    Características de la sobrevida de pacientes después de cinco años de haberse realizado prostatectomía radical. Hospital José Carrasco Arteaga. Cuenca, 2012

    Objective. Describe the characteristics of the patients after five years of having had radical prostatectomy for prostate cancer diagnosis in the Jose Carrasco Hospital of Cuenca city, with emphasis on the therapeutic response and the frequency of complications. Material and methods. With a descriptive data collected from the medical records of patients who underwent radical prostatectomy for prostate cancer from January 2002 to December 2006 and analyzed the findings on five years of control through the outer query. Results. Of the 264 prostatectomies performed in the Hospital José Carrasco in five years 32 of them were radical for diagnosis of prostatic carcinoma. The average age of the study population was 69,3 ± 8,7 years (49 - 86 years). 71,9% of patients had secondary education (18,8%) and higher (53,1%). The average hospital stay was 5,6 ± 3.27 days (2 - 22 days). 43,8% of patients were hospitalized within 5 to 6 days and 25% more than 7 days. Open prostatectomy (96,9%) was used more than laparoscopic prostatectomy (3,1%). Staging of carcinoma according to TNM classification most frequently reported for grades T1A, T1B, T3A and T3B. (N = 75%). 6.3% of the series had a staging of T4. According to Gleason, the average of 6.8 ± 1.62 says that most of the patients had carcinoma of intermediate aggressiveness. 18,75% had a Gleason score between 8 and 9. Early postoperative complications were the most frequent hemorrhage (15,6%) and urinary outflow obstruction (9.4%). Died before hospital discharge 3,1% of the prostatectomy (n = 1). The mortality associated with radical prostatectomy for this collection reached a rate of 6.3%. Conclusion. The results found in this collection are comparable with published literature. Radical prostatectomy supported by chemotherapy and radiotherapy appear to provide encouraging results for the management of prostate carcinoma.

    the prevalence of asymptomatic bacteriuria was studied in a geriatric population composed of 60 women ranging in age from 60 to 90 years, with an average age of 70.4 years. In 13 of the 60 people bacteriruria documented asymptomatic [21.7 percent]. Of the 23 cases from the rural area, [26.08 per cent] showed asymptomatic bacteriuria, and, of the 37 cases from urban areas, 7 [18.91 per cent] presented this box. In our series, an increase in cases with asymptomatic bacteriuria was not evidenced with increasing age. The sensitivity of the chemical and bacteriologic urinalysis, for asymptomatic bacteriuria is 100 percent, and specificity is 85.36 percent. In 7 cases [53.84 per cent] Escherichia coli was isolated in 5 cases [38.46 per cent] Enterobcter was isolated, and Klebsiella isolated in one case [7.69 percent]. A direct relationship between increased urinary pH and the manifestation of asymptomatic bacteriuria was found. Antibiotic sensitivity was ruled by quinolones represented by ciprofloxacin and norfloxacin for Escherichia coli and Enterobacter.     Factores de riesgo en la incontinencia urinaria femenina en mujeres de 18-70 años, Hospital José Carrasco Arteaga, Cuenca-Ecuador

    Urinary incontinence (UI) is a multifaceted problem that affects women and causes hygiene and social upheaval, which now become more important for the growing incorporation of the same into productive and economic work. Women are more susceptible to the elderly to suffer from incontinence which is explained in part by their anatomical feature, and partly because of the risk factors, it is estimated that 50de women experience urinary incontinence at some point in their lives and that 10of them suffer significant losses urine. A 70de people who have it do not see a specialist, and some take up to four years to do so. An estimated 10 to 12 million Americans suffer from urinary incontinence and almost all industrialized nations communicate prevalent rates of 8 to 15and 20 to 25se them qualify Advanced incontinence. In the Social Security hospital José Carrasco Arteaga IU studies have been few or have not released their results unknown problem situation. The aim of this study is to determine the risk factors; diabetes, BMI, parity, vulvovaginitis and estrogen deficiency. Check the relevance of subsequent specific studies. It is a case-control study conducted on a population of 152 female patients between 18-70 years of age attending outpatient urology department of José Carrasco Arteaga Hospital between December 2004 and February 2005. The presence of UTI defined by international standards. All patients a protocol was performed to confirm the presence 50 (cases) and / or 102 absence (controls) of IU. The UI is a common disease among women, are associated with constant doctor visits, hospitalization represents the complication which results in decreased labor activity and substantial health care costs. Disseminate the results of this study at all levels through the development of brochures on risk factors in the UI. Recommend further specific studies

    Prevalencia de complicaciones postoperatorias de cirugía con MALLAS TOT en mujeres con incontinencia urinaria de esfuerzo y factores de riesgo. Hospital José Carrasco Arteaga, Cuenca, 2009-2014

    Objective: To determine the prevalence of postoperative complications of surgery with TOT Sling in women with urinary incontinence. Material and methods. Information from the medical records of 283 women treated at the Urology Department of José Carrasco Hospital from 2009 to 2014. It describes the population collected by demographic variables: age, weight, height and BMI; obstetric history: pregnancies, births and cesarean sections and post stay. Prevalence and type of complications was calculated: dyspareunia, inguinal pain, fibrosis and externalizing mesh and sought association with BMI, obstetric history and accommodation. Results. The median age was 52 years (SD 9.23), weight 66 kg (SD 8.51), height 1.54 m (DE 0.06) and BMI of 27.93 (DE 3.84). The prevalence of complications was 27.2% of these: inguinal pain (19.9%), fibrosis (12.4%) were the most frecuent. The higher prevalence of complications was associated with obstetrical history of ≥ 4 pregnancies (OR 1.81 IC 95% CI 1.07 to 3.07) P = 0.027 and ≥ 4 births (OR 2.23 IC 95% CI 1.29 to 3.849) P = 0.004. Conclusion. There was a high prevalence of complications of the study, with similar results with other studies around the world. The risk factors for the presence of complications were age over 40, overweight or obesity, over 4 pregnancies and births.

    Tratamiento de urolitiasis mediante litotricia en el Hospital José Carrasco Arteaga, julio 2014 a junio 2015, Cuenca - Ecuador

    Objective: To determine the characteristics of urolithiasis treatment by lithotripsy in José Carrasco Arteaga Hospital, Cuenca - Ecuador, during the period of July 2014 to June 2015. Materials and Methods: retrospective descriptive study that was performed with the information from the database of patients treated with a diagnosis of urolithiasis and treated with lithotripsy in José Carrasco Arteaga Hospital in Cuenca 2014-2015. Once data was collected it was all entered in a database developed in Excel and then exported later to SPSS 21 system, for tabulation, processing and analysis. Results: The average age of the patients was at 45 years old with a standard deviation of 13 years; 64.9% of the cases were male; 64.9% of the population came from the province of Azuay. The procedure that was carried out the most was extracorporeal lithotripsy by shock wave (ESWL) with 38.5% of the cases (80 patients); followed by intracorporeal lithotripsy (LIC) plus ureteroscopy with 36.5% (76 patients), 24% (50 cases) presented on the population under ESWL plus LIC, finally 1% (2 cases) underwent LIC plus cystoscopy. The average size of litos was 12.42 mm with a standard deviation of 14.8 mm. The frequency of complications encountered was: pain 46.6%; hematuria 14.4%; incomplete fragmentation 37%; fever 3.4% and sepsis 0.5%. Conclusions: ESWL was the most commonly used procedure to treat urolithiasis with 38.5% of the cases; the combination with LIC was presented in 24%.
