Item Eficacia del esquema hannallah vs armitage en cirugías de abdomen inferior en niños de los Hospitales Vicente Corral Moscoso y José Carrasco Arteaga 2013(2014) Torres Orellana, Mónica Andrea; Gárate Osorio, Germania Yolanda; Morocho Malla, Manuel IsmaelObjective: To determine the efficacy in controlling the trans and postoperative pain of the Hannallah and Armitage caudal block techniques. Materials and Methods: With a randomized controlled clinical design, two analgesic schemes of regional anesthesia were included. Group one consisted of sixty four participants and group two consisted of sixty two participants from one to eleven years old. Group 1: Hannallah blocking. The dose was calculated to 2mg per kg of 0,5 % bupivacaine and volume of 0.5ml, 0.75 or 1ml/kg to the desired dermatome. Group 2: Armitage blocking. The volume was used at 0.5, 0.75 or 1ml /kg according to the desired dermatome with 0,25 % bupivacaine in patients of the pediatric ward of Vicente Corral Moscoso and José Carrasco Arteaga Hospitals. Hemodynamic changes, analgesic efficacy in trans and postoperative, potential adverse effects, quality control pain for 2 and a half hours were measured. Results: The groups were comparable on the variables of age, sex, and weight. Both schemes have demonstrated effective control of pain in the trans and postoperative with (p of 0,50). ). Heart rate in the trans-surgical and the recovery remained in (p 0.63 and 0.94) respectively. Blood pressure did not show a significant difference with (p 0.74 and 0.85) in the trans and postoperative. With respect to side effects, the incidence was similar with (p 0.078). In regional anesthesia epidural space identification is a procedure that requires refined skills. Some devices have attempted to facilitate the approach, but the main difficulty has been the widespread use. Recently it has been reported the usefulness of a stopcock which placed in the drum of the Tuohy needle becomes a variation of the pendant drop. Objective. To determine the usefulness of a variant of the technique of hanging drop Gutierrez to identify the epidural space. Material and methods. With simple random sampling 155 patients scheduled for lower body surgery under epidural anesthesia were included . A variant of the hanging drop was used Gutierrez through a stopcock attached to the drum of the Tuohy needle 18G to identify the epidural space. Was considered as a sign of correct identification of the epidural space to absorption of anesthetic vertical branch of the device and the percentage of success was measured. Results. The age of the sample was 33.9 ± 14.6 years old (18-75 years). The weight was 33.9 ± 14.6 kg (50-98 kg) and height 1.59 ± 0.6 m. Normal weight was found in 40%, overweight in 39.4% and obesity in 20.6 %. Space was located correctly in 98.1% of cases with an error of 1.9%. Compared to 18% error reported with classical technique Gutiérrez the difference was highly significant (P = 0.0004) Discussion. The correct localization of the epidural space with a simple and easy device management is emerging as getting a fully valid alternative especially for anesthesiologist in training. KEYWORDS. The most representative aspects found in our study is the relationship of the TV with the behavior and development of children; the continuity of the practice of punishment and repression against the game; cultural and low transmission loss of traditional games in the schools studied; the high degree of socialization of children through the practice of sports; the short time spent by parents to share with their children increased their work hours and spend their free time on television, the belief by parents that the child needs toys sofistificados for recreation; the delegation of parental roles to older children in the care of younger siblings; a significant percentage of the absence of the father in the home was noted due to neglect or migration, framed in the socio-economic and cultural means families under study context
