    Caso clínico: adulto con anomalía de Ebstein

    Introduction: Ebstein's anomaly is characterized by low implantation of the septal and posterior tricuspid leaflets; therefore, the right atrium is wide and its ventricle is small. Being a rather rare pathology in our environment, it is underdiagnosed and does not receive the appropriate treatment. Therefore, it is important to know it to avoid a delay in its clinical surgical management.Clinical case: The patient is 46-year-old male, with a history of congenital hearing loss, who presented dyspnea (functional class III) plus palpitations and atypical chest pain approximately 1 year ago. In the physical examination, he revealed a holosystolic murmur in the tricuspid focus and moderate use of accessory muscles. A transesophageal echocardiogram revealed atrial arrhythmia plus implantation of the tricuspid leaflets and it was started a clinical treatment plus flutter ablation. In the evolution he presented improvement of the symptoms and depending on the deterioration of the functional class, surgical treatment.Conclusions: in this case, clinical treatment and flutter ablation were adjusted according to the patient's symptoms, showing improvement

    As the game one of the most important activities for the psychomotor, social, emotional development of the child, must occupy a seat priviledged in our attitudes to children as this is what will promote integrated training that will allow cope with freedom in later life. Also we can not put aside the importance of toys starting from simple and traditional, which are transmitted from generation to generation, do we know our reality, history and join the family nucleus, and on the other hand we have the more technology, according to our era, providing new advances but somehow restricting the positive aspects of the above. That is why we have found that children give relevance to the game, making it the longest activity encompasses both ordinary days as weekends and holidays, noting their preference in practice in recreational green spaces both inside and outside home, the second option more applied. Despite this television also it occupies much time the child for multiple recreational programs are the most attractive, leaving aside the cultural type

    Reporte de uso de clavo TFN en fracturas proximales de fémur en pacientes del Hospital Vicente Corral Moscoso y Hospital del IESS, reporte de 4 casos

    OBJECTIVE: To report the results of proximal femur fractures treated with the trochanteric system titanium nail. Patients: at the Vicente Corral Moscoso Hospital, three cases of patients aged 16, 21 and 30 years old were treated; and another patient in the José Carrasco Arteaga Hospital who is 20 years old, the last one also presented a half-diaphyseal of femur and ipsilateral of tibia fracture. Three patients with subtrochanteric fracture (one with pseudoarthrosis of 8 months evolution), and a fourth patient with neck fracture and ipsilateral femoral of femur. The fracture was designed according to Russell-Taylor classification, and it was treated by open reduction and the T.F.N. system placement. The patients were maintained on radiograph control at 2, 6, 12, 24 weeks after surgery. RESULTS: The clinical and radiographic consolidation of the four cases was observed at 12 weeks after surgery, no significant postoperative complications arose in any case, there was no infection, and also the mobility was recovered successfully, although in one patient was observed a displacement. CONCLUSION: The surgical treatment with the TFN system is well indicated in subtrochanteric femur fractures, it is associated with good functional results, low morbidity and few complications, thus constituting an excellent therapeutic technique.
