Background: in Ecuador, cognitive impairment affects 16,3% of men and 25.7% of woman (1). There haven’t been studies of validation of the Spanish version of the MoCA in Cuenca. General Objective: Determine the diagnostic validation of the MoCA vs MMSE and evaluate cognitive impairment and dementia in the elderly in ¨Centro Municipal Diurno el Hogar de los Abuelos¨ and ¨Hogar Cristo Rey¨ in Cuenca, in 2019. Materials and Methods: an observational study, of the validation of diagnostics test between MoCA and MMSE in 93 elderly that go to these houses. ¨Centro Municipal Diurno el Hogar de los Abuelos¨ and ¨Hogar Cristo Rey¨. Results: Most frequent age category were elderly (27%), the most predominate sex being female (56%). Evaluating participants with MMSE, 35% of the results were normal, pathological suspicion (23%) and dementia (8%); meanwhile with MoCA we got pathologies in 85%. When comparing the joint results of both tests, 18% of the results where normal and 82% pathological, of this 82%, most where from woman (56%), elderly (30%) and senile (30%). The sensibility of MoCA was of 98%, the specificity 60%, VPP 75%, 59%, and the area under the curve was 65%. Conclusion: it was determined that MoCA has more sensibility and less specificity than MMSE, thus is it better determining diseases in sick people than MMSE, but it is not more useful detecting healthy people. It has a 65% probability in distinguishing the positives from the negatives.
