| Abstract: | Introduction: Class II malocclusion is one of the most common skeletal alterations in our
population, being the majority of cases originated by mandibular retrognathism. The airway in
this malocclusion is related to a lower average airway volume and this in turn with obstructive
sleep apnea. The use of mandibular propulsors such as the Herbst and Advansync have shown
an increase in airway volume, so they are therapies for the treatment of class II and obstructive
sleep apnea by increasing airway volume.
General objetive: To compare the airway volume in tomographies of patients before and
after Herbst and Advansync therapy in the Postgraduate Orthodontic Clinic of the Faculty of
Dentistry of the University of Cuenca in the period May 2017-January 2018.
Methodology: For this retrospective longitudinal study, CBCT scans of 29 subjects aged 11
to 17 years were used, 15 subjects were treated with the Herbst appliance and 14 with the
Advansync appliance. The tomographies were evaluated using NemoFAB software, Nemostudio
2020 before and after using the mandibular advancement device.
Results: In the comparison of T1 and T2 measurements in nasopharynx, oropharynx and
total volume, a statistically significant difference was determined due to the increase in the
volume of the segments. On the other hand, the hypopharynx did not show a statistically
significant difference when comparing air volumes in T2 with T1.
Discussion: DAMs have become one of the main options for the treatment of respiratory
disorders due to their effectiveness in increasing airway volume, however, it is recommended
to evaluate their long-term efficacy.
Conclusions: DAMs increase total pharyngeal, nasopharyngeal and oropharyngeal volume,
while in the hypopharynx there is no statistically significant difference. Airway volume variations
are not related to the sex of the patients. There is no statistically significant difference when
comparing the increase in airway volume in patients who used the Herbst or Advansync
appliance |
| Description: | Introducción: La maloclusión de clase II es una de las alteraciones esqueléticas más
comunes en nuestra población, siendo la mayoría de casos originada por retrognatismo
mandibular. La vía aérea en ésta maloclusión está relacionada con un menor volumen promedio
de la vía aérea y éste a su vez con la apnea obstructiva del sueño. El uso de propulsores
mandibulares como el Herbst y Advansync han mostrado un aumento volumétrico de la vía
aérea, por lo que son terapias para el tratamiento de clase II y la apnea obstructiva del sueño
mediante el aumento de volumen de la vía aérea.
Objetivo general: Comparar el volumen de las vías aéreas en las tomografías de pacientes
antes y después de la terapia con Herbst y Advansync en la clínica de Postgrado de Ortodoncia
de la Facultad de Odontología de la Universidad de Cuenca en el período mayo 2017- enero
2018.
Metodología: Para este estudio longitudinal retrospectivo, se utilizó las CBCT de 29 sujetos
de 11 a 17 años de edad, 15 sujetos fueron tratados con el aparato Herbst y 14 el aparato
Advansync. Las tomografías fueron evaluadas mediante el software NemoFAB, Nemostudio
2020 antes y después de haber usado el dispositivo de avance mandibular.
Resultados: En la comparación de medidas de T1 y T2 en nasofaringe, orofaringe y volumen
total se determinó una diferencia estadísticamente significativa debido al aumento del volumen
de los segmentos. Por otro lado, la hipofaringe no mostró diferencia estadísticamente
significativa al comparar los volúmenes aéreos en T2 con T1.
Discusión: Los DAM se han convertido en una de las principales opciones para tratamiento
de transtornos respiratorios debido a la efectividad en el aumento de volumen de las vías
aéreas, sin embargo, se recomienda evaluar su eficacia a largo plazo.
Conclusiones: Los DAM aumentan el volumen faríngeo total, nasofaringe y orofaringe,
mientras que en la hipofaringe no existe una diferencia estadísticamente significativa. Las
variaciones del volumen de las vías aéreas no tienen relación con el sexo de los pacientes. No
existe diferencia estadísticamente significativa al comparar el aumento del volumen de la vía
área de los pacientes que usaron aparato de Herbst o Advansync. |
