Abstract: | Prostate cancer is a common disease in men worldwide, the incidence of aggressive prostate
cancer is higher in Latin America than in other regions of the world, so timely and accurate
diagnosis is essential for disease management. The investigation of prostate biomarkers began
in 1960 with Rubin Flocks which found antigens in prostate tissue, in 1979 Dr. T Ming Chu
discovered and identified Purified Human Prostate Antigen (PSA); however, PSA was used for
the first time in 1991 as a biomarker for the detection of prostate cancer. Accurate and early
detection of cancer is vital as the survival rate decreases dramatically when the cancer has
migrated beyond the prostate organ. Due to the heterogeneity of the biomarkers used in the
diagnosis of prostate cancer and the lack of specificity of conventional imaging techniques, there
is no precise method for early detection of prostate cancer and even more so for predicting
aggressiveness. It is necessary to use new biomarkers to improve diagnosis.
The objective of this literature review was to analyze the usefulness of the Prostate Health Index
in the diagnosis of aggressive prostate cancer. The results showed that the quantified PHI in men
aged 45 to 75 years is useful in diagnosing the pathology over PSA, additionally it showed that
the measurement of PHI in the "gray zone" of PSA, that is, 4 to 10 ng/mL, allows clarify the
presence of prostate cancer and differentiate it from other pathologies. Due to the limitations of
the PSA diagnosis, the usefulness of the conventional PSA diagnosis together with the PHI was
analyzed, showing that their association improves the diagnosis and finally the use of the PHI was
evaluated to predict the presence of aggressive prostate cancer using the Gleason Scale. .
Therefore, this biomarker is presented as an alternative in the diagnosis and prediction of
aggressive prostate cancer due to the problems that PSA quantification presents in terms of the
rate of overdiagnosis and unnecessary biopsies. |
Description: | El cáncer de próstata es una patología habitual entre los hombres a nivel mundial, la incidencia
de cáncer de próstata agresivo es más alta en Latinoamérica que en otras partes del mundo por
lo que el diagnóstico oportuno y preciso es fundamental para un adecuado manejo de la
enfermedad. La investigación de los biomarcadores prostáticos se consolidó en 1979 cuando el
Dr. T Ming Chu, descubrió e identificó del Antígeno Purificado de Próstata Humana (PSA); sin
embargo el PSA se utilizó por primera vez en 1991 como- biomarcador para la detección del
cáncer de próstata. La detección precisa y temprana del cáncer es vital ya que la tasa de
supervivencia disminuye drásticamente cuando el cáncer ha migrado más allá del órgano
prostático. Debido a la heterogeneidad de los biomarcadores usados en el diagnóstico del cáncer
de próstata y la falta de especificidad de las técnicas de imagen convencionales, no existe un
método preciso para la detección temprana del cáncer de próstata y más aún para predecir la
agresividad del mismo, por lo que es necesario el uso de nuevos biomarcadores para mejorar el
diagnóstico.
Los resultados evidenciaron que el Índice de Salud Prostática (PHI) valorado en hombres de 45
– 75 años es útil en el diagnóstico de la patología por sobre el PSA, adicionalmente evidenció
que la medición del PHI en “zona gris” de PSA es decir de 4 – 10 ng/mL permite esclarecer la
presencia del cáncer de próstata y diferenciarlo de otras patologías, dadas las limitaciones del
diagnóstico mediante el PSA se analizó la utilidad del diagnóstico convencional mediante PSA
junto al PHI mostrando que su asociación mejora el diagnóstico y finalmente se evaluó el uso
del PHI para predecir la presencia del cáncer de próstata agresivo mediante el uso de la Escala
de Gleason. Por lo tanto este índice se presenta como un alternativa en el diagnóstico y
predicción del cáncer de próstata en la que se evidencia que es estadísticamente significativo en
comparación con el PSA para diagnosticar cáncer de próstata en edades comprendidas entre
45 – 75 años y en “zona gris”, además de evitar las biopsias innecesarias en un 30%; finalmente
se evidencia que el PHI mayor a 51 se puede relacionar con la Escala de Gleason para
diagnosticar el cáncer de próstata agresivo, dando respuesta a la problemática que presenta la
cuantificación del PSA en cuanto a la tasa de sobrediagnósticos y biopsias innecesarias. |
