    Características de infecciones en fracturas expuestas en el Hospital José Carrasco Arteaga el periodo 2014-2016.

    Background: Exposed fractures are considered an orthopedic emergency because of the complications they may present, surgical wound infection is the most frequent. Infections can evolve and have consequences as severe as amputation of the limb, which results in permanent impairment of the patient's quality of life General Objective: To determine the frequency and characteristics of infections in exposed fractures in patients attended in the emergency area of the Hospital José Carrasco Arteaga in the period 2014 - 2016. Methodology: A cross-sectional descriptive study was developed, using the computerized consultation system of the José Carrasco Arteaga Hospital. 315 clinical histories of patients who suffered exposed fractures were reviewed. The data were analyzed in the program SPSS Version 15.0 and the results are presented in the corresponding tables. RESULTS: Patients with infections in exposed fractures were present in 40.4% of the cases, the age with the highest percentage of infections was between 20 and 30 years with 43% and the sex with the highest percentaje was male with 63.3% Conclusion: There is a high frequency of infections in exposed fractures, which occurred in young adults and in the male sex, in fractures exposed with grade I of Gustillo, in lesions where the mechanism of fracture was direct, the location of the most common fracture was in the tibia, in patients who were hospitalized for 1 to 5 days, who had surgery and the fixation used was the internal one.

    Conocimientos, actitudes y prácticas de los entrenadores y monitores de la Federación Deportiva del Azuay en el método PRICE como tratamiento primario de lesiones deportivas. Cuenca. 2018

    Background: to treat an acute injury protocols have been implemented that have evolved over time, these protocols are: ICE, RICE and PRICE, the latter being the most used treatment today. Objective: to determine the knowledge, attitudes and practices of the coaches and monitors of the Sports Federation of Azuay in the PRICE method as primary treatment of sports injuries, in the city of Cuenca, 2018. Methodology: observational, descriptive and cross-sectional study of knowledge, attitudes and practices, conducted to all coaches and instructors of the Sports Federation of Azuay (FDA), in the second semester of 2018. The pilot test of the designed test was executed by the researchers, and then the respective surveys were conducted. A database was created in SPSS version 24, for its analysis and interpretation. Prior to the data collection, a consent was signed informing that the name of the interviewees and their coding will be kept anonymous. Results: of the study carried out in the 80 participants, among coaches and monitors, it was determined that 53.8% have knowledge of the PRICE method, 51.3% put into practice said knowledge and 88.75% show a positive attitude to learn and apply the PRICE method. It was determined that the coaches, with extensive experience, are the ones who most practice the procedures to treat sports injuries; It can be seen that foreign coaches are more prepared. The anatomical site that presents the highest frequency of injuries is the ankle, with 28.8%

    Conocimientos, actitudes y prácticas relacionados con la prescripción de ejercicio físico por parte de médicos que laboran en el Ministerio de Salud Pública en el cantón Cuenca. Distritos 01D01 y 01D02. 2018

    INTRODUCTION. 6% of deaths worldwide have physical inactivity as the main risk factor. Physical exercise generates multiple health benefits, so increasing your practice represents an individual and social need that would be achieved through its proper prescription. OBJETIVE. To determine the level of knowledge, attitudes and practices related to the prescription of physical exercise by physicians working in the Ministry of Public Health in the Cuenca Canton Districts 01D01 and 01D02. METHODOLOGY. This is a transversal descriptive research, whose universe is constituted by the 397 doctors who work in Health Districts 01D01 and 01D02 of Cuenca Canton. The sample is made up of 195 doctors, generals and specialists. The method is observational, a survey will be applied through a form, which was taken from the Agita Sao Paulo Manual. The data will be tabulated and analyzed by using the SPSS 18.0 program and presented in simple tables made in Excel 2010. For the analysis of the data, descriptive statistics will be used: frequency and percentages. RESULTS. 88.72% of the doctors surveyed worked in type A, B and C health centers. 55.38% were general practitioners and 44.62% specialized. Doctors have a general knowledge about the prescription of physical exercise, while in specific aspects (organization, type by population group and intensity) their knowledge is erroneous. His attitude towards prescription is positive. 91.28% of doctors do prescribe physical exercise, most do it in less than 5 minutes of the consultation time.

    Determinación de la condición física mediante la prueba de caminata de 2km. de la UKK en los estudiantes de décimo ciclo de medicina de la Universidad de Cuenca, periodo lectivo septiembre-enero, Cuenca-Ecuador 2014.

    Background: Determining the physical condition today is an ideal place to expose the health situation, this value can also be used as a predictor of cardiovascular damage. Objective: To determine the physical condition by walking test of the UKK 2km students in tenth cycle of medical school at the University of Cuenca. Methodology: A descriptive cross-sectional quantitative study was conducted. The universe included the 319 students in the tenth cycle of medical school with a sample of 124 people. The 2km walk test (UKK) and physical activity IPAQ survey was used in data collection. The data were processed by appropriate statistical tests using Microsoft Excel 2010 program. Results: 83.1% of students have an average physical condition and below it, similar to 96% of the evaluated male figure; unlike 77.7% of the female population they have a physical condition within the average and some above it. According to Ipaq test 26.6% of students perform low physical activity, 36.3% moderate physical activity and 37.1% high physical activity, finally the masculine gender refers more physical activity.

    Determinación del consumo máximo de oxígeno mediante la aplicación del Test de Course Navette en estudiantes de la carrera de Medicina de la Universidad de Cuenca, período marzo-agosto 2023

    Background: maximal oxygen consumption (VO2max) is a parameter which determine the aerobic physical condition of people, this being an indicator of cardiovascular health, which in good conditions improves academic performance, increases concentration and reduces risk of suffering chronic non-communicable diseases. Objective: determine the maximal oxygen consumption by applying the Course Navette Test in students of the University of Cuenca's Medical School during the March-August 2023 academic year. Methods: a cross-sectional, descriptive, quantitative, study was carried out on 271 students, applying a survey created by the authors to record pre- and post-test data; The Course Navette Test was used to obtain VO2max; The information was processed with SPSS 26, Minitab Express 1.6, Microsoft Word 2018; Frequencies, percentages, Mean and Standard Deviation were used as statistics, all expressed in tables. Results: VO2max was higher in men and in 18-year-old students, with an average of 40.82 and 38.24 respectively; overweight students averaged 35.4; Obesity Class I, 32.28 and Obesity Class II, 31.3. VO2max was better in students with a pre- test HR of 74.43 and a 5-minute post-test HR of 114.87. Conclusions: most students had a regular VO2max; However, as a young and apparently healthy population, VO2max values should be higher than those found in the present research.

    Frecuencia y causas de conversión de colecistectomía laparóscopica en el Hospital Cantonal de Paute.2014

    Introduction: The conversion of laparoscopic cholecystectomy to conventional surgery occurs for different reasons which represent an increase in the hospital stay and morbidity (14). The conversion rate worldwide is 7-12% (20). Objective: To determine the frequency of laparoscopic conversion and the most frequent causes in patients with laparoscopic cholecystectomy at Hospital Paute, 2014. Materials and Methods: It is a descriptive study. The data analysis was performed using SPSS 22 program and it is presented in frequency tables, percentage, value of P = <0.05 Odd Ratio (OR) with confidence interval (CI) of 95%. Results: The prevalence of conversion is 19.5% with an average age of 39.54 +/- standard deviation (SD) of 16,297; by age group: 77.8%, it is between 20-60 years. The 16.1% are female and 3.4% male. The preoperative diagnosis was cholelithiasis (57.9%) and acute cholecystitis (31%). The postoperative diagnosis was cholelithiasis (51.7%) and acute cholecystitis (32.2%). The most frequent causes of conversion: 5.0% severe inflammation; adhesions 4.2% and anatomical identification difficulty 3.4%. The longer operating time of 60 minutes was 43.3%. The 37.9% was for females and 5.4% for males. With an OR = 1.042, it means that more than 60 minutes in females´ surgical time, has 1,042 times greater risk of conversion that patients with less surgical time of 60 minutes, but the p-value is not statistically significant.

    Prevalencia de las lesiones y factores asociados en deprtistas de 13-17 años de la Federación Deportiva del Cañar 2014

    Background: Inresearchin medicine there is a significant sector of the population that deserves to be studied as are athletes. In the province of Cañar don’t have knowledge of the factors associated with the prevalence of injuries Objective: Establishing the prevalence of injuries and associated factors inathletes13-17years of the Sports Federation of Canar. Method and Materials: It is across-sectional study of prevalence in the Sports Federation of Cañar, in all disciplines. The information was obtained through surveys when ther prior informed consent. The information was summarized and tabulatedusingMicrosoftExcel2010 and SPSS 15.0, containing basal tables and a table of results. Result: The 54.9% of athletes were injured; 29.7% of injuries are 1 6 years old; 63.5% of injuries were male; 33.89% of the lesions are of moderate severity; 75.4% of injuries occurred in training; 15.2% of injuries occurred in Judo, ankle sprains accounted for47.4% and the most frequent; practice contact sport did not result as a risk factor(p =0.000000) whereas longer training them selves (OR =2.73; 95% CI =2.17-3.43)). Findings: Males are injured more than women, age more injuries is16 years, the most likely tube injured anatomical site is the ankle, the most frequent type of injury is as prain, sports discipline with Judo is more lesions, most lesions are of moderate severity and injury risk factor is the longer training for more exposure.

    Relación entre hallazgos ecográficos preoperatorios y quirúrgicos, con la dificultad de la colecistectomía laparoscópica. Hospital Homero Castanier Crespo-Azogues.2014

    Introduction: Abdominal ultrasound is the main diagnostic aid in the vesicular gallstones. The laparoscopic surgery is the surgical procedure of choice. Technical difficulties involve risks of intra- and postcholecystectomy complications. Objective: To determine the relationship of sonographic classification and surgical findings with technical problems for the prediction of difficult laparoscopic cholecystectomy and risk of perioperative complications. Materials and Methods: An observational, analytical, cross-sectional study was made in patients who were submitted to a laparoscopic surgery from June to September 2014, in the Homero Castanier Crespo Hospital in Azogues city by comparing proportions with p <0.05 as significant. Results: A total of 82 women (76.6%) and 25 men (23.4%) were studied. Their average age was 39.9 years with a SD of 14.74. Ultrasound diagnostic agreement and intraoperative findings was 82.2%. The adhesions to the gallbladder (77.6%), dissection of Calot's triangle (44.9%), dissection of the gallbladder bed (39.3%), they were the major difficulties. In the Braghetto classification, the grades IIA, IIB and III had higher percentage of difficulty. Conclusions: It was demonstrated that the technical difficulty increases progressively in IIA, IIB and III types from the Braghetto classification.
