    Evaluación de la utilidad de aplicación móvil para valoración del grado de pérdida hemática y shock en pacientes postparto del Hospital José Carrasco Arteaga Cuenca 2024

    Background: Globally, postpartum hemorrhage (PPH) occurs in 5–15% of deliveries and accounts for 25–30% of maternal deaths. In Ecuador, during 2021, PPH represented 7.78% of maternal mortality causes. Timely detection, availability of appropriate resources, and adequate response are crucial to preventing severe maternal morbidity and mortality. Objectives: To evaluate the utility of a mobile application in assessing the degree of blood loss and shock in postpartum patients at José Carrasco Arteaga Hospital, Cuenca, 2024. Methods: A descriptive study was conducted. Information was collected through surveys. Data were recorded in Excel, entered into the mobile application, and analyzed using SPSS. Results were classified as absent, mild, moderate, or severe postpartum hemorrhage based on clinical history, and as compensated, mild, moderate, or severe shock according to the mobile application. Results: Among 150 patients, 39.3% were aged 30–35 years, 80.7% resided in urban areas, 60.7% were privately employed, 46.7% were married, 52.7% had higher education, and 36.7% reported one pregnancy. Based on clinical history, 89.3% of patients did not present with hemorrhage, while 10.7% experienced postpartum hemorrhage. Regarding shock severity, a large percentage showed no signs of shock postpartum or at 01h00, 01h30, and 02h00. Similarly, the mobile application yielded comparable results, registering most cases as compensated shock. Conclusions: The mobile application demonstrated a high capability to identify cases of postpartum shock compared to clinical records, particularly in mild, moderate, and severe shock categories. This highlights its utility as a complementary technological tool in clinical settings to improve the detection and management of postpartum complications.

    Frecuencia de la intolerancia a la lactosa y síndrome de sobrecrecimiento bacteriano en pacientes con dispepsia sin signos de alarma. Estudio observacional descriptivo

    Introduction The prevalence and incidence of patients in private practice, presenting with dyspeptic symptoms as a chief complaint, is increasing, so we propose to determine the etiology in each patient, determining, lactose intolerance (LI) and intestinal bacterial overgrowth syndrome (SIBO), as the highest in prevalence. The exhaled H2 test is a practical and accessible method and has been validated in numerous studies as a diagnostic method. Objectives To establish the frequency of lactose intolerance and bacterial overgrowth in patients with dyspepsia without alarm signs in the Gastroenterology González specialty center in the city of Cuenca-Ecuador. Methodology This is a descriptive observational study, cross-sectional, which collected data through documentary analysis (medical records) and H2 exhaled test applied to 310 individuals of both sexes with an average age of 50±17 years, in the period February 2019 to February 2020. Results Lactose intolerance was observed in 29% of cases, while intestinal bacterial overgrowth in 20.3%.. Conclusions Lactose intolerance as well as bacterial overgrowth syndrome (SIBO), are frequent syndromes occurring in private practice, and should therefore be taken into account, when investigating a patient with dyspeptic symptoms, and should not be ruled out before performing a diagnostic test such as the exhaled H2 test.
