    Conocimientos y habilidades del personal de salud sobre consejería en lactancia materna del Hospital Vicente Corral Moscoso, Cuenca 2017

    Introduction: there is a relationship between lack of breastfeeding and malnutrition in children. The OMS recommends exclusive breastfeeding for six months and stay at least during the first two years of life for the benefits of breast milk. This is one reason why health personnel must be properly trained to support and promotes breastfeending. Objective: to determine the level of knowledge and skills of health personnel about breastfeeding at the Hospital Vicente Corral Moscoso that works in the period August - September 2017. Methodology: descriptive study in health professionals of the Hospital Vicente Corral Moscoso from August to September 2017. The ECoLa y survey was applied and information was processed with the PSPP program version 0.10.1 to generate frequency and percentage tables, measures of central tendency and dispersion. Results: it has been established that the level of knowledge and skill of health personnel is insufficient with 51% (30% with excellent category). Most of the sample has 10 or less years of experience. More than 60% of respondents have experience with breastfeeding with a mean breastfeeding of 8 months. In relation to the training on breastfeeding, 49% have not had any training on this subject. Conclusions: the study shows that there are gaps in the knowledge and skills on breastfeeding of health personnel surveyed, and it is necessary to work for the improvement of these parameters

    Conocimientos, actitudes y prácticas sobre alimentación complementaria de los/as cuidadores/as de pacientes atendidos en consulta externa del Hospital Vicente Corral Moscoso, Cuenca, 2018

    Background: the World Health Organization (WHO) and the United Nations Children's Fund (UNICEF) affirm that breastfeeding and complementary feeding are two fundamental pillars in life, growth, and development of children. They also reduce infant mortality rates, gastrointestinal infections, risk of malnutrition, and prevent allergic and celiac diseases. General objective: to identify the knowledge, the attitudes, and the practices on complementary feeding of the caregivers of patients that attended the medical consultation at the Hospital "Vicente Corral Moscoso". Methodology: is a cross-sectional descriptive study, with a sample of 297 caregivers of children from 6 months to 2 years that attended to the Hospital “Vicente Corral Moscoso” (HVCM), whose selection was made by systematic random sampling using the program Epidat 4.2. The form that was used was adapted from The Food and Agriculture Organization of the United Nations (FAO). With the obtained data, frequencies, percentages, mean and standard deviation were obtained; represented in tables. The statistical analysis was performed using SPSS version 15.0. Results: the knowledge in complementary feeding observed in the participants of this study was generally adequate (73%), the attitudes in feeding practices were correct in 81%; however, when reviewing the practices, only 49.3% provided the recommended diet. Conclusions: the caregivers have adequate knowledge and attitudes, but when it comes to putting in practice, inadequate feeding was observed.

    Estudio del crecimiento de niños preescolares en las guarderías de la área urbana de la ciudad de Cuenca

    El presente trabajo de investigación tiene como finalidad el estudio clínico basado en medidas antropométricas, realizado en niños preescolares de las guarderías del área urbana de la Ciudad de Cuenca considerando que las mismas tienen gran sensibilidad en la identificación y valoración de alteración en menor o mayor grado cuando esta se presenta. En nustro medio la falta de conocimiento sobre la importancia de una nutricion adecuada determina alteraciones en el crecimiento y desarrollo del niño preescolar lo que sumado a las condiciones socio económicas, culturales, ambientales propias de un país subdesarrollado, provoca que la mayor parte de niños sufran de desnutrición

    Interacción de la neurología pediátrica en la Atención Primaria de Salud (APS)

    At present, the health systems and development programs of health have become a patchwork of components. It is important to make a substantial reorientation of existing health systems at the level of Ecuador that focus on curative care, individual and position medicalized against international agreements, a prevention-based health, comprehensive and integrated promotion that could reduce 70% neurological disease burden in the country. The scientific development in the areas of neuroscience have given substantial advances in neurodevelopment, neuroimaging, neurophysiology, neuropsychology, interventional surgery in the treatment of abnormal movements and epilepsy. For the general practitioner and pediatrician that coexists in different stages of internal medicine, within a changing context is even more difficult to keep up, hence, that their decision to study and treat neurological diseases rely heavily on their preparation and their ability to self-criticism, that allows to define clearly and honestly its scope and limitations.

    Manifestaciones neurológicas en pacientes pediátricos con fenilcetonuria. Revisión bibliográfica

    Phenylketonuria (PKU) is a congenital autosomal recessive disease in which there is a shortage of the enzyme phenylalanine hydroxylase, which produces an injury to the central nervous system (CNS). The global incidence is about 1:20,000 live newborns. (1.2) The treatment is based on phenylalanine restricted diet which should be maintained for lifelong to prevent intellectual impairment. OBJECTIVE: To identify neurological manifestations of pediatric patients with PKU. METHODOLOGY: A review of the literature on the neurological manifestations for PKU by searching several electronic databases (MEDLINE , EMBASE, Cochrane , LILACS , PubMed) with English and Spanish limits from January 1970-2012. RESULTS: One cross-sectional study, one clinical case, three narrative reviews in pediatric populations and one clinical case combine children and adults. Intellectual disability is the main neurological manifestation; the primary diagnostic criterion was the serum level of phenylalanine. There are 29 different classifications on the cutoff to hyperphenylalaninemia and 23 for phenylketonuria.

    Neurodesarrollo en el hipotiroidismo congénito y sus particularidades electroencefalográficas

    Congenital hypothyroidism (CH) is a result of a deficiency of thyroid hormone (TH) by altering thyroid (primary hypothyroidism), or pituitary causes (secondary hypothyroidism) or hypothalamic (tertiary hypothyroidism). Thyroid hormone at the tissue level, is involved in the maturation and functional differentiation of various organs such as the development of the central nervous system (CNS), in which an insufficient supply of HT in time and place, resulting in permanent abnormalities, history highlighting the irreversible intellectual deficit. In the world’s population it has been estimated a frequency of 1: 300 to 1: 4000 newborns. The treatment is oral supplementation with thyroxin. OBJECTIVE: To describe the neurodevelopmental congenital hypothyroidism and electroencephalographic characteristics. MATERIALS AND METHOD: It is an observational, descriptive and cross-sectional study in preschool children diagnosed with HC, who were administered the Denver II scale, a comprehensive neurological evaluation and a digital electroencephalogram. RESULTS: A total of 12 patients, 7 (58%) female and 5 (42%) males were evaluated. All patients were evaluated by the scale of Denver II, 4 (33%) were delayed in their psychomotor development, predominantly in the language area. Regarding the neuro-pediatric assessment was found that 5 patients had neurological manifestations and perform digital electroencephalogram 7 (59%) had electroencephalographic changes, 5 (71%) showed immaturity and 2 (29%) dysfunction. CONCLUSIONS: Afer the analysis made throughout this investigation, the conclusions are: The results of this study show a heterogeneous pattern, however target the possible association between clinical manifestations and electroencephalographic findings. Patients with congenital hypothyroidism present a condition in the area of language which was determined by applying the scale Denver II. The abnormal clinical manifestations are mainly in the verbal area, but they are also presented as subtle manifestations or unspecific as inattention, irritability and behavioral disorders.     Prevalencia de infecciones respiratorias agudas y factores relacionados en niños de 0 a 5 años atendidos en el centro de salud nº 1 Pumapungo zona 6 distrito 01d01, en el año 2017

    Background: acute respiratory infections (ARI) are a group of diseases ranging from a common cold to severe bronchopulmonary processes, with a variety of causative agents that spread respiratory secretions. Objective: to determine the prevalence of ARI and related factors in children from 0 to 5 years of age served in Pumapungo Health Center No. 1 in 2017. Methodology: study, analytical, retrospective, 256 clinical histories of children who met inclusion criteria were reviewed. Sampling was randomized and statified according to months of care. The statistical analysis was performed using SPSS v15.0. The results were presented in personalized tables. Consequently, to establish the association relationship, was applied odds ratio and confidence interval of 95%, with p value <0.05. Results: the prevalence of ARI was 55.9% (95% CI 49.58 - 62.13). The high ARI was represented in 84.6% of cases, the decrease in 15.4%. Of the high IRAs, the most frequent was acute rhinopharyngitis with 67.8%, of the chronic bronchitis with 50%. Moreover, it was a statistical association with factors such as: complete vaccination scheme (OR 3.36, 95% CI: 1.-8.0), not benefiting from exclusive breastfeeding (OR 2.42, 95% CI: 1.29-4.58), and the presentation of an ARI according to months of year (OR 1.72, 95% CI: 1.0 - 2.9). Conclusions: the prevalence of ARI in the study population was high and a significant statistical relation of ARI was found with the vaccination schedule, breastfeeding and months of the year.
