The mandibular premolars have earned the reputation of possessing an unusual anatomy. The incidence of two canals in mandibular premolars is rare according to researchers in endodontics. At the time of treating mandibular premolars with two canals, one must be aware of their clinical and radiographic anatomy. These teeth require special sealing and configuration techniques. This article reports and discusses the treatment recommendations for an unusual occurrence of two canals with two sepárate apical foramina.
