Abstract: | ACKGROUND: Inflammatory fibrous hyperplasia associated with the use of removable dental pros-thesis (IFH) is an adaptive lesion caused by long-term trauma exerted by a poorly adapted remo-vable prosthesis on the oral mucosa, usually in the vestibular sulcus. Its diagnosis and treatment is imperative, due to its potential to cause discomfort to the patient, altering aesthetics, phonectics and chewing. CASE REPORTS: A 41-year-old denture wearer woman was referred due to discomfort in the right lower vestibular sulcus, the clinical examination showed a bilobed enlargement with an invagina-tion where the edge of the prosthesis fits, the patient has worn the prosthesis for 15 years. EVOLUTION: Resection with scalpel (conventional technique) was performed. The histopathologi-cal examination reported inflammatory fibrous hyperplasia and a new total removable bimaxillary prosthesis was made for the patient.CONCLUSIONS: HFI is one of the main oral lesions in older adults denture wearers and it causes aes-thetic and functional alterations; it is produced by the constant irritation caused by the settlement of the prosthesis borders on the mucovestibular sulcus as a consequence of alveolar resorption. Treat-ment is meant to eliminate the injury and its etiology; the absence of lesions on the mucosa and the bottom of the sulcus is completely necessary. |
Description: | INTRODUCCIÓN: La hiperplasia fibrosa inflamatoria asociada al uso de prótesis dental removible (HFI) es una lesión adaptativa causada por el trauma de largo plazo que ejerce una prótesis removible mal adaptada sobre la mucosa oral, generalmente en el surco vestibular. Su diagnóstico y tratamiento es imperativo, por su potencial de provocar incomodidad en el paciente, alterando la estética, fonética y la masticación.
CASO CLÍNICO: Mujer de 41 años portadora de prótesis, acudió por molestias en el surco vestibular inferior derecho, al examen clínico se observó un agrandamiento bilobulado con una invaginación en donde encaja el borde de una prótesis total removible, la cual utilizó por más de 15 años.
EVOLUCIÓN: Se realizó la exéresis de la masa con el uso de bisturí (técnica convencional). Al examen histopatológico, se diagnosticó una HFI y se elaboró una nueva prótesis total removible bimaxilar.
CONCLUSIÓN: La HFI constituye una de las principales lesiones orales en adultos mayores portadores de prótesis total y es causante de alteraciones estéticas y funcionales; se produce por la irritación constante causada por el asentamiento de los bordes de la prótesis sobre el surco mucovestibular como consecuencia de la reabsorción alveolar. El tratamiento va encaminado a eliminar la lesión y su etiología; la ausencia de lesiones de la mucosa y fondo del surco es completamente necesaria. |
