BACKGROUND: adnexal masses (AM) are a common finding in women of reproductive age and are often a diagnostic and therapeutic challenge for the treating physician, since they require a clear differentiation of benignity or malignancy. GENERAL OBJECTIVE: to validate the Gynecological Imaging Data and Reporting System (GI-RADS) in comparison with the histopathological diagnosis for the diagnosis of malignancy of adnexal masses in adult patients attending the SOLCA-Cuenca Cancer Institute 2014-2019. METHODOLOGY: diagnostic validation test. 101 clinical records that met the inclusion criteria were reviewed. He contrasted GI-RADS with histopathologic examination of adnexal masses. If it produced descriptive statistics, values of sensitivity, specificity, positive and negative predictive value were recorded for validation. RESULTS: 56.4% corresponded to women aged 40-64 years (mean 46.39 ± 15.02 years). The location of the AM was found more frequently in the right annex 48.51%. Histopathological, 80.2% of the women were diagnosed with benign adnexal masses. According to the GI-RADS, 71.29% and 21.78% of cases are classified as grade 3 and 4, respectively. Serous cystadenoma was the most frequent tumor in 38.61%. The evaluation values of the GI-RADS test were: specific 87.8%, sensitivity 68.42%, positive and negative predictive values of 56.52% and 92.31%. CONCLUSIONS: the GI-RADS had a good performance in discriminating patients who did not present malignant tumors, but it did not adequately classify women with malignant neoplasms.
