Item Aceptación de las medidas de bioseguridad, higiene y restricción frente a la COVID-19 en familias que conviven con adultos mayores de 70 años o más, Cuenca 2020(Universidad de Cuenca, 2020-12-17) Torres Sánchez, Jhoan Gonzalo; Maldonado Aguilar, María Cristina; Freire Argudo, Ulises VicenteBackground: In March 2020, the WHO classified COVID-19 as a pandemic and proposed measures to contain this health problem and consequences. Objective: Describe the acceptance of biosafety, hygiene and social restriction measures against COVID-19, in families living with adults older than 70 years or more, Cuenca 2020. Methodology: It is a descriptive transversal study, the ICP-COVID-19 database, second round was used. For the data analysis and processing we use the SPSS 22.0 v program. The results are presented in tables of frequency and percentages in the qualitative variables and arithmetic mean, mode and standard deviation for the quantitative variables. Results: Highest female sex record with 56,5%, 70.3% belonged to the urban area and the frequency of a one older adult at home was 66.8%; 80.8% had no home moves. In relation to biosafety and hygiene measures they were over 90% except for the most complex ones. In mobility restriction 98,1% did it so except when they went to the market with 78.9%. The mean acceptance was 7.73 (SD = 2.083). Conclusions: The highest frequency of records was obtained from the urban area, most families live in houses or apartments with gardens; in relation to habits, biosecurity and hygiene measures has been adopted in a good way, the mobility restriction was satisfactorily fulfilled. For half of the registries it was difficult to adapt to the change.Item Análisis del grado de satisfacción de los adultos mayores de las provincias de Azuay y Cañar(2017) Ortiz Segarra, José Ignacio; Freire Argudo, Ulises Vicente; Álvarez Serrano, Marlene Elizabeth; Tenemaza Alvarracin, David Fernando; Cobos Torres, Angel Ignacio; Zhindón Galán, Martha Dalila; Larriva Vásquez, María EulaliaObjective. Analysis of the perceptions about the exercise of the rights of the elderly in the provinces of Azuay and Cañar. Methodology. An analytical cross-sectional study was carried out involving 538 elderly people living in the coverage areas of the first-level health centers, belonging to the Ministry of Public Health and the Rural Social Security, through the application of a rights form, previously validated.Results. The mean age was 75.20 ± 7.22 years (65 to 100 years), the majority were women, married, with primary education and 33% illiteracy. 81% of participants consider their standard of living to be adequate. Living alone is a factor associated with the inadequate exercise of rights in a statistically significant way (OR 2.31, 95% CI 1.46-3.68). Age, marital status, occupation, education, self-identification, are also associated with inadequate exercise of rights, but not significantly.Conclusions. Eight out of ten older adults consider that the exercise of their rights is adequate, and in the group that feels dissatisfaction, solitude is a statistically significant associated factor. To address this problem, it was necessary to interpret the perceptions of young adults, diagnose and work on preconceptive knowledge, attitudes and practices (CAP) through an educational proposal with appropriate methodologies to achieve the exercise of the right to preconception. Objective: To evaluate an educational program with a family approach to promote preconception control, based on the perceptions of young adults, UNCOVIA, Brother Miguel parish, Cuenca, 2015-2016. Materials and methods: Two designs were used, one qualitative one that was used to elaborate an educative proposal on preconception and another quasi-experimental that evaluated the preconceptive CAP. The research had three phases: socio-demographic and family characterization; Ethnographic study on preconception with design of the educational program; And, evaluation of the CAP level before and after. Results: Results: The ethnographic study revealed biopsychosocial aspects and protective, destructive and regulating factors around preconception, which were classified into four categories: macrosystem, mesosystem, microsystem and ontosystem. The average of knowledge and practices increased from 18.62% (95% CI: 15% -22%) to 80.27% (95% CI: 75-84%, X2 McNemar's 6,96. P 0,05), and 35.80% (95% CI: 35-36%) at 52.27% (95% CI: 52.02% -52.52%; X2 McNemar's 4,12. P 0,05) respectively. Conclusions: The application of the educational module on preconception, significantly improves the level of knowledge and practices in relation to the initial state. Una revisión sistemática(Universidad de Cuenca, 2022-02-15) Ordóñez Ruilova, Diego Mauricio; Freire Argudo, Ulises VicenteIntroduction: drug adherence is a multiple and complex phenomenon that constitutes a challenge, because not only basic skills are required, but also the adequate cognitive, family, and social functioning that surrounds the environment of the elderly. Objective: to analyze the associated factors with drug non-adherence in elders with type 2 diabetes mellitus. Method: a systematic review of observational studies published between 2016 and 2020 was carried out, in Spanish and English and structured in qualified search engines such as Pubmed, HONcode Search, Science Direct, and Wiley online library. To search for publications, Boolean operators were used: O, Y, NO (in Spanish) and OR, AND, NOT (in English); the research strategies were based on keywords. Results: 14 articles were analyzed, in which the prevalence of pharmacological non-adherence ranged from 6.5% to 72.7%. Associated factors with statistical significance were observed: age (p = 0.039), social support (p = 0.014), educational level (OR 66.1; CI 6.93-630.43; p <0.001), not being married (OR 1.95; 95% CI 1.04–3.51; p = 0.038), anxiety (p = 0.010), depression (p = 0.048) and emotional distress (OR 1.01; 95% CI 1.00-1.03; p = 0.013). Conclusions and implications: non-adherence to antidiabetic drugs is varied, it depends on sociodemographic, psychological, and family factors present in the elderly. The elderly living in the community seek alternative and complementary methods to improve their diet and quality of life. This study aimed to investigate the impact of an educational intervention on knowledge related to healthy eating among older adults. This intervention is rooted in the principles of meaningful learning and incorporates culturally adapted materials. A quasi-experimental study design was employed using a pre-test–post-test control group. Study participants were a total of 109 elderly (intervention: n = 51, control: n = 58) people in Cuenca, Ecuador. The educational intervention based on Ausubel’s theory of significant learning and Vygotsky’s sociocultural theory was programmed to be carried out for one session per week, over 24 weeks, with a duration of 120 min per session. The measures were the general characteristics of the study participants and knowledge about necessary amounts, food sources and the consequences of deficits or excesses in the consumption of macro- and micronutrients. Data were collected from August 2018 to February 2019. Statistically significant differences were observed between the intervention group (IG) and the control group (CG) in terms of knowledge about healthy eating for older adults following the educational intervention. The outcomes of this study strongly suggest the efficacy of the program in improving knowledge related to healthy eating among older adults. Healthcare providers should prioritize food education based on meaningful learning, utilizing culturally adapted materials for the elderly individuals residing within the community.Publication Limitación Funcional y Factores Asociados en Adultos Mayores de la Zona 6 de Salud en Ecuador(2016) Freire Argudo, Ulises Vicente; Álvarez Serrano, Marlene Elizabeth; Soliz Rivero, Gardenia; Tenemaza Alvarracín, David Fernando; Ortiz Segarra, José IgnacioOBJECTIVE: To estimate the prevalence of functional limitation (FL) and socio-demographic factors, health problems and chronic diseases associated to senior adults population from 6th health zone.METHODS: This is an observational cross-sectional research performed from June to December of 2015 at primary care units influence area from 6th health zone of Ecuador. 714 elderly people were included and randomly selected from 33 primary health care units.RESULTS: Average age was 76±8.31 years (min: 65; max: 102 years), 63.8% of them were female. Prevalence of functional limitation in BLA (Basic Life Activities) was 17.65% and 62.61% for IAL (Instrumental Activities of Life). Associated factors with FL in BLA were: age ˃ 75 years, illiteracy, cognitive impairment, depression, social problems and fall hazard. Age ˃ 75 years, illiteracy, cognitive impairment, depression and the risk of falling were found to be associated factors to limitations in IAL.CONCLUSIONS: The functional limitation in elderly adults is associated with aging and depends of several determinants. Water quality, physical-chemical and microbiological analysis was performed by 2 laboratories with 3 samples taken from: source, reservoir and address.- Results: According to users: The water board is a group of second grade (51.28%), performing management is inadequate (54.87%), water is not potable (92.53%), according to the analysis is inadequate for human consumption and poor water quality associated with gastrointestinal disease (93.59%), which according to statistics from SCS is between the second leading cause of morbidity and affects more children.- Conclusions: accessibility and continuity of service is inadequate water, poor water quality exists, which is associated by users with gastrointestinal disease, more common in children, the analysis performed by laboratories determined that the water is unfit for consumption human.Publication Parámetros dietéticos y alimentación habitual de adultos mayores. Population and methods: the descriptive study involved 102 elderly adults belonging to the senior clubs of the first level health centers. The students of the Community Cycle of the medical career applied a validated form, with questions of sociodemographic conditions, dietary parameters and eating habits, with prior training and signing of the informed consent. Results: the average age was 76 ± 7.20 years, between 65 and 95 years, the majority was, married and retired women with primary education, they were unemployed and with low socioeconomic status. Almost everyone eats breakfast, lunch and dinner, with a normal consistency. One in four AM ingests vitamin, mineral and herbal supplements. They choose foods by taste and lower cost. Eight in ten people cannot prepare their food. Six in ten people feed poorly, it does not matter if they like or dislike what they eat. Conclusions: the parameters and eating habits of the AM are related to aging and several determining factors. The knowledge will serve to propose healthy eating and prevent related diseases.Item Sobrecarga del cuidador de la persona con discapacidad y su relación con la funcionalidad familiar, Sinincay 2015-2016(Universidad de Cuenca, 2017-11-11) Vintimilla González, María José; Freire Argudo, Ulises VicenteObjective: to determine the frequency of overload of the caregiver of the person with disability and its relation with the familiar functionality and associated factors, in the parish of Sinincay. Methodology: a quantitative, analytical and transversal study was carried out with 190 caregivers of persons with disabilities belonging to the parish of Sinincay in the city of Cuenca between the years 2015 and 2016. In order to establish the demographic characteristics of the population and the level of Caregiver overload was used data collection questionnaires and the Zarit test and to determine the family functionality the FF.SIL test. The variables were expressed in percentages and in absolute frequencies. Factors associated with the overload of the caregiver of the person with disability were determined by Odds Ratio (OR) with their confidence interval and Chi square. Statistically significant results were considered at p <0.05. Results: the characteristics of the population showed that the caregivers were predominantly female: 87.4%, under 65: 76.3%, married: 62.1%, performed QQDD: 65.3%, with 45.8%, nuclear family: 64.2%, the predominant disability is physical: 52.1%, the overload of the caregiver found was 55.8%, of the caregivers who presented overload 75, 5% had family dysfunction (p: 0.001). A statistically significant association between the caregiver's overload and visual impairment was found in the bivariate analysis (RP: 0.159, Confidence Interval: Lower: 0.04, Upper: 0.157 (p < 0.002). Conclusions: In the Sinincay parish there is a high frequency of caregiver overload with a statistically significant association between caregiver overload and family dysfunction and visual impairment
