    Características: síntesis cutánea postcolecistectomía laparascópica en Hospitales José Carrasco Arteaga y Vicente Corral Moscoso 2013

    Laparoscopic cholecystectomy is the gold standard for cholelithiasis. We have options like nylon and polyglactin 910 to close skin in laparoscopic procedures. These materials have specific physical characteristics and tissue effects that make a difference in the patient. Objective: To describe the clinical features presented in the close skin wounds after laparoscopic cholecystectomy with polyglactin 910 and nylon in and José Carrasco Arteaga and Vicente Corral Moscoso Hospitals 2013. Materials and methods: a longitudinal study. All patients between 18 to 90 years old that came to the surgery department of José Carrasco Arteaga and Vicente Corral Moscoso Hospitals were included in this research. Results: in patients that the skin wound were closed with nylon the postoperative pain at the 8 hours after operation were low in 94.06 % of patients; there were no pain in 98.75% at 7th day. Scars at 7th days were grade 1 in 99.06 %. The 1.56 % of patients showed signs of infection at 7th day and 0.31 % at 15th day. For polyglactin 910 the postoperative pain were low in 70.93 % of patients and there was no pain in the 100% at 7th day. Scars at 7th day were grade 1 in the 100%. Signs of infection were observed in the 1.16 % of the patients at 7 and 15 days. Discussion: apparently the use of absorbable sutures has a better outcome in terms of postoperative pain and scarring. The rate of infection is similar with these sutures and it is less than the expected laparoscopy index infections (1 to 3%). Conclusion: polyglactin 910 shows better results in terms of postoperative pain and scarring and similar rates of infection compared to nylon.     Estrategias terapéuticas de aneurisma abdominal en sindrome de Marfan a propósito de un caso

    It is presented a case of a patient who attends outpatient consultation at the Vicente Corral Moscoso hospital with a diagnosis of abdominal aortic aneurysm which was identified through tomography. Besides, the patient shows a biotype compatible with Marfan syndrome which constitutes a challenge for diagnosis and therapy. Marfan syndrome is present in 1 to 3 cases out of 10,000 patients, and it is caused by defects in a gene called fibrillin-1. Fibrillin- 1 plays an important role for the connective tissue synthesis of the body; accompanied with cardiac and vascular malformations as well.

    Utilidad de examen de orina en apendicitis aguda

    INTRODUCTION: The aim of this study is to observe the frequency of positive elementary microscopic examination of urine (EMO) in regard to acute appendicitis appendiceal phase, and at the same time to determinate the sensitivity of the EMO to determine the infectious appendix phase. MATERIALS AND METHODS: We performed a prospective comparative study, which evaluated the sensitivity of the positivity of the EMO, the appendix phase and the urinary symptoms seen in these patients. We included 100 patients (64 women and 46 men) with an average age of 60. RESULTS: From the patients operated on and with histological test, positive signs and symptoms were showed which were obtained in pre-surgical clinical assessment in the Vicente Corral Moscoso Hospital. EMO was considered positive when leukocytes per field exceeded 8, and presence of red cells per field larger than 4. Of the 48 cases (48%) of Suppurative Appendicitis phase only 12 of them (12%) showed positivity to make the EMO, followed in frequency by the inflammatory phase in which inside these 32 cases (32%) only 7 (7% ) of them have to do the EMO positivity. A 100% of patients with acute appendicitis suffer pain at the right fist percussion and 98% of them did not have urinary frequency. CONCLUSIONS: The positive EMO has low sensitivity to determine the appendix phase and it is higher in the gangrenous stage. EMO can be positive in patients with acute appendicitis 4-1 Patients with acute appendicitis may present positive urinary symptoms including right fist percussion and middle ureteral points.
