    Clinicopathological and molecular markers in metastasizing ameloblastoma: a systematic review of case reports

    Ameloblastoma is a benign odontogenic tumour that is aggressive and localised in nature, listed as the first or second most prevalent odontogenic tumour and rarely tends to metastasis, but when it does, it receives the definition adopted by the WHO in 2017 of metastasizing ameloblastoma. Materials and Methods This systematic review of clinical case reports of metastasizing ameloblastoma from the last 10 years, collected from PubMed, Science Direct and Cochrane digital databases, aims to search for association between clinical/pathological and/or molecular parameters of ameloblastoma and its metastatic potential. Results The targeted search yielded 14 publications with a total of 18 clinical cases, which showed a mean age for diagnosis of metastasizing ameloblastoma of 46 years, with no gender predilection and a high probability of occurrence in the yellow Asian race, favouring a pattern of distant dissemination. The highest frequency of metastasis was associated with mandibular primary lesions diagnosed in young patients, and, the most frequently found variant was the multicystic solid type, follicular subtype; distant metastasis was the predominant form of presentation, with the lungs being the main target. Conclusions At the moment , there is nothing that can predict metastatic potential in ameloblastoma. More standardised studies exploring the molecular terrain are needed, as this is a key and understudied factor.

    Guía de práctica clínica para facilitar el diagnóstico presuntivo de los arcinomas odontogénicos malignos de mayor prevalencia en América Latina

    Introduction: ameloblastic carcinoma and primary intraosseous carcinoma are the most diagnosed malignant odontogenic tumors in Latin America. Objective: to provid the dentist with an algorithm that facilitates the presumptive diagnosthe ameloblastic and the primary intraosseous carcinoma and guiding clinicians and head and neck surgeons in making therapeutic decisions was the objective of this review. Materials and methods: bibliographic review: Results: a clinical practice guideline was developed, under the Agree II regulations, based on evidence published in the last 20 years and under the question: Is adjuvant radiotherapy a survival benefit for patients diagnosed with primary intraosseous and ameloblastic carcinoma compared to from chemotherapy? Adjuvant treatments would provide a benefit in the survival of ameloblastic carcinoma but not in the case of primary intraosseous carcinoma. Conclusions: there’s no standardized treatment for the tumors reviewed; the available evidence recommends early surgery with wide resection as the best approach in both tumors.

    Púrpura oral no trombocitopénica traumática exacerbada por condición metabólica

    Introduction: the term purpura clinically translates into flat or raised lesions of violet, purple or red- brown color determined by extravasation of blood in a localized region of the skin or mucosa. Objective: To present a clinical case of purpura located in the oral mucosa, and probably exacerbated by the patient’s metabolic condition. Methods: The description of the clinical case was made under the CARE declaration. Additionally, a review of the literature of the last 10 years on the subject was carried out through manual search in oral medicine texts and digital search in the scientific bases MEDLINE/PubMed, Cochrane, Scielo, ScienceDirect and Google Scholar. Case report: 51 year old woman, partially edentulous and medi- cated with metformin due to a diagnosis of type II diabetes mellitus, who recorded non-palpable red- violet lesions, negative vitro-pressure, compatible with petechiae and vibex, distributed on the floor of the mouth, ventral face and lateral surfaces of the tongue associated with chronic trauma. Laboratory tests pointed to a scenario of poor metabolic control. Conclusions: Traumatic non-thrombocytopenic purpura is diagnosed, probably exacerbated by a metabolic condition. A histopathological study is required to confirm.

    Tumores odontogénicos malignos prevalentes en América Latina: una guía clínica

    Seventy six percent of the malignant odontogenic tumors diagnosed in Latin America are carcino- mas, with ameloblastic carcinoma and primary intraosseous carcinoma being the most prevalent. Based on this background, this guide develops under the Agree II guideline, the definition, diag- nostic criteria, prognosis and treatment with reference and counter-reference criteria of these two pathological entities with the highest diagnosis in the region. The objective is to provide elements that facilitate the presumptive diagnosis of this type and nature of odontogenic tumors for the den- tist, and on the other hand, to guide clinicians and head and neck surgeons in making therapeutic decisions, since until now, there is no standardized treatment for these two carcinomas.
