    Histeroscopía diagnóstica versus legrado uterino en hemorragia uterina anormal, Hospitales José Carrasco Arteaga y Vicente Corral Moscoso

    Introduction.- The abnormal uterine bleeding is frequent gynecological consultation. In patients peri- and postmenopausal women should be ruled out neoplastic etiology through endometrial histology. Objectives.- Study with 84 patients peri and postmenopausal, over 35 years, with abnormal uterine bleeding, in hospitals José Carrasco Arteaga, Vicente Corral Moscoso and Central Dispensary-IESS of Cuenca, to determine sensitivity, specificity and index of plausibility of the hysteroscopy against histology; estimate concordance and compare diagnostic capabilities of hysteroscopic biopsy versus curettage. Methodology.- Validation of diagnostic testing; and the application form for personal data, diagnostic hysteroscopy and anatomopathological. Data processing in SPSS and Epidat. Determination of sensitivity, specificity, and index of verisimilitude through statistical inference; concordance with the Kappa index; diagnostic capacity with Chi-square test. Results.- Main age group: women between 35 and 49 years (82.2 % ), mostly perimenopausal. Major alterations: metrorrhagia and menometrorrhagia (73.8 % ). Most frequent diagnoses: histeroscopic: endometrial hypertrophy; histopathology: normal endometrium. Concordance: 51% among hysteroscopy and hysteroscopic biopsy, and 31.4 % with curettage; 60.6 % for hysteroscopic biopsy and 66.2 % for curettage. Histopathological study: high sensitivity and low specificity. Histeroscopic study: low diagnostic efficacy compared with both biopsies. Poor concordance between hysteroscopy and curettage. Hysteroscopic Biopsy: higher diagnostic capacity in front of the curettage (likelihood ratio 18.00, p<0,001 ). Conclusion.- Hysteroscopy: reliable method to diagnose polyp, myoma and endometrial cancer; little sensitive and entitled to endometrial hyperplasia. Poor concordance between hysteroscopy and curettage. Higher diagnostic capabilities of hysteroscopic biopsy in front of the curettage.

    Magnesemia terapeutica alcanzada en mujeres con Preeclampsia severa atendidas en el Hospital Vicente Corral Moscoso Cuenca, enero – diciembre del 2014

    Objective Determine the percentage of pregnant women with severe preeclampsia that have subtherapetic magnesemia after the administration of 4 grams as impregnation and 1 gram/hour as maintenance of magnesium sulftate, in the service of Obstetrics of the Hospital Vicente Corral Moscoso, Cuenca-Ecuador during 2014. Materials and methods Descriptive study involved all women with severe pre-eclampsia who received magnesium sulfate according with Zuspan scheme to prevent eclampsia and signed informed consent. Patients with convulsions and kidney or liver failure were excluded. Data were collected through a questionnaire about socio-demographic variables, medical history, laboratory tests and signs of intoxication at 4 and 24 hours after initiation of treatment. The analysis was done using the program SPSS version 15. Results 108 cases were analyzed, there were no missing data. The average magnesium levels were 4.47 ± 1.69 mg/dl at 4 hours and 4.34 ± 1.54 mg/dl at 24 hours. The 69.4% had subtherapeutic levels at 4 hours and the 75.9% at 24. No patient had seizures. There was a case of intoxication that improved after the temporary de-crease of maintenance dose. Fisher test showed a significant difference between the magnesemia at 4 hours and nutritional status, p = 0.023; but not at 24 hours, p = 0.101. Conclusion 69.4% of women in this study after 4 hours and 75.9% at 24 hours showed sub-therapeutic levels of magnesium
