Abstract: | Acute appendicitis is the most frequent surgical pathology in our environment; its
annual mortality rate is 2.4 / 1000 appendectomies. The diagnosis is mainly
clinical, however, risk stratification based on scores can optimize the
management of these patients.
Objective: Determine the validity of the inflammatory response scale in acute
appendicitis (AIR) in patients aged 16 to 35 years. Vicente Corral Moscoso
Hospital. Cuenca 2020.
Methodology: This is a diagnostic test validation study; The sample considered
consisted of 177 medical records of patients who met the inclusion criteria,
selected by simple random sampling. For data collection, a questionnaire
validated by the author was created. The SPSS v 24 program was used to obtain
the measures of central tendency and dispersion, Sensitivity (S), Specificity (E),
Positive and negative predictive value (PPV and NPV), ROC curve.
Results: the average age of the participants was 25.11 (SD ± 5.3), and the most
representative group was young adults from 25 to 34 years old; in relation to sex,
there was no significant difference. 10.88% of the patients who had an AIR score
≥ 8 were positive for acute appendicitis on histopathological analysis. The S of
AIR ≥ 8 was 10%, the E = 100%, the PPV = 100%; NPV = 18.63%. The AIR test
efficiency ≥ 8 was = 25.99%. The probability of having a negative result (ARI <8)
was 89% (negative likelihood index).
Conclusion: the validity of the AIR test ≥ 8 to determine the probability that an
individual has acute appendicitis was low, therefore, this value is insufficient,
therefore, the help of complementary tests is suggested. |
Description: | Antecedentes: La apendicitis aguda es la patología quirúrgica más frecuente en
nuestro medio; su tasa de mortalidad anual es de 2,4/ 1000 apendicetomías. El
diagnóstico principalmente es clínico, sin embargo, la estratificación del riesgo
basada en puntajes puede optimizar el manejo de estos pacientes.
Objetivo: Determinar la validez de la escala de respuesta inflamatoria en
apendicitis aguda (AIR) en pacientes de 16 a 35 años. Hospital Vicente Corral
Moscoso. Cuenca 2020.
Metodología: Se trata de un estudio de validación de pruebas diagnósticas, Para
el estudio se consideraron 177 historias clínicas de pacientes que cumplieron los
criterios inclusión, seleccionados por muestreo aleatorio simple. Para la
recolección de los datos, se creó un cuestionario validado por el autor. El
programa SPSS v 24 fue utilizado para obtener las medidas de tendencia central
y dispersión, Sensibilidad (S), Especificidad (E), Valor predictivo positivo y
negativo (VPP y VPN), Razón de verosimilitud positiva y negativa, curva ROC.
Resultados: La edad promedio de los participantes fue 25,11 (DS ±5,3), y los
grupos más representativos fueron los adultos jóvenes de 25 a 34 años; en
relación al sexo, no hubo una diferencia significativa. El 10,88% de los pacientes
que tenían un puntaje de AIR ≥ 8 resultaron positivos para apendicitis aguda al
análisis histopatológico. La S de AIR ≥ de 8 fue del 10%, la E = 100 %, el VPP =
100%; VPN = 18,63%. La eficiencia del test AIR ≥ de 8 fue = 25,99%. La
probabilidad de tener un resultado negativo (IAR < 8) fue del 89% (Índice
verosimilitud negativo).
Conclusión: La validez del test de AIR AIR ≥ de 8 para determinar la
probabilidad de que un individuo tenga apendicitis aguda fue baja, por lo cual,
este valor es insuficiente, se sugiere por ello, la ayuda de exámenes
complementarios. |
