    A retrospective descriptive study of 100 patients operated by obesity with different techniques in the Enrique Garcés Hospital of the City of Quito, which shows the actual trend of the prevalence of gastric bypass and vertical sleeve gastrectomy, to which during monitoring were applied the SCORE OF BAROS evidencing a comprehensive improvement in the quality of life that complements weight reduction and resolution of comorbidities in patients.

    Los servicios de emergencias médicas en el Ecuador: Una tarea pendiente.

    Pre-hospital care is a relatively new discipline within the field of medicine. However, it is becoming one of the most complex and important areas for the management of acute pathology. Pre-hospital services do not currently fit a unique model because they must meet countries’ specific needs. The inadequate replication of a specific model could lead to failure of these services, causing inefficient and inaccurate management of time-dependent pathologies. In Ecuador, the development of pre-hospital emergency medical care has been uneven. Attempts to organize services have not been successful. A national program for emergency care is currently being implemented that aims to achieve coordination among institutions. However, there are still problems related to organizational structure, which we discuss in this paper.

    Percepción sobre el perfil profesional de la carrera de medicina de la Facultad de Ciencias Médicas de la Universidad de Cuenca. 2013

    Objective: To determine the criterion of graduate students of the School of Medicine and leaders of health institutions about the professional profile and competencies of graduate physician at the University of Cuenca Methods: A descriptive study was conducted with a sample of 310 physicians with no more than 10 years following graduation from the career and with the heads of health services regardless of their graduating year, who were given a survey to ascertain their views on the work performance against medical professionals profile offered by the Medicine School of the University of Cuenca. Results: From 310 surveys, 240 were applied to graduates of the Medical School and 70 were applied to the heads of health services, accounting for 77.5% and 22.5% of respondents, respectively. The average age of graduates was 29 years and the average age of the heads of health services was 44 years. As the years have passed from graduation until the survey was 4 years on average for graduates and 16.5 for departmental heads. In the perception of both groups on the level of university education are minor differences, with a positive balance on the criteria in the development of competencies placing in the categories of “Very Good” and “Good” when rating the various aspects of skills declaring graduate profile.

    Coexistencia gist con cáncer gástrico

    Gastrointestinal stromal tumors (GIST) in conjunction with adenocarcinoma are rare, these constitute the majority of non-epithelial neoplasms of the stomach. They can compromise the small intestine, mesentery and retroperitoneum. Originates from the interstitial cells of Cajal, wall of the gastrointestinal tract and can be distinguished from other tumors by immunohistochemistry. Pathogenesis of gastric cancer is related to environmental factors. Its incidence has declined in countries like the US and Britain, but remains high in others such as Japan, Chile and Italy. There is evidence that genetic predisposition plays an important role in a subset of patients. Gastric carcinomas originate from the basal cells of the foveal on a background of chronic atrophic gastritis with progressive dysplasia. Meet one recorded case in Cuenca concomitant gastric adenocarcinoma and GIST, which corresponds to a 75-year-old spoke gastric intramucosal well differentiated adenocarcinoma, also during surgery the presence of a tumor in the jejunum is noted for study revealed the presence of pathological GIST confirmed with immunohistochemistry techniques.     Enfermedad de stargardt o degenaración macular de stargardt

    The Stargardt Macular degeneration is considered a rare disease; one case per 10,000 people is presented. (1) It is Inheritance and it is given an autosomal recessive pattern. It is caused by a mutation of ABCA4 gene, which has a transport function through the membrane of the photoreceptor cells. There are 558 different mutations that can cause it. (2) The Stargardt disease and fundusflavimaculatusare the same disease, the second is a more advanced state of lipofuscin storage.(3)

    Patologías más frecuentes en el servicio de emergencia pediátrica del Hospital Vicente Corral Moscoso Cuenca, mayo-julio 2011

    In the data collected in the pediatric emergency service in the Vicente Corral Moscoso Hospital, in the period from May to July 2011 was known what are the 10 most frequent causes of attention that compared to the pathology registered in hospitals, in which similar studies were made, we observed in Mexico the upper respiratory tract infections, followed by accidents and poisoning, infections of the digestive tract, acute asthma, pneumonia. In Argentina the main complaint was upper respiratory tract infection, asthma attacks and bronchiolitis, trauma, gastroenteritis, pharyngitis, otitis, rash, pneumonia, fever without source. In our hospital was registered similar pathology of the upper respiratory tract, lower accidents within these registered trauma, penetrating wounds, and also gastrointestinal disorders. Objective: To determine the ten most common diseases in the pediatric emergency service in the Vicente Corral Moscoso Hospital May-July 2011. Material and Methods: A descriptive study was made; it was based on the universe of total patients who are attending to the pediatric emergency in the Vicente Corral Moscoso Hospital during the months May- July 2011. It was taken as reference the attentions the pediatric emergency service in 2010. They were 15,718 children. The Epiinfo program version 6.0 and SPSS version 15 was used. A total of 300 patients treated in the pediatric emergency were taken during the months of May-June and July 2011. Results: The 10 most frequent pathologies treated in the pediatric emergency service in the VCMH are: 1) ARI (Acute Respiratory Infection) with pneumonia (8.3%). 2) ARI without pneumonia (8%). 3) Stab wounds (7.3%). 4) Pharyngoamigdalitis (6.7%). 5) Mild ECT (Encefalo craneal trauma) (5.7%). 6) Inflammatory acute abdomen (4.7%). 7) UTI (Urinary Tract Infection) (4.3%). 8) Foreign body ingestion (4.3%). 9) Polytrauma (4%). 10) ADD (Acute Diarrheal Disease) without dehydration. All other diseases classified as others represent 43.3%. Conclusions: The importance of this study was to determine the most common diseases in the pediatric emergency service in the Vicente Corral Moscoso Hospital, for better information about them and thus take preventive actions necessary to achieve holistically health pediatric community in our city

    Estudio cualitativo del aprendizaje de ciencias básicas en medicina. La neurociencia como modelo de "aprender a aprender", un estudio en la Facultad de Ciencias Médicas de la Facultad de Ciencias Mëdica de la Universidad de Cuenca-Ecuador

    Introduction. A change in the model of professional training in the Ecuadorian University needs urgent modifications that are related to the behaviour of the actors immerse at the process; teachers being tutors - counselors and students being apprentices. Methodology. It is a descriptive observational study analyzing the instruments used in a training course in basic science, particularly neurophysiology in the first stages of the medical career and his correlation with the styles developed by the students in his learning process against the final results. Results. The sample includes 42 students during a cycle of 6 months in the neurophysiology course; 24% fulfill a minimal level of knowledge about prerequisites (neuro- anatomy); the learning tools show an important dichotomy between a collaborative, problem based learning during the daily activities and tools of examination based on repetitive learning at the first half of the course and problem based examination towards the end. The 19 % of the students fails and does not demonstrates to manage and generate tools of self-learning; of the 81 % that success the course, only a third develop tools and habits of self-learning. Conclusion. The specialization in teaching is a basic tool for education in medicine schools; it is not enough to ´know very much´ but to ´be able to show and tutored the learning process´. A new education university system must be based on the commitment of all the actors, better tools of learning and especially harmonize the evaluation tools with the career goals

    Caracteristicas de la Investigación en el posgrado: avances y limitaciones. Facultad de Ciencias Médicas. Universidad de Cuenca. 2009-2014

    Objective: To determine the characteristics of the postgraduate research in order to generate research policies. Methods: A cross-sectional analytical research was conducted to determine the characteristics of research in the various postgraduate levels. A stratified random sample of the database of postgraduate research contained in the website of the University of Cuenca was used. Chi2 statistic was used, and a level of significance was accepted p <0.05 in hypothesis testing. The sample consisted of 98 research papers. Results: The model for biomedical research is predominant (79.6%) in the Masters is the social (65.5%). (p: 0.000). Most are clinics (78.6%) investigations. Research with social components developed in the masters (72.4%). (p: 0.000). The number of researchers males (50.0%) is similar to that of women (50.0%). The investigations are made by an author. Investigations are applied (100.0%) and quantitative (94.9%). Only the Masters there are qualitative (6.9%), mixed (6.9%) and other (3.4%) investigations. (p = 0.051). Dominated by observational studies (78.6%); the intervention constitute 21.4% (p = 0.889). The descriptive research (36.7%), analytical (39.8%) have greater weight than those of intervention (23.5%), p: 0.321). In the analytical cross-sectional research predominate (89.7%), and the experimental intervention (78.3%). There is no international cooperation and inter is minimal (7.1%) referred to the Masters. There is good correspondence with the lines of institutional research (57.1%) in most research and the use of international bibliographic standards is high (99.6%). Conclusions: The fundamental characteristics of research at the graduate has not changed substantially. The predominant biomedical research remains, quantitative, clinical, observational, there has been progress in reducing the descriptive investigations and have increased (transverse) and experimental analytics. The master has developed more social health research. Research has failed to overcome the academic and personal interest level, no international collaboration and inter is scarce. It has improved the technical component, is majority use international bibliographic standards
