Abstract: | Clinical case thymus tumor
Introduction: Thymomas are epithelial neoplasms of the thymus gland, it is located behind the
sternum and plays an important role in the immune system before birth. Thymus tumors are rare
and represent 2% of mediastinal tumors. The gold standard in treatment is the surgical act;
Therefore, the objective is to describe the clinical case as a contribution to the scientific
community.
Case report: A 67-year-old female patient, with a history of type 2 diabetes mellitus and obesity,
productive cough of three months of evolution, went to the clinician for control, who requested a
CT scan and revealed a mass located in the anterior mediastinum. a biopsy is then performed,
confirming the diagnosis of thymoma.
Conclusions: Thymus tumors (thymoma) are rare pathologies, most of them are diagnosed as
findings, they are generally asymptomatic, difficult to identify, so high diagnostic suspicion is
needed. Its treatment is surgical after obtaining a biopsy for planning surgery.
Clinical case persistent omphalomesenteric duct in adults.
Introduction: When naming umbilical diseases, reference is made to multiple pathologies that
can affect the umbilical cord, they can be acquired or congenital. The most representative are
the persistence of the urachus and the omphalomesenteric duct, which are common in children
but rare in adults.
Case report: A 68-year-old male patient, from a rural area, a farmer, with a history of hypertension,
osteoarthritis with treatment, conventional prostatectomy more than 20 years ago, and umbilical
discharge since childhood. He went to the emergency room after a physician's evaluation, due to
the presence of “purulent” discharge at the umbilical level, pain, and erythema. He was admitted
to internal medicine with a diagnosis of cellulitis versus abscess. The surgery service was
consulted, which decided on surgical resolution with the following finding: entero-umbilical fistula
70 cm from the ileocecal valve due to persistent omphalomesenteric duct. On the eighth day of
hospitalization, he was discharged, with a favorable evolution, outpatient treatment and outpatient
control.
Conclusions: The persistence of the omphalomesenteric duct in adults is a rare entity. In general,
patients suffering from this condition are asymptomatic or report small amounts of umbilical
secretion that goes unnoticed. Its diagnosis is incidental, usually during imaging tests. and it is
more infrequent is that it presents complications in advanced ages. The definitive treatment when
diagnosed is surgical. |
Description: | Caso clínico tumor de timo
Introducción: Los timomas son neoplasias epiteliales de la glándula del timo, ésta se ubica detrás
del esternón y desempeña un papel importante en el sistema inmunitario antes del nacimiento.
Los tumores del timo son infrecuentes y representan el 2% de los tumores de mediastino. El Gold
estándar en el tratamiento es el acto quirúrgico; por lo que, el objetivo es describir el caso clínico
como aporte a la comunidad científica.
Caso clínico: Paciente femenina de 67 años, con antecedentes de diabetes mellitus tipo 2 y
obesidad, tos productiva de tres meses de evolución, acude a control con el médico clínico, quien
solicita una TAC y se evidencia una masa localizada en el mediastino anterior, luego se realiza
una biopsia, lo cual confirma el diagnóstico de timoma.
Conclusiones: Los tumores de Timo (timoma), son patologías infrecuentes, en su mayoría son
diagnosticadas como hallazgos por lo general son asintomáticos, de difícil identificación por lo
que se necesita alta sospecha diagnostica. Su tratamiento es quirúrgico previa obtención de
biopsia para planificación de cirugía.
Caso clínico persistencia conducto onfalomesenterico en adultos
Introducción: Al nombrar enfermedades umbilicales se hace referencia a múltiples patologías
que pueden afectar al cordón umbilical, pueden ser adquiridas o congénitas. Las más
representativas son la persistencia del uraco y del conducto onfalomesentérico, que son
comunes en niños pero raras en adultos.
Caso clínico: Paciente masculino, de 68 años, procedente de la zona rural, agricultor, con
antecedentes de hipertensión, artrosis con tratamiento, prostatectomía convencional hace más
de 20 años y secreción umbilical desde la niñez . Acude a emergencia posterior a valoración de
facultativo, por presencia de secreción “purulenta” a nivel umbilical, dolor y eritema. Ingresa a
medicina interna con diagnóstico de celulitis versus absceso. Se interconsulta al servicio de
cirugía que decide resolución quirúrgica con el siguiente hallazgo: fistula entero umbilical a 70 cm
de la válvula ileocecal por persistencia de conducto onfalomesentérico. Al octavo día de
hospitalización es dado de alta, con evolución favorable, tratamiento ambulatorio y control por
consulta externa.
Conclusiones: La persisten del conducto onfalomesentérico en adultos es una entidad rara, por
los general pacientes que sufren esta condición son asintomáticos o refieren secreción umbilical
en escasa cantidad que pasa desapercibido, su diagnóstico es incidental, por lo común durante
la realización de exámenes de imagen y es más infrecuente es que presente complicaciones en
edades avanzadas. El tratamiento definitivo al ser diagnosticado es quirúrgico. |
