    Acceso a los servicios de salud y nivel de satisfacción de los usuarios del Subcentro de Salud de El Valle en la ciudad de Cuenca durante el año 2013

    Introduction: the evaluation of the accessibility and user satisfaction to local health benefits is important because knowledge-generating processes that guide policies changes in the public health.- Objective: to determine access to health services and satisfaction of users of ambulatory care facilities, El Valle, in the Cuenca city in 2013.- Methodology: this was a descriptive cross study, were collected demographic data and indicators of accessibility to health services and user satisfaction, the selected sample was 406 randomly selected users, and then they sign the informed consent, data were digitized at a base in SPSS 19 for analysis and presentation using frequency tables. Results: the mean age of the population stood at 33.23 years with a standard deviation of 14.86 years, in 85.7% of cases the respondents were women, adequate geographic accessibility was identified in 74, 4% economic in 97.8%, transport 87.2%, the average time between home and the health center was 59.98 minutes, 45.51 minutes between arrival at the health center and obtaining an appointment for care and 25.95 minutes between obtaining a turn and medical care, the information provided by the physician was adequate in 92.1% of cases, cleanliness of the institution was not adequate, respect to infrastructure only 56.2% of users said it was good, 83.5% of the users considered adequate human resources, user satisfaction was adequate in 90.4%.- Conclusions: access to health services geographically, economically and culturally was adequate in most services.AU

    Análisis de la mortalidad en el Ecuador 1960 - 1999

    Mortality was studying at home as one of the most sensitive indicators of the health-disease situation in relation to the characteristics of the development of the country through the prevailing development models from the sixties until the end of this century . The models were considered for the study are: the exporting Agro, including the sixties, the model of import substitution to the 70 and the neoliberal model from the eighties corresponds. For the analysis of mortality has taken the following indicators: General Mortality, proportional mortality by age and sex, and child mortality; all the indicators are analyzed in each decade. It is noted that the exporting Agro model characterized by backwardness and poverty predominates mortality from infectious and parasitic diseases. In the import substitution model that matches petrolo export and foreign indebtedness, the health situation remains in their similar characteristics. Mortality on the neoliberal model with new profiles in which coexist enfermdades of human poverty and backwardness with the modernization, such as ischemic heart, traffic accidents, tumors, vascular disease and hypertensive brain, violence etc.

    Angioplastia primaria con trombectomía versus angioplastia primaria convencional en pacientes con infarto agudo del miocardio con elevación del segmento s-t. Hospital José Carrasco Arteaga. Cuenca, 2009- 2016

    BACKGROUND During primary angioplasty (PA) in patients with ST-segment elevation myocardial infarction (STEMI), intracoronary thrombi may embolize into the microcirculation and increase the size of infarction, a phenomenon that can be reduced by thrombectomy, however recent studies have not shown improvement in coronary reperfusion and major clinical events, so its routine use is controversial. METHOD A sample of 206 patients with STEMI from the registry of the Ecuadorian Institute of Social Security, 107 in the PA with thrombectomy group and 99 in the PA conventional group, were included. The objective was to compare coronary reperfusion (ST-segment resolution, final TIMI 3 flow, no reflow and recovery of the left ventricular ejection fraction) and major clinical events (cardiac death, non-cardiac death, perforation, stent thrombosis, stroke, major bleeding, reinfarction, cardiogenic shock, emergency revascularization, NYHA 4 heart failure) at 30 days. During the analysis we used chi squared, analysis of variance, relative risk and confidence interval (CI) 95% and a level of significance <0.05. RESULTS The ST-segment resolution ocurred in 29.9% of patients in the PA with thrombectomy group versus 43.3% in the PA conventional group (RR 0.68, 95% CI: 0.47-0.99, P: 0 , 04). Death from cardiac causes ocurred in 4.7% of patients in the AP with thrombectomy group versus 13.1% in the PA conventional group (RR 0.35, 95% CI: 0.13 - 0.96 P: 0.032). The other parameters of reperfusion and major clinical events did not have difference. CONCLUSIONS Thrombectomy was not associated with improvement in coronary reperfusion however it decreased cardiac death.

    Aplicación de la escala de Wilson vs LEMON para la predicción de via aerea dificil y su relación con la escala de Cormack, Hospital José Carrasco Arteaga. Cuenca 2018

    Introduction: One of the main complications is the difficult airway in patients undergoing general anesthesia, so it is important to be prepared for management using predictors. Objective: To determine the predictive value of the Wilson and LEMON scale for the prediction of difficult airway and its relationship with the Cormack scale at the José Carrasco Arteaga Hospital in the city of Cuenca in the period January - December 2018. Methods: An observational, descriptive evaluation study of diagnostic tests was carried out during 2018. The population consisted of patients who underwent surgery under general anesthesia and intubated by laryngoscopy. The sample was 350 patients, the presence of predictors of the difficult area was determined using the Lemon and Wilson scales and their results were compared with the Cormack scale. The data were analyzed with the SPSS 22.0 free version system. Results The prevalence of difficult airway according to the Cormack scale was 13.14%, according to the Lemon scale of 8.6% and Wilson 5.1%. sensitivity 47.83% 26.09% specificity 97.37% 98.03% Validity index 90.86% 88% Positive predictive value 73.33% 66.67% Negative predictive value 92.50% 89.76% Kappa index 53% 32.5% Youden index 0.45 0.24 Probability ratio 18.17 13.22 Reason for likelihood 0.54 0.75 Conclusions due to the low prevalence of the scales comparing Cormack's, it is concluded that they are not useful for predicting difficult airways, but for easy airways.

    Características clínicas y radiológicas de las mujeres de 40 años que se realizaron mamografía de Screening en el Instituto del Cáncer SOLCA Cuenca, durante el eríodo 2006-2011

    Objective: The objective of this study is to determine the clinical and radiological characteristics of screening mammography in women aged forty and over who attended a specialist in Cancer Institute SOLCA Cuenca, during the period 2006-2011. Materials and Methods: We performed a retrospective study in 859 women with histories which met the inclusion criteria. We used a formulary with 9 questions. Statistical analysis was performed using Excel 2010 Microsof Office. Results: The study was conducted with 859 medical records of which 56% were aged between 40 and 49 years, 55.76% were from urban areas, the 60.65% were engaged in housework, 25.84% had their menarche at 13 years, 24.26% had their menopause between 40 and 49 years, 45.29% did not have any symptoms at physical examination, the results about mammography was benign in 80.55%, only in 203 women was made biopsy, and only in 33 were found invasive Ductal Carcinoma, and of these, at 12.12% was found advanced cancer, with metastases. Conclusions: Most women attending screening mammography performed in the Cancer Institute SOLCA Cuenca had between 40 and 49 years however the diagnosis of breast cancer is more common after the age of 50 so that our study a low percentage of diagnosed cases of cancer in women who underwent screening mammography found.

    Características de los accidentes de tránsito y su repercusión clínica en los pacientes ingresados en el Hospital Teófilo Dávila, Machala agosto 2005-marzo 2006

    In order to identify the characteristics of traffic accidents suffered by patients admitted to HTD, in the period August 2005-March2 2006, to present a need that goes unnoticed and is a real public health problem. The research results reported 172 patients admitted; the 80,23corresponde to male and female 19,77al. As the group of young adults (20-39 years) the largest percentage of income 53.49. The 31,40de patients had primary education, 53,4habían consumed alcohol. The types of accidents: 42,44choques; 23,26atropellamientos. The type of vehicle greater involvement; cars with a 34.88, followed by motorcycles with 19.77. The type of injury: 30.81 polytrauma; craniocerebral trauma, fractures of the lower limbs with 16,86y with 9.88 deaths. The highest percentage of these patients hospitalized 37,21permanecieron a period of 0-3 days

    Características de los artículos científicos de las revistas de la Facultad de Medicina periodo 2000-2012

    Objective. – To know the features of scientific journal articles from the Faculty of Medicine, period 2000 -2012. Materials and methods. – Descriptive study, done by review of articles published in the Journal of Faculty of Medical Sciences, period 2000-2012. The data collection instrument was a proven form, which consist of bibliometric indicators. The results were processed in the SPSS 15.00 system. RESULTS. – Most articles were published in 2012 totaling 32; 207 males and 117 female authors; characterized by a single author in 46%; original works predominated with 51.6%; quantitative in 50.8%. A 56.4% of the articles don’t comply with Vancouver norms. International collaboration is 3.9%; and inter-institutional collaboration 7.8%; of which 2.3% do so with the MSP. 52.3% of the articles have co-authored. Inter-institutional coauthored with 15.6% of articles. The hospital is the place where most of the research took place, being the patients the biggest beneficiaries with 39%. The most frequent references were books with 83.6% and magazines with 52.3%. CONCLUSIONS. – The study shows that most of the articles published were descriptive quantitative studies in which no Vancouver norms were applied. The impact factor and H factor were neither found in any article. KEYWORDS.
