    La colecistectomía laparoscópica en el Hospital Vicente Corral Moscoso de Cuenca, 1994-2008

    Laparoscopic cholecystectomy (LC, Cole-Lap) is the treatment of choice for benign biliary disease, which represents a high incidence in our city and country. In our hospital surgeons in training and teachers have been participated in the development of this technique actively. The goal is share the experience of 14 years of laparoscopic cholecystectomy at the Vicente Corral Moscoso Hospital. Methods: we present the descriptive and retrospective case series of 2.200 patients who underwent laparoscopic cholecystectomy from May 1994 until January 2008. Results: of 2.200 CL, 79% are female. The average age was 43 years with intraoperative diagnosis of chronic cholecystitis in 82%. Complications occurred between major and minor in 38% of which the perforations in gallbladder bile output and bleeding from the cystic artery were the most frequent. Injury to the bile duct occurred in 0.12%. Twenty five percent of one hundred the first surgeries presented complications while thirty four percent of one hundred last surgeries presented complications. The average operating time was 35 minutes in 2008. Discussion: laparoscopic cholecystectomy in our study proves to be a safe and effective procedure in patients with acute or chronic calculous cholecystitis.

    Funduplicatura de Nissen laparoscópico en el tratamiento de la enfermedad por reflujo gastroesofágico. Hospital Vicente Corral Moscoso. Cuenca, 1997-2008

    Introduction: Approximately between 25 and 50 percent of patients with gastroesophageal reflux disease (GERD) have persistent progressive disease in them, surgery is the best option and the only resource able of restoring the gastroesophageal barrier. The Nissen fundoplication of choice, is performed by laparoscopy since February 1997 in our hospital. Objective: To describe demographic and clinical aspects of patients undergoing laparoscopic Nissen fundoplication at Vicente Corral Moscoso Hospi - tal. Method: Empirical method will be applied to facilite the collection and analysis of data as videos statistical records and surgery of laparos - copic Nissen fundoplications. Results: From February 1997 to July 2008, there were 58 fundoplications, 36 men and 22 women, with an average age of 42 years old. Predominant symptoms as heartburn, regurgitation, abdominal pain and dysphagia. Diagnosed by upper endoscopy and upper gastrointestinal series at all. Immediate postoperative complications: September ileus, dysphagia 5, urinary retention 4. Mean operative time: 181 minutes in 1997, 102 minutes in 2008. Hospital stay: 4 days. 49 patients were fed in 24 hours. No conversion or mortality. Conclusions: Laparoscopic surgery in the treatment of GERD is increasing and is an effective long- term treatment. But surgeons must also review the successes, mistakes, complications and limi - tations of these long-term applications with more variable

    Impacto post quirúrgico de la cirugía bariátrica y comorbilidades asociadas. Hospital José Carrasco Arteaga: Cuenca-Ecuador, 2014-2018

    Background: Obesity is a chronic disease with multifactorial etiology, the high incidence and complications associated to this, determine his severity; that is justification to study obesity´s treatment, especially bariatric surgery and her impact not only on weight and body mass index (BMI), but also on associated comorbidities. Objective: To determine the post-surgical impact of bariatric surgery at the José Carrasco Arteaga Hospital during the years 2014-2018, in obese patients subjected to this intervention and their associated comorbidities: mean blood pressure, blood glucose, cholesterol and triglyceride blood levels. Materials and methods: Retrospective descriptive study. 90 medical records that adjust to the inclusion criteria, were reviewed. The data was collected using a form and was tabulated in Microsoft Excel and SPSS 20. The qualitative variables are presented in absolute frequencies and percentages, and the quantitative variables were analyzed with arithmetic means and standard deviation. Results: 90 patients enrolled in the bariatric surgery program were studied, 75.56% women, mean age 42.88 ± 10.40 with mean BMI 43.66 ± 5.13. 77 were operated and 66 were followed up. 6 and 12 months after surgery, BMI decreased to 32.26 ± 3.90 - 29.73 ± 3.93, and the excess weight loss was 62,40±18,01% and 76,33±21,39% respectively. Additionally there was a reduction in: mean arterial pressure (MAP) 7.11 - 10.73mmHg, glycemia 28.21 ± 17.29 - 47 ± 25.73mg / dL, Triglycerides 40.53 - 70.11mg / dL and Cholesterol of 32.17 - 47.35mg / dL respectively. Conclusion: Bariatric surgery will allow the decrease in BMI and in levels of MAP, glycemia, cholesterol and triglycerides.

    Prevalencia y factores asociados a complicaciones poshernioplastia inguinal, Hospitales Vicente Corral Moscoso y José Carrasco Arteaga, 2018-2019

    Abstract: Background: hernia is one of the most frequent pathologies in General Surgery, a common reason for surgical consultation, the resolution of this pathology currently has extensive debates about the surgical approach related to open or laparoscopic hernioplasty, with advantages and disadvantages exposed to each. Objective: to determine the prevalence and factors associated with complications of inguinal posthernioplasty at the Vicente Corral Moscoso and José Carrasco Arteaga Hospitals, 2018-2019.Material and methods: It’s analytical, cross-sectional study. 240 clinical records that met the inclusion criteria were reviewed. Data were collected using a form and were tabulated in the SPSS version 15 program. The qualitative variables were analyzed with frequency and percentage and the quantitative variables with arithmetic mean and standard deviation, the associated factors were analyzed with Odds Ratio, 95% CI, Chi square and binary logistic regression (p <0.05).Results: 240 patients who had the inclusion criteria were analyzed, the majority were men (81.7%), the mean age was 53.4 years SD ± 16.57. The prevalence of postoperative complications was 27.5%, with those occurring within the first 24 hours being more frequent with 15.8%. Among the factors associated with statistical significance were: male sex (OR 4.6; 95% CI 1.5-13.5; p <0.00) and surgical technique (OR 3.4; 95% CI 1.2-9; p <0.01).Conclusions: prevalence of postoperative complications was similar to the literature consulted and was associated with factors such as: male sex and surgical technique.

    Utilidad de la cirugía bariátrica en el tratamiento de la obesidad y comorbilidades en el Hospital José Carrasco Arteaga. 2009-2013. Cuenca

    Introduction: Bariatric surgery is an effective treatment option in patients with obesity and comorbidities. General Objective: To determine the usefulness of bariatric surgery in the treatment of obesity and comorbidities. Material and Methods: Retrospective descriptive study in a cohort of 71 patients diagnosed with obesity, who underwent bariatric surgery and were admitted to the department of surgery at the hospital "José Carrasco Arteaga" in the period between 2009 and 2013. The patients underwent anthropometric assessment, blood glucose levels, lipid profile (cholesterol, triglycerides, HDL, LDL) and blood levels, preoperative voltage, at the third month, sixth month, and also after the postoperative year. Results: We have analyzed 71 patients with morbid obesity (57 women and 14 men) with a mean age of 45.04 ± 10 years. The average preoperative weight was 110.16 ± 18.6 kg and BMI of 42.6 ± 4.6 kg / m2. In the third month, sixth month, and a year after the surgical procedure BMI was 34,54Kg / m2, 31.64 kg/m2, 30,36Kg/m2 respectively, the weight reduction for the third month, the sixth month, and after the subsequent year surgery was 86,64Kg, 82,05Kg, 78,77Kg respectively, the percent of loss of the excess of the weight was 45.9%. We also found out that 90.8% of dyslipidemic patients, 100% of diabetics and 98.5% of hypertensive patients normalized their corresponding profiles after surgery. A 7.04% of patients had postoperative complications, among which wound dehiscence was found to be the most frequent; derived from these complications the rate of mortality reached a 5.6%. Conclusion: Bariatric surgery is an effective method for the control of obesity in different degrees and comorbidities associated with this disorder such as hypertension, diabetes and dyslipidemia
