    Afectación psicológica en adolescentes expuestos a violencia intrafamiliar de la parroquia Ricaurte. Cantón Cuenca. 2015-2016

    Introduction: Violence as a social problem at a global, national and local level results from the use of physical force or power, causing injury, death or psychological damage, affecting the psychological sphere of adolescents, with negative repercussions on their future. Objective: To determine the different types of psychological affection in adolescents exposed to intrafamily violence. Materials and Methods: A cross - sectional, analytical study was carried out on the psychological involvement of adolescents exposed to intrafamily violence, Ricaurte parish, Cuenca canton. With structured questionnaires to 255 participants, collecting information on sociodemographic, family characteristics, violence, detecting psychological affection analyzing prevalences, a bivariate and multivariate analysis determining association by OR and 95% CI. Considering significant results at p <0.05. Results: The prevalence of psychological affection was for moderate anxiety 22.35%, severe depression 14.90%, medium-low self-esteem 25.88% and verbal aggressiveness 52.16%. The type of psychological violence showed 45.1%. The bivariate analysis showed a statistically significant association between anxiety OR = 2.16 (95% CI: 1.18-3.95) (p = 0.011), depression OR = 1.91 (95% CI = 1.11-3.29 ) (P = 0.018), verbal aggressiveness OR = 5.29 (95% CI = 1.16-24.17) (p = 0.01) and rage OR = 2.15 (95% CI = 1.26-3, 68) (p = 0.005), with types of violence. The multivariate analysis found an association between family functionality and adolescent psychological distress OR = 2.68 -2.80, (95% CI = 1.459 - 5.260) (p <0.05). CONCLUSIONS: Exposure to intrafamily violence triggers in the adolescents psychological consequences determined the functionality of the family, which makes it necessary to address this significant social problem

    Análisis del grado de satisfacción de los adultos mayores de las provincias de Azuay y Cañar

    Objective. Analysis of the perceptions about the exercise of the rights of the elderly in the provinces of Azuay and Cañar. Methodology. An analytical cross-sectional study was carried out involving 538 elderly people living in the coverage areas of the first-level health centers, belonging to the Ministry of Public Health and the Rural Social Security, through the application of a rights form, previously validated.Results. The mean age was 75.20 ± 7.22 years (65 to 100 years), the majority were women, married, with primary education and 33% illiteracy. 81% of participants consider their standard of living to be adequate. Living alone is a factor associated with the inadequate exercise of rights in a statistically significant way (OR 2.31, 95% CI 1.46-3.68). Age, marital status, occupation, education, self-identification, are also associated with inadequate exercise of rights, but not significantly.Conclusions. Eight out of ten older adults consider that the exercise of their rights is adequate, and in the group that feels dissatisfaction, solitude is a statistically significant associated factor. This situation reveals the importance of generating social networks of support between peers, with inter-institutionalsupport and especially of local community organizations, to achieve the full exercise of rights, without exclusions.

    Associations between agency and sexual and reproductive health communication in early adolescence: a cross-cultural, cross-sectional study

    Purpose: To assess the extent to which adolescents aged 10–14 have communicated about sexual relationships, pregnancy, and contraception and how agency in the form of voice and decision-making along with an enabling socioecological environment are associated with sexual and reproductive health (SRH) communication. Methods: Using data from the Global Early Adolescent Study, we included 1,367, 697, and 1,424 adolescents in Kinshasa, Cuenca, and Shanghai, respectively. Patterns of SRH communication and agency levels were described by site and sex. Multivariable logistic regressions assessed odds of SRH communication first in relation to socioecological characteristics and second with levels of agency, after adjustment for social environmental factors. Interaction terms tested sex differences in associations. Results: Experiences of SRH communication ranged from one in ten in Kinshasa to about half in Cuenca. Pregnancy was the most discussed SRH topic. Socioecological factors consistently related to SRH communication included older age and pubertal onset, while others varied by context. In multivariable analyses, voice was linked to all forms of SRH communication in Kinshasa and Cuenca with adjusted odds ratios ranging from 1.6 to 2.2, but not in Shanghai. In Cuenca, decision-making was associated with a 50% and 60% increase in odds of communication about pregnancy and contraception, respectively. In Kinshasa, a stronger association between voice and pregnancy discussions was observed for girls than boys. Conclusions: Developmental characteristics and voice were linked to communication about SRH among young adolescents across two contexts. Results suggest agency may play a role in shaping antecedents, like communication, to sexual behaviors. © 2020 Society for Adolescent Health and Medicine

    Barriers and facilitators to cervical cancer screening among under-screened women in Cuenca, Ecuador: the perspectives of women and health professionals

    Background: Cervical cancer screening is a cost-effective method responsible for reducing cervical cancer-related mortality by 70% in countries that have achieved high coverage through nationwide screening strategies. However, there are disparities in access to screening. In Ecuador, although cervical cancer is the second most common cancer in women, only 58.4% of women of reproductive age have ever been screened for cervical cancer. Methodology: A qualitative study was performed to understand the current barriers to screening and to identify strategies that could increase uptake in Azuay province, Ecuador. Seven focus group discussions (FGDs) were conducted with under-screened women and health professionals (HPs). The FGDs were recorded and transcribed. Content analysis was done using the socio-ecological framework to categorize and analyse the data. Results: Overall, 28 women and 27 HPs participated in the study. The two groups perceived different barriers to cervical cancer screening. The HPs considered barriers to be mainly at the policy level (lack of a structured screening plan; lack of health promotion) and the individual level (lack of risk perception; personal beliefs). The women identified barriers mainly at organizational level, such as long waiting times, lack of access to health centres, and inadequate patient–physician communication. Both groups mentioned facilitators at policy level, such as national campaigns promoting cervical cancer screening, and at community and individual level, including health literacy and women’s empowerment. Conclusions: The women considered access to health services the main barrier to screening, while the HPs identified a lack of investment in screening programmes and cultural patterns at the community level as major obstacles. To take an integrated approach to cervical cancer prevention, the perspectives of both groups should be taken into account. Additionally, new strategies and technologies, such as self-administered human papillomavirus (HPV) testing and community participation, should be implemented to increase access to cervical cancer screening

    Características clínicas, manejo y evolución de pacientes con sindrome de tunel carpiano. Hospital José Carrasco Arteaga, enero 2013 – diciembre 2016

    Background: Carpal Tunnel Syndrome consists of compression of the median nerve at the wrist level, due to postures maintained in flexion of this joint, or repetitive movements using force; producing pain and various associated symptoms. General Objective: Describe the clinical characteristics, management and evolution of patients with diagnosis of Carpal Tunnel Syndrome, in the Hospital José Carrasco Arteaga. Methodology: Descriptive study in all the cohort of patients (168) diagnosed with STC in the Hospital José Carrasco Arteaga, with data collected from medical records, from January 2013 to December 2016. The variables were tabulated through tables and analyzed in Microsoft Excel 2013 and SPSS 15.0. Results: Of 168 patients, the female sex predominated (81%), the average age was 55.5 years ± 12.1 SD, and the most common occupations were office workers and housework with 25.6% each. The most frequent symptoms were pain (90.5%) and paresthesia (95.8%); and signs Phalen and Tinel were positive in 79.8% and 75% respectively. The average evolution time was 42.1 months. As conservative management, neuromodulators (62.5%) and non-steroidal anti-inflammatory drugs (58.9%) were the most used. Surgical treatment was performed in 67.3%, followed by 14.15% referred to persistent pain. Conclusions: STC affects female sex more and predominates in people over 55 years of age, mostly office workers, with evolution time greater than one year, and management is mainly conservative, in cases advanced surgical decompression, with a low percentage of complications.

    Comparación de las complicaciones maternas y neonatales entre el parto en posición vertical y en posición Litotómica en el Centro de Salud Carlos Elizalde en el período 2014-2016

    Background: The vertical and lithotomic position during the childbirth have advantages and disadvantages. This research presents its findings. Objective: Identify and compare maternal and neonatal complications associated with vertical and lithotomy delivery. Methodology: This research was analytic cross-sectional. 884 maternal medical records attended in the Carlos Elizalde Health Center of Cuenca 2014-2016 was reviewed. We studied: postpartum hemorrhage, perineal tears, need for episiotomy, APGAR score, presence of meconium fluid and neonatal trauma. We use frequencies, percentages, mean, median and standard deviation. We determine its relationship with the Chi square test and Odds Ratio. Results: 80,4% chose the lithotomy and 19.6% the verticality. With the first position, a major percentage required episiotomy (5,1% vs. 0,6%), but with the vertical there were more tears (48,6% vs. 45,6%), postpartum hemorrhage (0,8% vs. 0,3%). %). Neonates with APGAR under the first minute (0,6% vs 0,6%) decreased to the fifth minute, with vertical posture (0% vs 0,4%); the presence of meconium liquid (17,9% vs 17,3%) the variation was minimal. Vertically, cephalohematoma was major (2,3% vs. 0,3%), and caput succedaneum with lithotomy (15,6% vs. 11,6%). The vertical position, is a protective factor against episiotomy (OR: 0,109 IC: 0,0148 – 0,8007; p: 0,008). The presence of tears (OR: 0,887 IC: 0,636-1,237; p: 0,719), postpartum hemorrhage (OR: 0.818 IC: 0,234-2,860; p: 0,092), APGAR during the first minute (OR: 1,027 IC: 0,114-9,252; p: 1) and fifty minute, the presence of meconium liquid (OR: 1,043 IC: 0,675- 1,611; p: 0,847), caput succedaneum (OR: 0,70 IC: 0,42-1,17; p: 0,178) y cephalohematoma (OR: 2,78 IC: 0,77- 9,96; p: 0,216) weren´t associated statistically with position during the delivery’s expulsive phase. Conclusion: The lithotomic position is more frequent; in this position there were more cases of tears grade II, IV in contrast the vertical where were tears grade I and III mainly. Cephalohematoma is more frequent in the vertical and caput succedaneum in the lithotomic. The vertical position protects against the need for episiotomy

    Complicaciones obstétricas en adolescentes atendidas en el servicio de obstetricia del hospital “Homero Castanier Crespo” en el periodo enero- diciembre 2008

    Background: a retrospective descriptive quantitative study took place in the obstetric department of the Homero Castanier Crespo Hospital in Azogues city 2008 data was obtained from the Statistics Department. Objective: To determine the frequency of obstetric complications in adolescents attending in the obstetric department of the Homero Castanier Crespo Hospital in 2008. Materials and Methods: all clinical histories that registered teen pregnancies in the obstetric department during 2008 were reviewed in Homero Castanier Crespo hospital. The study`s sample was obtained from these histories, and excluded those without complications. To collect data from the clinical histories, a form was filled, then the information was processed with Excel program and analyzed in SPSS program. Results: The 69.09% of pregnant teens were between 17 and 19 years (late adolescence). 65.5% came from rural areas. 43.64 hadn’t completed middle school. 40% of them are married. The main obstetric complications, were cesarean in 92 cases (41.82%), urinary tract infection represented 30.91%, the preterm labor in 10% of teen pregnancies, though preterm birth ocurred in the 2.73% of the cases; premature rupture of membranes and cefhalopelvic disproportion ocurred in 8.64%, pre-eclampse accounts for 4.55%, grade II perineal tear was made in the 1.82%. The deferred abortion with 3.64%, followed by incomplete abortion 2.73%, 2.27% complete abortion, threatened abortion accounted for 1.82%, abortion in progress at the 0.91%. There were no septic abortion and inevitable abortion. Conclusions: Teen pregnancy is very common in our society especially in rural population, most of these teenagers were married and with an incomplete middle school. The main complication is cesarean.

    Conocimiento y prácticas de prevención de cáncer de cuello uterino en mujeres con lesiones histopatológicas. Cuenca, Ecuador 2021

    Knowledge and practices are essential to promote health, prevent disease, maintain health, and cope with diseases in general and cervical cancer in particular. The objective of this study was to describe the sociodemographic characteristicsand prevention practices in women with histopathological lesions of the cervix, for which 174 female patients,between 30 and 89 years of age,were interviewed at the SOLCA and Vicente Corral hospitals in the city of Cuenca. The results show that the participants, mostlyliving in urban and marginal urban areas, married, with primary and secondary education, with economic difficulties and mestizo self-identification, more frequently presented histopathological lesions of the LIEAG, LIEBG and Cancer type. Although they have information about HPV, very few know that it is the main cause of CCU, that it is transmitted sexually, that there are vaccines and early detection tests. Among the facilitating factorsto carry out the Pap smearare:the follow-up visit, medical recommendation, own initiative and recommendation of a friend or relative; while personal factors, problems in the health service, economicand familydifficulties, constitute the main barriers.
