| dc.description | Antecedentes: La hipocalcemia es una complicación frecuente después de la cirugía de tiroides y puede ser sintomática o asintomática. Muchos factores se han relacionado con la hipocalcemia sintomática, tales como: la paratohormona, la edad, el tiempo operatorio y el tipo de cirugía. Objetivo: Determinar la correlación de la paratohormona con la hipocalcemia sintomática en el postoperatorio precoz en pacientes sometidos a tiroidectomía total en el periodo 2017- 2018 en el Hospital José Carrasco Arteaga. Método y materiales: Estudio analítico de los niveles de paratohormona a la 1 y 6 horas postoperatorias, la edad, el tiempo operatorio, el tipo de cirugía y la citología preoperatoria con las manifestaciones clínicas de hipocalcemia. La muestra estuvo conformada por 171 pacientes operados de tiroidectomía total con o sin vaciamiento ganglionar nivel VI de cualquier etiología del Hospital José Carrasco Arteaga en el periodo 2017- 2018. Resultados: La prevalencia de hipocalcemia sintomática fue del 43.8%. Se encontró asociación significativa de las manifestaciones clínicas de hipocalcemia con los niveles bajos de paratohormona a la 1 y 6 horas post cirugía, un tiempo operatorio mayor a 120 minutos y la tiroidectomía total con vaciamiento ganglionar nivel VI (p<0.05). Por otra parte, el grupo mayor de 50 años (p 0.711) y la patología maligna (p 0.751), no están significativamente relacionados a la presencia de sintomatología de hipocalcemia. Conclusiones: Los niveles de paratohormona, tiempo operatorio y tipo de cirugía se relacionan con la hipocalcemia sintomática. La medición de la paratohormona a la 1 hora post cirugía tiene el mismo valor pronóstico que la obtenida a las 6 horas para predecir el desarrollo de sintomatología. | es_ES |
| dc.description.abstract | Background: Hypocalcemia is a frequent complication after thyroid surgery and may be symptomatic or asymptomatic. Many factors have been related to symptomatic hypocalcemia, such as: parathyroid hormone, age, surgery time and type of surgery. Objective: To determine the correlation of parathyroid hormone with symptomatic hypocalcemia in the early postoperative period of patients with a total thyroidectomy between 2017 and 2018 at José Carrasco Arteaga Hospital. Method and materials: Analytical study of paratohormone levels after 1 hour and 6 hours post-surgery, age, surgery time, type of surgery, and pre-operatory cytology with clinical manifestations of hypocalcemia. The sample consisted of 171 patients who underwent a total thyroidectomy with or without level VI lymph node emptying of any etiology at José Carrasco Arteaga Hospital in the period 2017-2018. Results: The prevalence of symptomatic hypocalcemia was 43.8%. A significant association of the clinical manifestations of hypocalcemia was found with low levels of parathyroid hormone at 1 and 6 hours post-surgery, a surgery time longer than 120 minutes and total thyroidectomy with a level VI lymph node emptying (p <0.05). On the other hand, the group older than 50 years (p 0.711) and the malignant pathology (p 0.751) are not significantly related to the presence of hipocalcemia symptomatology. Conclusions: Paratohormone levels, operative time and type of surgery are related to symptomatic hypocalcemia.
