Objective: To determine the prevalence of the cleft lip and palate (CLAP) in the main cities of Ecuador: Quito, Guayaquil and Cuenca, in the period from 2010 to 2018. Methods: A descriptive, observational, cross-sectional and retrospective study was carried out by reviewing the database of studies conducted in hospital settings in the cities of Quito, Guayaquil and Cuenca of patients diagnosed with FLAP born in the period from 2010 to 2018. The information was collected using a form for the live birth with syndromic, non-syndromic and mother CLAP. Results: The frequency of CLAP was found to be 14.97 per 10,000 live births, consistent with that of the countries in the region. 1132 patients diagnosed with CLAP were treated in the national health system according to hospital records, in the period 2010 to 2018; Regarding the average age of children who required CLAP care, it was 3 years (SD: ± 3.2). Their sex ratio was 38.19% women vs. 61.68% men Conclusion: No studies have been found in Ecuador that have unified criteria on diagnosis and classification of CLAPs that help formulate public health policies that improve the quality of life of the cracked patient

Condylar hyperplasia (CH) is a generic term for a pathological situation that is used to describe conditions that cause excessive growth and overdevelopment of the mandibular condyle and also impacting on the jaw, this bone formation is responsible about of 50 % of all deformities facial and it occurs most frequently between 11-30 years old. This case report is about a 21-years old female who showed unilateral condylar hyperplasia of left side with alveolodentary compensatory. Was carried out high condilectomy through an endoaural approach removing 5 mm of the upper condyle and the mandibular osteotomy was performed through intraoral approach with dissection and preservation of the inferior alveolar nerve, removing 8 mm of the inferior aspect of the mandible according to the aesthetic requirements. An appropriate treatment for the condylar hyperplasia focused on correcting the side effects such as functional or aesthetic it’s of great benefit for the patient because it allows improve their quality of life. A correct diagnosis is very important to planning a successful treatment.

Medición en tomografías de estructuras óseas craneales para determinar el dismorfismo sexual en la población andina. Parte I
(2022) Ochoa Rivera, Vanessa Estefania; Villavicencio Martinez, Henry Daniel; Cazar Almache, Marcelo Enrique; Villavicencio Caparó, Ebingen
Determine through digital tools the sexual dysmorphism in the Andean population of the south of Ecuador. Methods: The surfaces of the foramen magnum were measured in 385 Cone Beam computed tomography (CBCT) with a full window size of the cranial facial mass. The surface of the foramen magnum was measured in axial sections, obtaining length and width values by three observers, the surface of the great hole was calculated through the formulas established by Routal and Teixeira. Results: The measurement of the foramen magnum has a sensitivity close to 70% to determine the sexual dysmorphism of the skulls analyzed. Conclusion: This study validates the digital measurement of the foramen magnum surface obtained from CBCT being a tool to be used in the definition of sex through the cranial bones. It is definitive if the values obtained are in high o low numbers, but you will need a complementary measurement with another structure when intermediate values are present.

Análisis estético dentofacial: base de la terapéutica en deformidades faciales
(2018) Cazar Almache, Marcelo Enrique; Abril Cordero, Luz Marina; Recendez Santillán, Norith De jesús
The analysis and diagnosis of dentofacial alterations is the basis of the surgical orthodontic treatment of facial skullmass, facial deformities from a functional approach to a esthetics through clinical and technological tools such as imaging studies: cephalometric, cone beam tomography and stereolithographic models. The study of the facial aesthetics of the human being has been developed since antiquity based on the classic canons, which have marked as standard in the collective subconscious, the greek standards as the standard norm of facial beauty and that do not always coincide with all ethnic groups as in the case of Ecuadorian society. The present work intends to give to the student of pre and post of our means a methodology to approach the complexity of the aesthetic analysis dento facial coupling the classic interpretation to our multiculturalism.

Rotación del cuerpo adiposo de la mejilla para aumento de la proyección labial: reporte de caso
(2016) Cazar Almache, Marcelo Enrique; Palacios Vivar, Diego Esteban; Miranda Villasana, José Ernesto
The treatment of patients with skeletal abnormalities or deformities begins with planning based on facial analysis and cephalometric tracing. Often the values obtained from that analysis reveal abnormal skeletal conditions or insuffi cient soft tissue. Autologous fat grafts and using the buccal fat pad provide viable options to correct these defects and achieve a favorable aesthetic and functional result. The tissue characteristics of the buccal fat pad make it a structure that has good adaptability and a low rate of infection and rejection. It is often overlooked and simply resected to achieve more defi ned cheekbones and facial contours. It has multiple therapeutic uses and should always be considered an option in reconstructive procedures. We present the case of a 17-year-old female patient diagnosed with a dentofacial deformity, soft tissue defi ciency in the lip and perinasal region, and a failed orthodontic camoufl age treatment. Orthognathic and rhinoplasty surgery were performed to correct the hard-tissue defect, and the buccal fat pad rotated and advanced to enhance soft-tissue projection.

Retención dentaria múltiple: reporte de un caso y revisión de literatura.
(2021) Saenz Delgado, Johanna Monserrath; Cazar Almache, Marcelo Enrique; Proaño Benenaula, Paula Bernarda
Multiple dental retention is a pathology that presents several dental organs located inside the alveolar processes of both the maxilla and the mandible. Taking into consideration that this pathology may be related to local or systemic factors that condition its presence and may be accompanied by additional pathologies, such as, in this case, dentigerous cysts. Next, the case of a 20-year-old male patient with 13 retained teeth is reported, who has a systemic disease not related to said pathology. The objective is to inform and encourage a correct anamnesis, detailed clinical and radiological analysis, which allow us to reach a correct diagnosis and a good treatment plan; in order to generate a basis for future research

Tratamiento de quiste óseo traumático mandibular de grandes dimensiones. Revisión de la literatura y reporte de un caso / Treatment of a Large Mandibular Traumatic Bone Cyst: Literature Review and Case Report
(2012) Cazar Almache, Marcelo Enrique; Miranda Villasana, José Ernesto; Brau Figueroa, Adrián; Recendez Santillán, Norith de Jesús
Background: The traumatic or simple bone cyst is frequently found in the maxillofacial area mostly in the posterior body of the mandible; its average diameter is 1-3 cm and the usual treatment is bone curettage. Method: A review of literature and a case of a large mandibular traumatic bone cyst are presented. An alternative therapy to treat large pseudocysts is proposed. Results: A 19-year-old female patient with diagnosis of a large simple bone cyst (diameter: 10 cm) was treated at the Regional “Vicente Corral Moscoso” Hospital that is affiliated to the Ministry of Health of Ecuador. The treatment consisted of intracavitary curettage and autologous iliac crest bone graft, achieving a fast and adequate bone mandible reconstruction.

The maxillary sinus foreign body is a frequent consequence of accidents in dental treatments. These unfortunate events are due to the close relationship of the root tips of posterior teeth relative to the floor of the maxillary sinus. Extruded endodontic materials can act essentially as foreign bodies and potentially can lead to various pathophysiological responses. Not always produce reaction, in some cases are asymptomatic, and kept only under observation. The microorganisms isolated in patients with sinusitis of odontogenic origin differ from non-odontogenic, its predominant by anaerobic bacteria. The present case refers to accidental displacement of gutta-percha cones to the maxillary sinus; together with the presence of communication oroantral allow entry and colonization of Escherichia coli, an aggressive and rare bacteria in this location. For therapeutic treatment we did an approach through the oroantral communication, and prescribe the sultamicilin as the antibiotic.

Tercer molar ectópico en seno maxilar: reporte de un caso
(2017) Torres Cruz, Yonatan Josué; Cazar Almache, Marcelo Enrique; Miranda Villasana, José Ernesto; Palacios Vivar, Diego Esteban
Ectopic tooth are dental organs found in a uncomon regions. There are publication that report ectopic tooth in the maxillofacial region being common sites palate, paraorbitaria region, nasal cavity, maxillary sinus, chin, coronides process and condyle. Cases have also been reported in non-dental areas such as ovary, anterior mediastinum region usually related to a teratoma. Ectopic tooth are often radiographic fi ndings because they are generally asymptomatic. Its therapeutic when found in the maxillary sinus could be conservative under periodic clinical-radiographic vigilance. Removal of these dental organs is necessary for their tendency to form cysts, develop odontogenic tumors and trigger neuropathic disorders. Surgical options are endoscopic, transnasal, Caldwell Luc or quadrangular approach. We present a case of a female of 32 years old with a diagnosis of ectopic third molar in maxillary antrum
