination; clinical lesions suggestive were analyzed with swabbed culture tests. OC was diagnosed when patients exhibited oral suggestive lesions synchronously with positive culture results. Nutritional assessment was performed with Controlling Nutritional Status (CONUT) system. Statistical analysis was performed with IBM_SPSS-20 software; associations were analyzed using Chi-square test, or Fisher’s exact test, and multiple logistic regression. For all tests, 𝑝 ≤ 0.05 was considered as a statistically significant result. Results: Out of 220 hospitalized aged patients, 62 (28.1%) were diagnosed with oral candidiasis. The mean age was 78.3±7.8 years The Demographic/clinical variables (sex, age, residence area, education, use of dental prostheses) indicated no correlation with oral candidiasis. Nutritional variables (hypoalbuminemia and severe undernutrition were significantly correlated with oral candidiasis (p=0.009; p=0.007). Conclusions: 28.1 % of the hospitalized aged patients had oral candidiasis. Hypoalbuminemia and severe undernutrition are risk factors associated with oral candidiasis.

    Oral mucosal melanoma: a case report and literature review.

    Oral mucosal melanoma is an unusual and aggressive malignant tumor that mainly affects the palate of men aged between 50 and 60 years. We present a literature review focusing on the etiopathogenesis and the clinicopathologic features of this entity. In addition, we reported a rare case of an oral mucosal melanoma arising in the left cheek of a 60-yearold man. Computed tomography scan revealed infiltration of the tumor to other anatomic structures including the maxillary sinus, the zygomatic bone and the pterygoid processes. Based on its extension, the lesion was considered inoperable and treatment with three-dimensional conformal radiation therapy was proposed but the patient only attended to the first session and died from cancer progression 6 months after the diagnosis. This paper reinforces the importance of inclusion of this malignant tumor in the differential diagnosis of pigmented lesions of the oral mucosa.

    Diabetes mellitus y su asociación con el carcinoma oral de células escamosas: una revisión de la literatura

    Oral squamous cell carcinoma is a malignant tumor characterized by the uncontrollable, excessive, and irreversible proliferation of squamous cells of the oral epithelium, representing most of all oral cancers. Interestingly, despite advances on therapeutic procedures such as radiotherapy, chemotherapy, and surgical techniques, the survival rate has not improved in recent decades. On the other hand, diabetes mellitus is a chronic disease with clinical presentations and metabolic disorders characterized by increased blood glucose levels. Various studies suggest a possible relationship between these two entities; evidencing that a person with diabetes and, in addition, poorly controlled, may have many oral manifestations, such as the possible development of oral malignant lesions. This manuscript aims to present and discuss the updated scientific evidence on the relationship between diabetes mellitus and oral squamous cell carcinoma.
