Item Actividad de la enfermedad relacionada con calidad de vida y adherencia al tratamiento en pacientes con artritis reumatoide. Hospital IESS-Cuenca, 2019(Universidad de Cuenca, 2020-06-01) Cáceres Gutiérrez, Jennifer Alexandra; Cando Sánchez, Karina Marisol; Guevara Pacheco, Sergio VicenteBackground: rheumatoid arthritis is a chronic inflammatory disease that causes joint destruction, negatively impacting quality of life. Periodic evaluation of disease activity determines prognosis and therapeutic success. Due to the aforementioned, it is necessary to evaluate the quality of life and in a complementary way the adherence to treatment as fundamental pillars in the control of the disease. General objective: to determine the activity of the disease and its relationship with quality of life and adherence to treatment in patients with rheumatoid arthritis at the IESS-Cuenca Hospital, 2019. Methodology: analytical, observational, cross-sectional study in 265 patients who attended the rheumatology outpatient clinic of the IESS-Cuenca Hospital. Prior to signing the informed consent, sociodemographic information, time of illness and comorbidities were collected on a registration form prepared by the authors. In addition, validated questionnaires were applied for health-related quality of life and adherence to treatment. The data obtained was analyzed in the SPSS version 12.0 software. Results: 83.8% of the study population corresponded to the female gender. The average age was 55.8 ± 12.8 years. Regarding disease activity, 45.3% were in remission and 4.5% in high activity. Quality of life had an average score of 6.89 and was rated as good in 86.8% of cases. Treatment adherence was 67.9%. A significant association was found between disease activity and quality of life (p = 0.001). MATERIAL AND METHODS.- In this study there take part 100 patients shortlisted with diagnosis of osteoporosis by means of densitometria, considering variables as: age, density, trabecularización, thickness cortical, vertebral saucers, vertebral height, vertebral deformities, degree of vertebral compression,- RESULTS 34% of the participants present some type of deformity to column level dorsolumbar: 21 persons present wedge, 10 biconcave, 2 combination of wedge and biconcave and 1 person presents crushing. Methods and Materials Case and control study in patients from the Hospital Vicente Corral Moscoso and José Carrasco Arteaga of Cuenca-Ecuador; 52 patients with rheumatoid arthritis who satisfy the criteria of ACR-American College of Rheumatology and 52 healthy patients. The different types of HLA from each 52 patients with rheumatoid arthritis and controls were identified through PCR - Polymerase Chain Reaction. Association of HLA-DR1* with rheumatoid arthritis and rheumatoid factor, anti-citrullinated and rheumatoid nodules. The statistical analysis was done by the OR (IC 95%) and 2 of Pearson with Fisher correction and the information was processed in SPSS v15. Results The most frequency alleles were HLA-DRB1*09 (17,3%) OR 3,42 (IC95% 1,21 – 10,14) and HLADRB1* 04 (32,7%) OR 1,81 (IC95% 0,93 – 3,55). The rheumatoid factor and anti-cyclic citrullinated peptide antibodies were presented in 82,7% of patients with rheumatoid arthritis and rheumatoid nodules was detected in the 19.2% of patients with rheumatoid arthritis . An association between rheumatoid nodules with high values of rheumatoid factor and anti-ccp and these in turn with HLA-DRB1*14 y HLA-DRB1*04 alleles were found. The majority of patients with HLA-DRB1*09 alleles with heterozygote predomination had positive anti-ccp and rheumatoid factor. The objective was to analyze the problems affecting the quality of care provided to external users and is based on the satisfaction of the same. To carry out this research we use methods such as direct observation and surveys of patients and their families that assess the quality of care from their point of view, because they are involved in direct or indirect form respectively. The results were duly tabulated, analyzed and interpreted using charts and graphs. The disease is characterized by having vascular disease, autoimmunity and fibrosis. Produced multiple organ damage, generates significant social and health impacts and to date there is no cure. Objective: To determine the characteristics of the Systemic Sclerosis patients at the Hospital Vicente Corral Moscoso and José Carrasco Arteaga. Methods and Materials: It is a quantitative, descriptive and retrospective study. The universe is comprised of patients diagnosed with systemic sclerosis in both hospitals. The information was collected from medical records and analyzed in Microsoft Excel 2013 and SPSS 19. Quantitative and qualitative variables were used, showing through frequencies and percentages. A level of statistical association for p < 0.05 was considered. Results: Systemic sclerosis is more common in women. The limited form is predominant. The most common clinical manifestations are skin, followed by musculoskeletal and Raynaud's Syndrome. The group of 50-64 years is showing greater degree of general involvement; in younger people skin and musculoskeletal affectations are more frequent. Antinuclear antibodies were the best diagnostic value, being positive in more than 70 % of patients. The most common treatments are based on controlling symptoms, paracetamol being the most used (80.4 %). Conclusions: Systemic sclerosis affects more women than men; the most common age group is between 25-64 years. The limited form prevails over the diffuse. PATIENTS AND METHODS An analytical cross-sectional study was conducted in subjects of rural and urban areas, who are over 18 years, using the methodology COPCORD (Community Oriented Program for the Control of Rheumatic Diseases) adapted to Ecuador. We identified individuals with rheumatic symptoms, house by house. The revision to confirm the diagnosis was made by a rheumatologist. The statistical analysis was univariate and multivariate. RESULTS A total of 2500 people were surveyed. The 59.6% were female, with an average age of 42.8 years, the 25.5% of participants performed domestic activities. The MSK pain occurred in 32.2% of people, which was significantly associated with living in rural areas, have lower education, lower income to $ 340, increased physical load of 8 pounds, more functional disability, and cooking with firewood in the rural area. CONCLUSION The MSK pain has a high prevalence in the population of Cuenca, Ecuador; producing disability especially in rural areas. Clinical case: a 40-year-old female patient who presented with musculoskeletal pain and muscle weakness in the upper limbs accompanied by scaly papular lesions on the back of the hands and erythema on the eyelids, with increased serum muscle enzymes, suspected IIM, and a muscle biopsy was performed. Introducción: la disección de aorta (DA) consiste en una rotura de la capa íntima de la pared del vaso permitiendo el paso de sangre a la capa media separando la luz verdadera de la falsa, es una patología poco frecuente, que puede presentarse en pacientes con antecedentes de traumatismo torácico. Caso clínico: paciente masculino de 35 años, con antecedente de accidente en vehículo motorizado hace 18 meses, acude al servicio de emergencia por un cuadro de hemiparesia derecha más disartria de 4 horas de evolución, se realiza tomografía de cráneo sin encontrarse lesión, catalogándose como evento cerebrovascular isquémico, con recuperación neurológica espontánea, posterior presenta disnea de grandes esfuerzos, se realiza ecocardiograma reportando DA ascendente más aneurisma, se instaura tratamiento mediante medidas de soporte, con derivación para cirugía cardiotorácica. This research is descriptive, is to study and classify the object synovial fluid according to their macroscopic and microscopic findings crystal characteristics, as well as related diagnostic variables age and sex. We worked with hospitalized patients with synovitis and outpatient Vicente Corral Moscoso Hospital and Social Security of Cuenca, in 1998. 72 liquid were studied, 34.7 percent of patients with rheumatoid arthritis, 20.8 percent with arthritis, 13.9 percent drop and the remainder from diseases such as osteoarthritis, septic arthritis, systemic lupus erythematosus; being the most affected sex male 59.7 percent and the most prevalent age 50 to 69 years followed by 30 and 39. The aim of this research is to validate and cross-culturally adapt the COPCORD instrument in the indigenous population as a screening test for the detection of these diseases. METHODS: It is a descriptive study, the universe of 210 indigenous people over 18 years residing in Saraguro - Loja, Ecuador. During the period from December 1, 2016 to January 30, 2017. The COPCORD was adjusted to the Spanish language, a transcultural adaptation and validation was made to the context of the indigenous group. The information was entered into the statistical program of Stata Version 11, analyzed: Cronbach's alpha, correlation matrixes with the Spearman test. The variables were correlated with the diagnosis established by the rheumatologist. RESULTS: The average age was 46.1 years, 64.7 % of the female gender, the screening test to detect a rheumatic disease has sensitivity of 92.3 % and specificity of 57.9 %. The internal consistency of the questionnaire presented a dimensionality in the biomechanical load and functional capacity sections; and multidimensionality in the path of musculoskeletal pain and comorbidities. Significant correlations of the COPCORD were observed compared with the rheumatologist's evaluation. CONCLUSIONS: By carrying out the cross-cultural validation and adaptation of the COPCORD questionnaire, its usefulness could be demonstrated as a screening test for the detection of musculoskeletal pain and rheumatic diseases in the population of Saraguro.of approach including the laparoscopic approach
