Methods: Case-control in 174 patients over 18 years, presenting to the Clinic of Gastroenterology in Spain. Dyspepsia was defined according to the Rome Consensus II. The cases are 87 patients diagnosed by endoscopy ruled peptic ulcer, gastric cancer, esophagitis, constituting as functional dyspepsia, which had positive urease test. The controls constituted patients with functional dyspepsia after endoscopy and urease test-negative. Dyspepsia degree was determined by the presence of key symptoms. Symptoms were rated. Material and methods: A descriptive study was conducted in a universe of 135 patients. The data were obtained by direct source and analyzed with the help of Excel and SPSS software. Results: Male patients predominated (62.2%). The minimum age was 18 years, the maximum was 94.44.4% were aged 65 years or older. A history of alcohol intake was 57.8% and 14.1% NSAID. Chronic liver disease in 52.6% of cases. Hematemesis was detected in 88.9% and minor bleeding were classified as 71.9%. In assessing endoscopic esophageal varices was observed in 48.9%, erosive gastritis 18.5%, gastric ulcer 17.7%, duodenal ulcer in 10.3% and erosive esophagitis, gastrointestinal malignancies, Mallory Weiss, erosive duodenitis and vascular malformations within 10%. Depending on the scale of Rockall 31.1% of patients were at high risk of recurrent bleeding. According to the Child Pugh scale, was classified as Child C 37.8% of patients. The mortality at discharge was 17.03%. Conclusions: History of chronic alcohol intake (57.8%), chronic liver disease (52.6%). Endoscopically detected esophageal varices in 48.9%. With the scale of Rockall was observed 31.1% of patients at high risk of recurrence of bleeding. They have the advantage of its low cost. The necessity has been revised of carrying out clinical studies that validate them appropriately, in the places where they are used. Objective. To validate a preparation of ureasa test made in our unit ("laboratory test") in comparison with CLOtest and histologic result. Methods. prospective, randomized study of ureasa tests validation and results comparison, in patients with digestives symptoms and endoscopy indication. Random gastric antral biopsies were performed for the CLO-test and the lab test. In addition, two biopsies were performed for the histological study. We excluded patients who had recently taken medication that could change the HP diagnosis. The evaluation of the results was independently carried out. Results. 105 patients were included (mean age 45.43 ± 17 DS). 70.4% tested positive on the histological exam, whereas 65.8% and 64.3% tested positive on the CLOtest and on the lab test respectively: these differences were not statiscaly significants (p>0,05). The global value of the laboratory test was 93.33% (95% IC: 86.27 – 97.04); with 7 false negative and no false positive. The sensibility, specificity, value positive and negative predictive values were 90.54%, 100%, 100% and 81.57% respectively. The results of the CLOtest were similar: global value of 95.23% (95% IC: 88.70 – 98.23); 5 false negative and no false positives. The sensibility, specificity, value positive and negative predictive values were 93,24%, 100%, 100% and 86,11% respectively. None of the parameters showed significant differences (p>0.05). The reaction times of the tests were similar. Conclusion. The validation of our laboratory test provided similar results to the CLOtest.Item Validación del test de BISAP como sistema pronóstico en pancreatitis aguda, Hospital Vicente Corral Moscoso, Cuenca, Agosto 2009 - Julio 2010(Universidad de Cuenca, 2010-11-11) Villacis Barrazueta, Alicia Ximena; Calle Astudillo, Angel Gustavo; Vintimilla Maldonado, Jaime RosendoBackground: The BISAP score is a simple system which allows the prediction of acute pancreatitis mortality within the 24 hours of hospitalization. The system which englobes clinical variables (laboratory and imagenology tests) was validated in 2008. Aim: Determine the validity of the BISAP score in the prediction of both the prognosis and severity of acute pancreatitis in the Vicente Corral Moscoso Hospital within august 2009 and july 2010. Materials and Method: In order to validate the BISAP score, a study was performed with 57 patients with a diagnosis of acute pancreatitis at the moment of hospital admission. As a reference test, the Atlanta criteria, associated to the clinic evolution, were used. The data were obtained by direct interview and were analyzed with the SPSS software, and Excel. Results: Of the analyzed patients, 71.9% were women and 28.1% were men. The median age of the patients was 38 years. The gallstones was the most frequent cause (66.7%). According to the reference test 71.9% of the cases were mild and 28.1% of the cases were severe. According to the BISAP score 77.2% of the cases were mild and 22.8% of the cases were severe. The sensitivity and specificity for the BISAP score was of 75% and 97.56% respectively. The predictive positive value was 92.31% and the predictive negative value was 90.91%. Conclusions: The BISAP score allows to predict the severity on the acute pancreatitis.
