Item La estructura y funcionalidad familiar como factor causal en el inicio de las relaciones sexuales de los adolescentes en etapa temprana de la parroquia Checa, Cuenca 2015(Universidad de Cuenca, 2017-11-11) Sagñay Novillo, Andrés Felipe; Abril Matute, Cumandá PatriciaBackground: Sex at an early age represent a serious problem in society, due to the various consequences that entails both physical and social. Family dysfunction is among the main risk factors for early sexual activity. Objective: To determine the family structure as a causal factor in the onset of sexual relations of adolescents in early stage of the Checa parish, Cuenca 2015. Results: 51,1% in adolescents referred onset of sexual relations. The main reason was the love with 43,2%, 53,4% reported using contraceptives of which 82,4% is represented by condoms and 36,5% reported using it almost always. 56,7% exhibited a nuclear family and 59,7% a family with good function. The associated risk factors were male gender, performing household chores by the father, the evidenced protective factors are the housework of mother, parents and health personnel teaching about sexual activity and belong to a family with good functionality. Discussion: This study demonstrates that being part of a family that function properly is a preventive component to the early onset of sexual activity, which is necessary campaigns, policies and actions directed to the maintenance of appropriate family functionalism also increase the studies to determine the factors associated with dysfunctional familiesItem Estudio clínico aleatorizado multicéntrico de la eficacia de la detención controlada de la cabeza fetal para evitar el desgarro del periné(2014) Ballesteros Quijije, Favia Gabriela; Abril Matute, Cumandá Patricia; Arévalo Peláez, Carlos EduardoBackground: The protection of the perineum during delivery, has been recommended in several books of obstetrics, for this performing various maneuvers as controlled stopping of the fetal head and modified Ritgen maneuver, which have not been evaluated deeply, more research should be conducted clinics randomized to recommend its use and thus perform episiotomies narrowly. The goal of this study is to determine the efficacy of controlled stopping of the fetal head compared with modified Ritgen maneuver to avoid perineal tear in the second stage of nulliparous. Methods and Techniques: We conducted a multicenter randomized clinical trial, double blind, carried out in Vicente Corral Moscoso and Jose Carrasco Arteaga Hospitals at Cuenca-Ecuador. The study group was make up 59 nulliparous, which was run controlled stopping of the fetal head, and 57 nulliparous control group in which to perform the modified Ritgen maneuver, in total 116 patients. Results: The incidence of perineal tear in the experimental group was 46 patients with a total of 59, while in the control group were 42 of 57 patients was estimated RR 1.05, 95% CI (0.86-1.28) and a p value 0.590. Conclusions: There is less likely to occur with perineal tears maneuver Ritgen modified, not statistically significant, therefore the implications for clinical practice they're right for the two groups is both types of maneuvers can be used in order to prevent tearing of the perineum. The level of pain experienced and efficacy in decreasing pain influence the satisfaction of the mother. Objective: To determine the efficacy of Tramadol 100mg IM, to decrease the pain of active labor, interference with the duration of the active phase, vitality fetal, neonatal and significant adverse effects. Methods and techniques: We conducted a multicenter randomized controlled trial, blinded assessor and the statistical results in the “Hospital José Carrasco Arteaga” and “Vicente Corral Moscoso” of Cuenca- Ecuador, from December 2012 to september 2013, the study group was formed 73 parturients to Tramadol were administered during the active phase of labor, and for the control group were given 73 parturients N - butyl scopolamine. Results: it was found that at 30 minutes and when evaluated with tramadol, the pain was constant, but with the N-butyl scopolamine there was an increase, reaching the latter to be severe at 2 hours, which never happened to manage the Tramadol, even after 4 hours of evaluated, showing that there is a decrease in pain and a constant moderate pain all the time evaluated. Regarding the duration of the active phase, prolongation was not seen in the same, either impaired fetal vitality, but if there was a greater change in the newborn Apgar, which is not clear whether might be attributed to other causes. Regarding adverse effects of matched: nausea and vomiting were higher in the tramadol group without becoming a major contraindication for administration. Several studies show that adolescents each time set romantic relationships and initiate sexual activity at younger ages. Objectives: To determine the factors associated with the initiation of early sexual relations in adolescent students of the parish Tarqui, Cuenca 2015. Materials and Methods: We conducted an observational, analytical cross-sectional study with a sample of 211 adolescent students of the Tarqui in the city of Cuenca. A validated questionnaire was applied to determine the prevalence of sexual intercourse, the characteristics of the same and the associated factors. There was a descriptive analysis using frequencies, percentages, graphics and is reported average and standard for the deviation numerical variables. The association was determined using the prevalence ratio and its confidence interval of 95%. It was considered statistically significant results to the values of p<0.05. Results: The prevalence of sexual intercourse is of 17.1% CI95%: 12.2 - 22.18. The mean age was 17.4 years. In its most 80.6% the frequency is occasional, with a couple 33.3% defined as stable by 58.3%. The activity valued by the scale of Peeting that prevailed is the kisses 28.0%, the main reasons for initiating sexual activity were the curiosity 9.0%. It was low condom use 15.2% and the main source of sexual information were the parents 82.5%. Association was found with the Family Apgar RP 0.36 CI95%: 0.15-0.87, p=0.020; with the have a partner or in love or 8,510 CI95%: 3.64-19.9, p=0.000; be grade repeater RP 3.30 CI95%: 1.45-7.50, p=0.003; with the consumption of alcohol RP 4.98 CI95%: 2.34-10.6, p=0.000 and with the family structure RP 2.66 CI95%: 1.27-5.57, p=0.008.
