    Tratamiento de hiperkalemia con solución polarizante en bolo versus solución continua, Hospital José Carrasco Arteaga (IESS) Cuenca, Ecuador 2010

    OBJECTIVE The objective of this work was to determinate the efficacy of the administration of a minimum quantity of dextrose-50ml, 50% water + 10 Ul insulin, in comparison to the continuous dextrose infusion, 10 % water + 20 Ul insulin, in the hiperkalemia's treatment. Materials and methods A randomized and opened clinical study was done in 50 patients diagnosed with hiperkalemia, affiliated at Internal Medicine’s Department of the Hospital José Carrasco Arteaga, during August 2009 to July 2010. The sample was calculated applying the formula for difference of stockings, the confidence level of the 97.5%, the statistical potency of the 99%, the variance of 0.429 and the minimum variation of 0.8 mEq/L of K+. The experimental group received 50 ml of dextrose, 50 % water and 10 Ul crystalline insulin administered during 20 minutes, and the control group received 1000 ml of dextrose, 10% water and 20 Ul crystalline insulin at the course of 24 hours. RESULTS In the beginning the groups were similar in age, sex, previous diagnoses and clinical manifestations (p>0.05). Posterior to the intervention, the average concentrations of potassium zeroed at 6, 12, 18 and 24 hours were equal (p >0.05); just like the concentrations of glucose, chlorine, sodium, creatinine and clinical manifestations; except, nausea and vertigo, valued at 18 hours, were significantly different in favor of the control group (p< 0.05).
