    A school-based intervention improves physical fitness in Ecuadorian adolescents: a cluster-randomized controlled trial

    Background: Effective lifestyle interventions are needed to prevent noncommunicable diseases in low- and middle-income countries. We analyzed the effects of a school-based health promotion intervention on physical fitness after 28 months and explored if the effect varied with important school characteristics. We also assessed effects on screen time, physical activity and BMI. Methods and results: We performed a cluster-randomized pair matched trial in schools in urban Ecuador. The intervention included an individual and environmental component tailored to the local context and resources. Primary outcomes were physical fitness (EUROFIT battery), screen time (questionnaires) and physical activity (accelerometers). Change in BMI was a secondary outcome. A total of 1440 grade 8 and 9 adolescents (intervention: n = 700, 48.6%) and 20 schools (intervention: n = 10, 50%) participated. Data of 1083 adolescents (intervention: n = 550, 50.8%) from 20 schools were analyzed. The intervention increased vertical jump (mean effect 2.5 cm; 95% CI 0.8-4.2; P = 0.01). Marginally insignificant, adolescents from the intervention group needed less time for speed shuttle run (intervention effect = −0.8 s, 95% CI −1.58-0.07; P = 0.05). The proportion of students achieving over 60 minutes of moderate-to-vigorous physical activity/day decreased over time with the change in proportion significantly less in the intervention schools (6 vs. 18 percentage points, P < 0.01). The intervention effect on speed shuttle run was significant in larger schools while the effect on vertical jump was larger in mixed gender school compared to small and female schools. The proportion of schools that met the recommendations for physical activity increased with 37% in intervention schools with half-day schedule compared to the controls in the pair. No significant effects were found on screen time and BMI. Measurement of physical activity in a subsample was a limitation. No adverse effects were reported. Conclusions: A school-based intervention with an individual and environment component can improve physical fitness and can minimize the decline in physical activity levels from childhood into adolescence in urban Ecuador.

    OBJECTIVE: To evaluate the relationship between self-perception of body image measured by the test of silhouettes with nutritional status measured by body mass index in adolescents 15 to 18 years by sex in the city of Cuenca from 2009 to 2010. METHOD AND MATERIALS: We made a descriptive study in students 4th-6th year of three public schools. Adolescents were surveyed in classrooms and then proceeded to weigh and to heigh each one getting their nutritional status. Variables were used: sex, age, height, weight, BMI, nutritional status, steps to take to get a proper weight for your height and self-perception of body image, that information was processed in Epi Info (TM) 3.4.3, EpiDat 3.1 and Excel 2007. RESULTS: Of the sample of 497 adolescents, 323 women (65%) and 174 men (35%), 72.8% had a normal nutritional status, 19.9% overweight, 3.4% obesity and 3.4% underweight. 58.8% of adolescents have an adequate perception of body image versus 38.4% that showed distortion, perceived with more weight. In females, the greater the distortion, perceived with more weight in 41.5%, a similar result in males that are 35% more weight misperception. CONCLUSION: The problem of distortion in the perception of body image is clearly evident in both sexes. It is very important to know the value of distortion in the adolescent population of our country as it could be related to dissatisfaction with physical appearance, low self esteem and eating disorders, hence arises the need for future studies that analyze the causes and effects on distorted self-perception, and these results raise action plans to combat these problems

    Cambios en los patrones alimentarios de los habitantes del barrio El Vergel, Cuenca – Ecuador, enero - julio 2014

    Objectives: To determine the changes in the eating patterns in habitants of the neighbourhood El Vergel, Cuenca - Ecuador Material & Methods: It is a descriptive, prospective and longitudinal study, a sample of 126 families, all with more than 20 years of residence in the neighbourhood El Vergel. Validated by the Chilean society of nutrition, food science and Toxicology surveys were used. We applied the trend survey of consumer food (TSCF) to determine their eating patterns for 20 years ago, a survey where there was the type of food consumed by the inhabitants 20 years ago and the number of times that each of them was consumed weekly. To determine their current eating patterns, we used the food reminder 24 hours modified (FR24Hrs). We made a comparative analysis of the results of the two surveys applied to determine the changes in the eating patterns. Results: The daily diet in the El Vergel neighborhood 20 years ago is composed by 50% from carbohydrates, 21.1% of proteins, 10.1% of fats and 17.9% of vegetables and Greens. Within the current food pattern was found that carbohydrates make up approximately 60% of the total of nutrients in the diet, followed by protein 18.2%, fat 15.40% and vegetables and vegetables 6%.     Caracteristicas del cáncer dermatológico diagnosticado en biopsias de piel por el laboratorio de anatomía patológica del Hospital Vicente Corral Moscoso en el periodo junio 2011 – junio 2013

    Introduction: The dermatological cancer is the most common our society, the correct diagnosis and treatment are critical in their impact within the patient's prognosis. In Ecuador, its geographical position, this cancer has a special connotation, by exposure to ultraviolet light, which is the most important for the genesis of this disease risk factor. Methods: A descriptive cross-sectional biopsies of skin samples processed by the Laboratory of Pathology of the Hospital Vicente Corral Moscoso in the period June 2011 to June 2013 was performed. The inclusion criterion was the malignancy of the samples regardless of their stadium. Results: The total sample was 401 of which 63.3 % (254) for females (147) were men. The percentage of malignancy reported is 29.4 %, of which 50 % is basal cell carcinoma, squamous cell carcinoma 28.6 % and 15.8 % melanomas. The localization of malignant lesions, 59.3 % are in the face, 20.3 % in lower limbs, trunk 12.7%, 4.2 % lead and 3.4% in the upper limbs. Conclusions: dermatologic cancer is present in the third of the skin samples processed. 2-1 relationship between women and men was found. The age at which most occurs is between 65 and 84 years, the lesions are located mostly on the face, in two of the three cancers , melanoma is the most aggressive type is commonly found in the lower limbs.     Concepciones y formas de tratamiento de dolor crónico en la medicina tradicinonal

    As traditional Andean medicine a daily practice, we decided abservar the characteristics of this in different communities and learn how they define and they treat chronic pain. The research was conducted in the communities of Saraguro [Llincho, San Lucas] and Canari [Correhurco and Sigsihuayco]. with two groups of respondents: 1. Residents of the communities that practice traditional medicine which was applied semi-structured interviews. 2. Key informants [Yachags, Amautas and capacs] by a focus group. For the inhabitants of these communities pain is defined as terms that correspond to experiences. The source of pain is given by the bad air, spoiled food, climate change and sorcery. Most of these people rely on traditional medicine using herbs, clean, ceremonies, that provide benefits in healing. The pain is very diverse because each person plants use their own discretion, so that it can not give a general approach to treatment. For key reports Saraguro pain is defined as an energy imbalance, caused by disorders in the relationship with society, nature and himself. People are a source of good and bad energies, affecting himself causing pain [bad energy domain] and also affect surrounding quines. Treatment consists in tracing the origin of the problems with physical obervación, behavior, urine and ceremonies. The ceremonies are guided by the Yachag who maintains an appropriate balance to positively influence others and coping with negative stimuli presented. In the ritual all people who want to get their bad anergías and drinks is achieved by certain preparations involved, [that cause vomiting, crying, screaming], lashes and clean with medicinal plants. In the communities of the Cañar Yachag agree on the definition of pain when referring to this is caused by an energy imbalance, but the origin is given by the factors that triggers Saraguro take them. The treatment is done based on the source of pain

    Diferencias en calidad nutricional de los alimentos expendidos en los bares de los colegios del grupo control y los del grupo de intervención del programa nutricional ACTIVITAL de la ciudad de Cuenca

    Introduction: Obesity is a problem with an increasing occurrence in young people and in developing countries. Poor nutrition is a risk factor for obesity and other chronic noncommunicable diseases. Methods: the recipes prepared by the staff of the bars in the control group for two days randomly selected using a kitchen scale to the nearest 1 gram were measured. The average of each bar recipe nutritional content compared with recommendations for snacks. It is further estimated the average energy density of prescriptions per school (kcal/g) and the average weight in grams of fruits and vegetables to compare these parameters with data already available from schools in the intervention group. Results: None of the schools offers healthy food. When comparing the control group with refreshments intervention group no significant differences in the content of fruit were found, but the energy density of the schools in the intervention group was modestly lower than the control group ( P = 0.01). Conclusion: Although the energy density of food was lower in the intervention group , overall not comply with recommendations . New preventive measures are urgently needed to improve the nutritional quality of the bars in the city of Cuenca and reverse the occurrence of overweight, obesity and chronic noncommunicable diseases.     Estudio comparativo de la prevalencia del embarazo no planificado en embarazos atendidos en el Hospital Vicente Corral Moscoso Cuenca - Ecuador 2013, en comparación al estudio de similares características realizado en el año 2006 en la misma institución (Galindo y Col)

    In order to compare the prevalence of unplanned pregnancy with a similar study realized in 2006, we did a comparative, analytical and descriptive study at the Vicente Corral Moscoso Hospital of the City of Cuenca - Ecuador, through the modality of surveys and informed consent of the respondents. The women surveyed were selected by random mode, based on the number of pregnancies attended in 2012, which was 6880, the sample of this study was obtained with a CI: 95% and an error of 4.47%, a sample of 395 women who did answer the validated survey. As results of our study, we obtained that 20% of the sample was adolescent and 58% were between ages 20 and 35years, regarding instruction only 2% had no formal education, while encouraging 60% overshoot basic education, reaching 14% to third level, whether ongoing or culminated. The predominant religion in the sample was the Catholic religion with 69%, also is important to say that in the sample the 77%, have their religion on ranges as important or very important, being this not a factor directly related to the unplanned pregnancies. The main risk factors, which we identified were: The adolescence is a predetermining factor to unplanned pregnancies, with a P <0.05 demonstrating a significant relationship, in the same way age of Sexual Active Life Start, becomes risk factor, women who started before the age of 18, are more exposed, with a statistical value of P <0.05. In the same area, we found in our study, a statistically significant relationship between the number of sexual partners reported by respondents and unplanned pregnancies, so those who have remained in their sex life more than two partners, are exposed to an increased risk of unplanned pregnancies, P <0.05. Compared to the previous study, we found a decrease in unplanned pregnancy 67.9% vs 58%, with similar socio-demographic characteristics. We also found that in our study only 36% didn´t used a contraceptive method, this is lower than the 88.9% that were found in the previous study. Finally found as a protective factor to make family planning, also found a significant relationship with P <0.05, which follows the logic of statistical a social analysis, of being a positive influential factor in preventing unplanned pregnancies.
