    Absceso cerebral. Reporte de caso

    Introduction: brain abscess, characterized by focal suppurative collections of the brain parenchyma, constitutes a global health problem due to its high morbidity and mortality and economic burden for the health system.Clinical case: It is a case of a35-year-old woman, who the last trimester of pregnancy had right suppurative otitis media without treatment. During the mediate puerperium, the patient had an intense headache and self-medicated oral analgesics without improvement. The woman goes to the hospital, where a Computerized Axial Tomography (CT) was made, it reports two hypodense lesions, encapsulated in the temporal lobe, surrounded by vasogenic edema with post-contrast ring enhancement; In Nuclear Magnetic Resonance (NMR), the two collections restrict the diffusion sequence with a drop in the Apparent Diffusion Coefficient (ADC) signal. A broad spectrum antibiotic and immediate surgical drainage with placement of an external drain are started. It has a favorable evolution.Conclusion: brain abscess is life threatening, its prognosis depends on timely diagnosis. The severe ear infection contributed to the development of this pathology, being a rare cause in adults

    The Primary Health Care and its holistic vision go beyond the limited medical model. Since the Alma Ata international conference until now, about 37 years have passed and it is still the best way to reform the health, reduce morbidity, increase equity and improve significantly the effectiveness of health systems. Although there are great advances in public health and some encouraging national indicators, it is important to emphasize the disease prevention activities and health promotion, because according to WHO data, the load morbidity could be reduced in 70% around the world if it is applied correctly. This article compiles a review in the main biomedical scientific literature databases which are available (PubMed, Lilacs, Scielo, EBSCO, SeCiMed), to resume, update and strengthen the concepts, approaches and progress made by the APS in order to share them with current and future health professionals

    Características de la diabetes mellitus tipo 2 en pacientes de la etnia shuar atendidos en el Hospital General de Macas, 2012

    Introduction: since the discovery of diabetes me - llitus and its complications have been a tragedy for humanity and Ecuador. In indigenous peoples, also confirmed a dramatic increase in diabetes, have been forced off their land and adopt a se - dentary lifestyle. In the Macas General Hospital is common to find the Shuar diabetics, who usually come when they have any complications. This vulnerable group is undergoing a transition period towards urbanization and requires special care in their treatment and monitoring. Objective: Identify the characteristics of type 2 diabetes mellitus in patients treated at the Shuar Macas General Hospital in 2012, and their kno - wledge of and attitudes toward their illness. Materials and methods: It is a descriptive study case series conducted in Shuar diabetics attending the Macas General Hospital. The data were obtained from the monthly report of disease, and medical records, which are in the electronics HOS - VITAL. To have evidence of their knowledge and attitudes, surveys and interviews were used to obtain variables, and information was processed in Excel 2010. Results: We found 227 diabetics treated from Ja - nuary to June this year in the area of consultation and hospitalization, which corresponds to the Shuar patients’ sample that formed 41, ie 18.06%. The characteristics of the sample indicated a higher prevalence of women 51 to 60 years, who reside mostly in the cantonal head. Of the 41 Shuar diabetics, the 56.10% were in hospital, ad - mitted for decompensated diabetes mellitus, and of these, 48.39% were insulin dependants. Among the chronic complications, 50% had diabetic foot. In surveys and interviews, 22% had diabetes of 1-5 years, 64% are aware of their disease, but 86% are unaware of the complications. With regard to treatment there is no medical checks in regularly monitoring, and mentioned other empirical treatments such as verbena water. Conclusions: In the Macas General Hospital, al - though we identified a small group of diabetics Shuar, found that most go in bad condition pre - senting uncontrolled hyperglycemia, or diabetic foot, and do not seek regular medical checks for outpatient so that the majority was identified in ward. This is based on lack of knowledge and poor attitudes to their disease, and this adds the location and preservation of cultural beliefs that somehow don’t allow conducting a proper medical intervention.

    Limitación Funcional y Factores Asociados en Adultos Mayores de la Zona 6 de Salud en Ecuador

    OBJECTIVE: To estimate the prevalence of functional limitation (FL) and socio-demographic factors, health problems and chronic diseases associated to senior adults population from 6th health zone.METHODS: This is an observational cross-sectional research performed from June to December of 2015 at primary care units influence area from 6th health zone of Ecuador. 714 elderly people were included and randomly selected from 33 primary health care units.RESULTS: Average age was 76±8.31 years (min: 65; max: 102 years), 63.8% of them were female. Prevalence of functional limitation in BLA (Basic Life Activities) was 17.65% and 62.61% for IAL (Instrumental Activities of Life). Associated factors with FL in BLA were: age ˃ 75 years, illiteracy, cognitive impairment, depression, social problems and fall hazard. Age ˃ 75 years, illiteracy, cognitive impairment, depression and the risk of falling were found to be associated factors to limitations in IAL.CONCLUSIONS: The functional limitation in elderly adults is associated with aging and depends of several determinants. The knowledge of these factors will contribute for early detection of dependency and to improve health attention coverage and quality

    Salud integral y derechos del adulto mayor. Estudio en la comunidad de Maluay - El Valle

    The 6.5% of the Ecuadorian population are elderly, died about 34,000 each year. Their welfare is a political and social concern, but their rights are often violated by a society living their daily lives in fighting stronger and forget to reach out to the stars of the past, who show a negative impact on your overall health. Sectional study of 40 seniors Maluay community (95.2%) with comprehensive health was assessed through the MSP application forms. Documentaries and direct observation techniques were applied. Most are women between 65 and 74 years, mean 69.5 years, illiterate / as (93%) with no activity or engaged in fieldwork and no drinking water. Their conditions are affected by chronic degenerative diseases that confers some degree of disability. At risk of malnutrition (73%), cognitive impairment (65%), almost half with some difficulty with balance and gait. Although they are independent in basic activities of daily living, need some help in instrumental activities. Socially are accepted (78%). The quality of life of older adults, especially in rural areas is not appropriate despite the support of public policies. Knowledge of risk factors and needs to develop prevention strategies and promoting a holistic approach should be the priority in public health, as we go through a transition that addresses the emerging risks associated with industrialization, urbanization and population aging.
