Abstract: | Background: Intrauterine Growth Restriction (IUGR) is a pathology in which the
fetus is smaller than expected according to gestational age. Fetal Doppler is a tool
that offers detection of uteroplacental insufficiency; key in the diagnosis and
monitoring of IUGR.
Objective: to determine if the alteration in Doppler flowmetry in fetuses diagnosed
with IUGR is associated with adverse perinatal outcomes (APR), Hospital Vicente
Corral Moscoso. Basin, 2018.
Methodology: cross-sectional analytical study. The universe of clinical records for
the year 2018 was reviewed under inclusion and exclusion criteria. A data collection
instrument was developed based on the variables proposed. The data was tabulated
with the statistical program SPSS version 15. Descriptive statistics were applied;
frequencies and percentages were calculated for qualitative variables, and mean
and standard deviation for quantitative variables; To find the association between
Doppler flowmetry and RPA, RP and its 95% CI with p value were applied.
Results: The majority of pregnant women were young (60%), nulliparous (42%);
belonged to the urban area (58%) and had a cesarean delivery (64%). In the
newborn, 52% had low birth weight. 90% belonged to IUGR I and 74% to late IUGR.
14% of the neonates presented mild-moderate depression at the first minute, 4%
depression at five minutes; 10% died at birth and 34% were admitted to the ICU.
Type II and III IUGR, low birth weight and growth percentile less than 3 presented a
significant association for RPA. Based on the altered Doppler flowmetry, the RPA
were present in 6% of the umbilical artery, 22% of the uterine artery; both with a p
value <0.05. |
Description: | Antecedentes: la Restricción del Crecimiento Intrauterino (RCIU) es una patología
en la que el feto tiene un tamaño menor al esperado acorde a una edad gestacional.
El doppler fetal constituye una herramienta que ofrece una detección de
insuficiencia uteroplacentaria; clave en el diagnóstico y seguimiento de RCIU.
Objetivo: determinar si la alteración en la Flujometría Doppler en fetos con
diagnóstico de RCIU se encuentra asociada a resultados perinatales adversos
(RPA), Hospital Vicente Corral Moscoso. Cuenca, 2018.
Metodología: estudio analítico transversal. Se revisó el universo de expedientes
clínicos del año 2018 bajo criterios de inclusión y exclusión. Se elaboró un
instrumento de recolección de datos en base a las variables planteadas. Los datos
se tabularon con el programa estadístico SPSS versión 15. Se aplicó estadística
descriptiva; para las variables cualitativas se calcularon frecuencias y porcentajes,
y para las cuantitativas medias y desviación estándar; para buscar la asociación
entre la flujometría doppler y RPA se aplicó RP y su IC del 95% con valor de p.
Resultados: La mayoría de gestantes fueron jóvenes (60%), nulíparas (42%),
pertenecieron a el área urbana (58%) y tuvieron un parto por cesárea (64%). En el
neonato el 52% presentó bajo peso al nacer. 90% pertenecieron a la RCIU I y 74%
a la RCIU tardía. El 14% de los neonatos presentaron depresión leve-moderada al
primer minuto, el 4% depresión a los cinco minutos; el 10% murió al nacimiento y el
34% ingresó a la UCI. La RCIU tipo II y III, el bajo peso al nacer y el percentil de
crecimiento menor a 3 presentaron asociación significativa para RPA. En base a la
flujometría doppler alterada los RPA se presentaron en 6% de la arteria umbilical,
22% de la arteria uterina; ambos con un valor p <0,05. |
