Item Adenomas hipofisarios: características sociodemográficas, clínicas y terapéuticas de 250 casos(2021) Barahona Ulloa, Wilson FabricioIntroduction and objective: Pituitary adenomas are present in 23% of the general population, being mainly small and asymptomatic. Despite the existence of global and regional data, in Ecuador there are no large-scale records on this pathology. The objective of this study is to descriptive diagnosed pituitary tumors ́socio-demography, clinical, and therapeutic features over a 5-years period in one of the most important specialty hospitals in the country. Materials and methods: An observational study with descriptive design was performed. Data from clinical records of patients with a definitive diagnosis of pituitary adenoma that have been treated from January 1, 2012 to December 31, 2016 were collected at the specialty hospital José Carrasco Arteaga. Results: There were 250 cases with pituitary adenomas, 179 women (71,6%), 126 (50,4%), adults. The most frequent diagnostic age was between 20 and 39 years. The microadenoma was observed in 164 times (65,6%). The clinical presentation was mostly symptomatic in 225 people (90%). Excess hormonal production was detected in 176 patients (70,4%). Prolactinoma was the most common subtype, 146 (58,4%). The most used treatment was pharmacological, in 200 cases (80%). Conclusions: The results are similar to the literature reviewed, although in this study the symptomatic presentation prevailed mostly over the incidental form and the percentage difference between female and male was greater, especially among non-functioning subtypes.Item Características de la acupuntura estandarizada como tratamiento de lumbalgía crónica en Cuenca 2015(2016) Jaramillo Monge, Kadir Paúl; Marquez Maldonado, Carol Estefanía; Achig Balarezo, David Ricardo; Sacoto Molina, Adrian MarceloBackground. - Chronic low back pain is a common cause of medical consultation and absenteeism; there are several pharmacological and non-pharmacological measures to relieve it, where acupuncture, art belonging to Chinese medicine, is a treatment option for improving pain. Objective. - To determine the characteristics of standardized acupuncture as a treatment for chronic low back pain in Cuenca. Methodology. - A descriptive study was conducted in an unknown population of patients with chronic back pain who were treated with standardized acupuncture. The sample was not probabilistic for convenience. Data were collected using a validated and analyzed in SPSS software v22.0 form. Pain intensity was assessed by visual analogue scale (VAS) and functional limitation by the Oswestry disability index (ODI). Results. – In total, 64 patients participated in this study. The age ranged between 20 and 82 years. 62.5% were women, 95.31% live in urban areas, 59.3% attended by postural causes. Patients were treated with acupuncture standardized reduced EVA (7.41 versus 2.77 in 3 months) comparing the before and after treatment. 54.7% reduction was obtained using the functional limitation ODI before and after treatment. There were statistically significant difference between patients who underwent the treatment and reduction of back pain disability by McNemar test (p <0.0001). Objective: To identify the characteristics of gestational trophoblastic disease in patients in the area of gynecology and obstetrics Hospital Vicente Corral Moscoso (HVCM) in the period between January 2010 and January 2015. Methodology: This was a descriptive study, whose universe consists of the medical records of 35382 patients treated at the gynecology and obstetrics HVCM. The sample was represented by 50 cases presenting gestational trophoblastic disease in the study period. Were collected data on a form previously designed and tested. Tabulation and analysis was performed using the same program Microsoft Excel and SPSS 15.0 software. Results: 50 cases were identified, of which 30% were found among the 21 to 30 years. Gestational age at diagnosis was between 9 to 13 weeks. 46% was found in the range multiparous. Most patients did not present precedent related conditions. Conclusions: Gestational trophoblastic disease is a disease that is present in 0.14% of patients treated in the area of gynecology and obstetrics and the conditions HVCM associated different one the child presented a global level.Item Evaluación de atención y memoria en sujetos drogodependientes.(Universidad de Cuenca, 2018-11) Fajardo Balbuca, María Fernanda; Carmilema Tapia, Juan Carlos; Sacoto Molina, Adrian MarceloThe exposure to psychoactive substances may predispose the individual to suffer alterations cognitive order. Attention and memory are the functions that are commonly affected. Objetive: The present study evaluated the levels of attention and memory in patients with use disorder by substance use in three specialized centers. Method: It is an exploratory-descriptive study with a 60 participants. The neuropsychological evaluation was performed with the Neuropsi battery for attention and memory. The analysis was carried out using descriptive statistics with frequencies and percentages. Results: In this case was observed that the researched population showed a greater deterioration in the area of memory with a 70% of alteration, while in the attention was evidenced 33.3% of deficit. Working memory is preserved in most of the population. Conclusions: The results indicate that cognitive deterioration in attention and memory levels are present in subjects with substance use disorder being the memory the most affected function. There was not a considerable relationship between the time of consumption and cognitive damage.Item Framingham versus PROCAM para establecer el riesgo cardiovascular. ¿Existe diferencia?(2020) Auquilla Pauta, Katherine Estefanía; Cabrera Capelo, Adriana Jackeline; Sacoto Molina, Adrian MarceloIntroduction: cardiovascular diseases are the main cause of death worldwide, there are scales that allow determining the risk of suffering, hence the importance of studying their behavior in our population.Objective: to determine the concordance of the Framingham and PROCAM cardiovascular risk scales.Materials and methods: an analytical study was carried out with 500 patients between the ages of 35 and 65 who met the inclusion criteria, the data was taken from the clinical records of the outpatient clinic of Internal Medicine of the Hospital José Carrasco Arteaga, center third-level care, in the period January-December 2016. The agreement was established through the Cohen kappa index and the Bland and Altman method.Results: the kappa index calculated between the Framingham and PROCAM cardiovascular risk scales is 0.5246; (p <0.0001).Conclusion: the agreement found between Framingham and PROCAM is moderate. A case of a 23-year-old woman who was diagnosed with severe heart failure is presented. During her hospitalization, there was evidence of symptoms associated with hypocalcemia, secondary to hypoparathyroidism, so the patient was treated with replacement of calcium immediately. Objective: To determine the prevalence of neuropsychiatric RAM type inpatient pediatric services, clinic, surgery and obstetrics and gynecology of Vicente Corral Moscoso Hospital (HVCM) of the city of Cuenca. Method and Materials: A descriptive study was conducted from January 2014 to complete the sample. The sample was 336 and conglomerates. Objective: To determine the prevalence of condom use among sex workers attended in the Health Area 4 "Carlos Elizalde". Yanuncay, Cuenca. Methodology: Transversal descriptive studio, the sample with a total of 19 patients was taken by convenience, and the chosen were those patients undergoing mechanical ventilation that developed AVN of a total of 167 admitted ventilated. The data collection was carried out through the review of medical histories and their subsequent entry into the questionnaire. Results: The prevalence of pneumonia associated with mechanical ventilation is 11.38%, age with more frequency ranged 50 to 64 years. Comorbidities of patients the DMTII was that most frequent. The most frequent causal germ found was the Staphylococcuss Aureus. The days of hospital stay ranged from 11 to 30 days, Mortality represents a 26.3% corresponding to a total of 5 patients of the 19 who made AVN. The data may be used by students, medical staff, authorities of the Hospital J.C.A. Conclusions: The prevalence of AVN in the U.C.I. of the Hospital J.C.A is low, with a mortality of one-quarter of the total of patients with AVN, it is important to consider that there is a probability of developing especially when age and days of stay increase.
