Publication Alteraciones anatómicas del conducto nasopalatino en pacientes dentados y desdentados en el sector anterosuperior utilizando tomografía computarizada de haz cónico(2019) Cazar Almache, Marcelo Enrique; Abril Cordero, Luz Marina; Palacios Vivar, Diego Esteban; Abril Cordero, Maria Fernanda; Sibri Quizhpe, Cristina BelenBackground: The nasopalatine canal (NPC) is a long and thin intraosseous passage present in the anterior midline of the maxilla that connects the palate with the floor of the nasal cavity. The objective of the study was to compare the measures of nasopalatine duct and its shape by cone beam tomography in dentate patients with respect to edentulous patients in the frontal anterior sector. Materials and methods: descriptive and comparative study, was taken from the files of the Imagenology Department 150 tomographies of anterosuperior dentate patients and 52 tomographies of edentulous in the frontal anterior sector. Unsing the Excel program and its tool “random”, 30 tomographies of dentate patients were selected as well as the 30 tomographies of edentulous. Were measured in the oblique sagittal plane the anteroposterior distance of the nasopalatine canal at the level of its upper, middle and lower third. To determine its height, the axial plane was taken from the level of the upper bifurcation to the palatal opening. The acquisition of images was done through the i-Dixel software of the 3D Accuitomo 170 MORITA, DICOM system. Results: the dentate patients present higher heights in relation to the edentulous patients. In edentulous patients the diameter have a larger size at the NPC channel level. The measurements at the lower, middle and upper levels in the cortices of the diameter of the NPC in dentate patients are lower in relation to the edentulous patients. Conclusion: Patients of 40 to 70 years old, male and female, with frontal anterior edentulism presented structural topographic variations that consist of greater width, smaller longitudinal dimension and larger transversal dimension being enlarged canals.Publication Descripción de técnica de osteotomía subapical en el tratamiento de deformidad mandibular postraumática(2017) Viteri Luzuriaga, Luis Arturo; Palacios Vivar, Diego Esteban; Cazar Almache, Marcelo Enrique; Miranda Villasana, José Ernesto; Torres Cruz, Yonatan JosuéPost-trauma deformity after a complex trauma fracture or mishandle is not an often situation and represents a challenge for health professionals who treat the facial area. Treatment begins with a preand post-trauma radiographic interpretation, clinical assessment and measurement of study models. The aim of treatment is to recover function, aesthetics and correction of the facial sequel. The treatment plan for this condition may be orthodontic, surgical, rehabilitation or a combination of these. Several surgical techniques to correct maxillary deformities have been described. The most common osteotomies are at body, ramus and alveolar level. This article presents the case of a patient with right alveolar mandibular deformity as a consequence of an inadequate management of facial trauma re-treated with a subapical segmental osteotomy and cortical release to move the alveolar nerve.Publication Infección del seno maxilar por escherichia coli: revisión de la literatura y reporte de caso(2015) Cazar Almache, Marcelo Enrique; Miranda Villasana, José Ernesto; Palacios Vivar, Diego Esteban; Cedillo Orellana, Sara IvannaThe maxillary sinus foreign body is a frequent consequence of accidents in dental treatments. These unfortunate events are due to the close relationship of the root tips of posterior teeth relative to the floor of the maxillary sinus. Extruded endodontic materials can act essentially as foreign bodies and potentially can lead to various pathophysiological responses. Not always produce reaction, in some cases are asymptomatic, and kept only under observation. The microorganisms isolated in patients with sinusitis of odontogenic origin differ from non-odontogenic, its predominant by anaerobic bacteria. The present case refers to accidental displacement of gutta-percha cones to the maxillary sinus; together with the presence of communication oroantral allow entry and colonization of Escherichia coli, an aggressive and rare bacteria in this location. For therapeutic treatment we did an approach through the oroantral communication, and prescribe the sultamicilin as the antibiotic.Publication Rotación del cuerpo adiposo de la mejilla para aumento de la proyección labial: reporte de caso(2016) Cazar Almache, Marcelo Enrique; Palacios Vivar, Diego Esteban; Miranda Villasana, José ErnestoThe treatment of patients with skeletal abnormalities or deformities begins with planning based on facial analysis and cephalometric tracing. Often the values obtained from that analysis reveal abnormal skeletal conditions or insuffi cient soft tissue. Autologous fat grafts and using the buccal fat pad provide viable options to correct these defects and achieve a favorable aesthetic and functional result. The tissue characteristics of the buccal fat pad make it a structure that has good adaptability and a low rate of infection and rejection. It is often overlooked and simply resected to achieve more defi ned cheekbones and facial contours. It has multiple therapeutic uses and should always be considered an option in reconstructive procedures. We present the case of a 17-year-old female patient diagnosed with a dentofacial deformity, soft tissue defi ciency in the lip and perinasal region, and a failed orthodontic camoufl age treatment. Orthognathic and rhinoplasty surgery were performed to correct the hard-tissue defect, and the buccal fat pad rotated and advanced to enhance soft-tissue projection.Publication Tercer molar ectópico en seno maxilar: reporte de un caso(2017) Torres Cruz, Yonatan Josué; Cazar Almache, Marcelo Enrique; Miranda Villasana, José Ernesto; Palacios Vivar, Diego EstebanEctopic tooth are dental organs found in a uncomon regions. There are publication that report ectopic tooth in the maxillofacial region being common sites palate, paraorbitaria region, nasal cavity, maxillary sinus, chin, coronides process and condyle. Cases have also been reported in non-dental areas such as ovary, anterior mediastinum region usually related to a teratoma. Ectopic tooth are often radiographic fi ndings because they are generally asymptomatic. Its therapeutic when found in the maxillary sinus could be conservative under periodic clinical-radiographic vigilance. Removal of these dental organs is necessary for their tendency to form cysts, develop odontogenic tumors and trigger neuropathic disorders. Surgical options are endoscopic, transnasal, Caldwell Luc or quadrangular approach. We present a case of a female of 32 years old with a diagnosis of ectopic third molar in maxillary antrumPublication Tumor pardo maxilar: reporte de caso(2019) Abril Cordero, Luz Marina; Cazar Almache, Marcelo Enrique; Espinoza Soto, Pedro Esteban; Palacios Vivar, Diego Esteban; Cañar Rivera, Gabriela ElizabethIntroduction: Brown tumor is a rare pathology resulting from uncontrolled hyperparathyroidism, which produces areas of localized bone destruction. They are present at the facial level in the mandible and maxilla, ribs, clavicle, pelvic waist and upper and lower extremities. Its name is given by the pigmentation of the extravasation of blood and hemosiderin to the tumor. It can manifest itself in any decade of life. Has greater predisposition in women of the fourth and fifth decade of life. Its presence at the level of maxillary bones requires a complex system of studies and multidisciplinary management for diagnostic confirmation and treatment. Clinical case: The case of a 36 year old patient with no personal or family history is reported. She comes by referral for placement of a bone graft for dental mobility. In the image evaluation, there is a right radiolucent zone in the body of the mandible, in relation to the teeth. The lesion usually presents as a well-delimited unilateral or multilocular radiolucent area; another important radiographic finding in hyperparathyroidism is the absence of hard laminae around the dental organs. Conclusions: The brown tumor should be considered as a differential diagnosis of mandibular tumors due to the fact that not every tumor has a primary etiology
