Abstract:
Third molar displacement (TM) is a rare complication that occurs in both the maxilla as well
as the mandible and can be of complete teeth or root fragments. The objective was to conduct
a systematic review of clinical cases of third molar displacement in the PubMed database,
obtaining a total of 50 articles. The most frequent site to which a third molar is displaced in the
maxilla is to the infratemporal fossa while in the mandible it is to the submandibular space.
The incidence was higher in women than in men with a 2:1 rate; the distolingual position of
the tooth and the thin bony cortical lamina are predisposing factors for displacement and the
main symptoms reported by the patients were limited mouth opening and pain. A correct
imaging diagnosis will facilitate better treatment planning to reduce complications in surgery
and if any complication occurs during or after the procedure, an experienced specialist should
treat it. Likewise, the early or late extraction of the displaced third molar will be decided by the
specialist evaluating the symptomatology of each patient.

El desplazamiento del tercer molar, es una complicación poco frecuente que ocurre tanto en
el maxilar como en la mandíbula y pueden ser de dientes completos o de fragmentos de
sus raíces. El objetivo de este artículo fue realizar una revisión sistemática de
desplazamientos accidentales del tercer molar en la base de datos de Pubmed en la que se
obtuvo un total de 50 artículos. El sitio más frecuente al que se desplaza un tercer molar en
el maxilar es a la fosa infratemporal mientras que en la mandíbula es al espacio
submandibular. La incidencia fue mayor en mujeres que en hombres con una relación 1.2:1;
la posición distolingual del diente y la lámina cortical ósea delgada son factores
predisponentes para el desplazamiento. Los principales síntomas reportados por los
pacientes fueron limitación en la apertura bucal y dolor. Un correcto diagnóstico
imagenológico nos facilitará una mejor planificación del tratamiento reduciendo
complicaciones en la cirugía. El momento de la extracción del molar desplazado lo decidirá
el clínico evaluando la sintomatología de cada paciente.
