    Complicaciones gestacionales y perinatales asociados a la edad materna en los extremos de la vida fértil

    Introduction: The pregnancy at any age is a very important fact biopsychosocial, however in adolescence and at advanced ages leads to a series of situations that can undermine the health of the mother and the life of the son, and become a public health problem. Methodology: an observational study was conducted with a analytical cross-sectional sample of 729 maternal that came to the Hospital Vicente Poultry Moscoso of the city of Cuenca. We evaluated the obstetric complications during pregnancy, labor and the newborn. To determine the association used the odds ratio with its confidence interval is considered to be statistically significant values of p<0.05. Results: according to the age, 251 (34.3 %) patients had ages less than 20 years, 181 (24.8 %) patients had ages over 34 years and 297 (40.7 %) patients had between the ages of 20 and 34 years. It was found a statistically significant association between adolescent girls and the pregnancy-induced hypertension, preeclampsia, premature birth, anemia, compromise of the fetal well-being, track completion of delivery and complications of the labor. In women of advanced age there was an association with the gestational hypertension, premature rupture of membranes, respiratory distress and income to pediatric intensive care unit.     Estudio multicéntrico de la eficacia del amnisure© en comparación con la cristalografía para el diagnóstico de ruptura prematura de membranas subclínica

    Introduction Premature rupture of membranes happens in 8% of pregnancies to term and 30-40% of preterm. The consequences of this disease are very serious for both mother and the product of pregnancy. Objective Determine the effectiveness of AmniSure © compared to Crystallography for diagnosing subclinical Premature rupture of membranes in pregnant women attending hospitals Vicente Corral Moscoso and José Carrasco Arteaga during 2014-2015. Material and methods Diagnostic test validation was performed in 130 patients. The sample size was calculated with the sensitivity of 97% for AmniSure © and 84% of crystallography, and the McNemar test was applied. The data were obtained by direct interview, and analyzed with SPSS version 15 software. Results The minimum age was 13 years, the maximum 40 and the median 25. The median gestational age of 36 weeks. 43.8% had rupture of membranes before 37 weeks gestation, and 56.2% between 37 and 42. 14.6% had a previous diagnosis of premature rupture of membranes. © AmniSure sensitivity was 89%, specificity 78%, the positive likelihood ratio 41.4%, the negative likelihood ratio 14%, 53% positive predictive value and negative predictive value 96%. Conclusion The AmniSure © is a valid test for the diagnosis of premature rupture of membranes

    Prevalencia de restricción de crecimiento intrauterino y factores asociados, Hospital José Carrasco Arteaga, Cuenca, 2015-2019

    Introduction: intrauterine growth restriction (IUGR) is an important cause of fetal and neonatal morbidity and mortality. Objective: to determine the prevalence of IUGR and the associated factors in patients from the Gynecology and Obstetrics area of the José Carrasco Arteaga Hospital, Cuenca Ecuador, in the period 2015-2019. Materials and methods: a cross-sectional analytical study was carried out. The sample consisted of 350 electronic medical records from the AS-400 system. The data was analyzed using the statistical program SPSS version 24, where frequencies (f), percentages (%), prevalence ratio (PR) were estimated. Results: the mean age of the maternal population was 29 years; 50% of pregnant women were young adults; 71% were multiparous, 55% of pregnancies ended by cesarean section. The prevalence of IUGR was 8.57%, with an asymmetric predominance (56%). An associated factor for developing IUGR was hypothyroidism (PR 2.8; 95% CI 1.2 - 6.7; p = 0.019) and this relationship was significant; On the other hand, there was no statistical significance with obesity (PR 1.87; 95% CI 0.79 - 4.44; p = 0.35), anemia (PR 0.3; 95% CI 0.1 - 2, 7; p = 0.3), placental pathology (PR 1.3; 95% CI 0.2 - 8.5; p = 0.20), pre-eclampsia (PR 2.2; 95% CI 0.9 - 4.9; p = 0.06) and urinary tract infection (PR 1.6 95% CI 0.8–3.3; p = 0.14). Conclusion: The prevalence of intrauterine growth restriction was similar to that of similar studies from other regions and the associated factor was hypothyroidism.

    The work is part of the proposed Tobacco, poverty, disease in the city of Cuenca, 2001. A study was performed in order to identify the most common diseases in hospitalized patients who consume cigarette in the José Carrasco Arteaga Hospital in Cuenca.1997- 2001. Loa results obtained by the consumption of snuff were: the total number of investigated (1563pacientes) 38.1 (596pacientes) fuman.En patients who smoke the most common diseases in accordance with the systems were: Genitourinary a condition 32.9siendo The most frequent benign prostatic hypertrophy (BPH); in Cardiovascular with 32.8siendo Cerebra Vascular Event (CVD) more frequent; respiratory 15.6siendo more frequent with Chronic Obstructive Pulmonary Disease (COPD), other (diseases unrelated to smoking) with 9.4siendo common as cataract; In the digestive system with greater frequency 9.0presento gastritis.De patients who smoke 79.4son 20.6son male and female. Depending on the age the highest percentages were observed in elderly (> 65) with 44.5; older adults (36-64 years) with 41.6; 13.1 Young adults, teenagers 0.8. According to the 50 primary education, secondary 34.1 Higher 12.2 Illiterate 3-7En the data were divided according to occupation: Employees 33.1 Retired 31.7 10.7 Farmers, Artisans 4.4 Drivers 4 QQDD 4 Professional 3.2, Informal 3.2 Health Professionals 2.2 2.2 Unemployed, Students 0.8, Other 0.3 and 0.2 Religious are those who smoke more. According to the source 74.2 are from urban areas and 25.8 belong to the rural area
