dc.description | Antecedentes: el monitoreo electrónico fetal durante el trabajo del parto, puede asociarse a la reducción de las crisis epilépticas neonatales; sin embargo, fue relacionado con el aumento de las tasas de cesáreas en los centros de salud. Objetivo: estimar la validez del monitoreo electrónico fetal como predictor de compromiso de bienestar fetal en embarazos de bajo y alto riesgo obstétrico. Hospital Vicente Corral Moscoso, Cuenca - 2020. Método: estudio cuantitativo observacional de validación de test diagnóstico. La población estuvo constituida por 412 gestantes con diagnóstico de embarazo a término. Las variables a contrastar fueron: el monitoreo electrónico fetal con el test de Apgar del recién nacido. Se diseñó un formulario para recoger los datos. El análisis estadístico se realizó con el programa SPSS 25. Los resultados se presentaron: para variables cualitativas frecuencias y porcentajes, para variables cuantitativas media y desviación estándar; sensibilidad (S), especificidad (E), valor predictivo positivo (VPP), valor predictivo negativo (VPN) y curva ROC para estimar el valor predictivo del monitoreo electrónico fetal (MEF). Resultados esperados: el MEF en pacientes con riesgo obstétrico alto, como predictor del compromiso bienestar fetal al minuto, tuvo una S = 30%, VPP = 5%, E = 80,94% y valor VPN 94,93%. Asimismo, el MEF tuvo valor pronóstico bajo, sin significancia estadística (AUC = 0,5537; IC95% 0,4020 - 0,7054; p = 0,0774) para predecir compromiso del bienestar fetal. Conclusiones: el MEF, tiene una baja exactitud como test diagnóstico para predecir compromiso del bienestar fetal en embarazos de bajo y alto riesgo obstétrico. | en_US |
dc.description.abstract | Background: Electronic fetal monitoring during labor may be associated with reduced neonatal seizures, however, it was linked to increased rates of caesarean sections in health centers. Objective: To estimate the validity of electronic fetal monitoring as a predictor of fetal well-being compromise in pregnancies of low and high obstetric risk. Vicente Corral Moscoso Hospital, Cuenca - 2020. Method: Quantitative observational study of diagnostic test validation. The study sample consisted of 412 pregnant women diagnosed with pregnancy at full term. The variables to be contrasted were electronic fetal monitoring with the Apgar test of the newborn. A form was designed to collect the data. The statistical analysis was performed using the SPSS 25 program. The results were presented; for qualitative variables frequencies and percentages, for quantitative variables mean and standard deviation; sensitivity (S), specificity (E), positive predictive value (PPV), negative predictive value (NPV) and ROC curve to estimate the predictive value of electronic fetal monitoring (EFM). Expected results: The EFM in patients with high obstetric risk as a predictor of fetal well-being compromise at one minute, had an S = 30%, PPV = 5%, E = 80.94% and NPV value 94.93%. The EFM also had a low prognostic value without statistical significance (AUC = 0.5537; CI95% 0.4020 - 0.7054; p = 0.0774) to predict fetal well-being compromise. Conclusions: The EFM has a low accuracy as a diagnostic test to predict compromise of fetal well-being in pregnancies of low and high obstetric risk. | en_US |
