Abstract: | Background: Hepatic blood flow and its parameters can be affected by hemodynamic changes and these can be measured in Doppler ultrasound. Authors note that Doppler analysis is valuable for classifying liver disease according to prognosis and severity.
Objective: To correlate liver disease and hemodynamics of the hepatic vein and artery and portal vein assessed by Doppler ultrasound. Methodology: Doppler in 80 cirrhotic patients correlated with the Child-Pugh score, prospectively collected data, analysis in SPSS version 2.1. and evaluated by Spearman's correlation.
Results: The mean age was 56.89 + 13,87, 66.25% were men. The Child-Pugh classification corresponds to class A (23.6%), B (23.6%) and C (52.5%). The velocity of the portal decreases as the cirrhosis worsens (p = 0.01), the diameter, wave morphology and type of flow of the portal showed a positive correlation as the Child score increases with r: 0.583, p = 0.001; r: 0.684; p = 0.001 and r: 0.255, p: 0.05 respectively. The hepatic vein wave and hepatic artery resistance index were significantly higher in class C (p = 0.01).
Conclusion: Of the study population, 68.75% were adults, mostly men and with a Child C score (52.50%). The speed of the portal decreases, the diameter of the portal increases, the flow reverses and fasicity is lost with the worsening of the Child-Pugh class. The change in the shape of the hepatic vein and increased resistance index of the hepatic artery suggest progressive hepatic dysfunction. |
Description: | Antecedentes: El flujo sanguíneo hepático y sus parámetros pueden verse afectados por cambios hemodinámicos y estos pueden medirse en ecografía Doppler. Autores señalan que el análisis Doppler es valioso para clasificar la enfermedad hepática, según pronóstico y gravedad.
Objetivo: Correlacionar la enfermedad hepática y la hemodinámica de la vena y arteria hepáticas y vena porta valorada por ecografía Doppler.
Metodología: Doppler en 80 pacientes cirróticos correlacionada con la puntuación Child-Pugh, datos recolectados prospectivamente, análisis en SPSS versión 2.1. y evaluada mediante correlación de Spearman.
Resultados: La media de edad fue 56,89 + 13,87, el 66,25% fueron hombres. La clasificación de Child-Pugh corresponde a clase A (23,6%), B (23,6%) y C (52,5%). La velocidad de la porta disminuye según empeora la cirrosis (p=0,01), el diámetro, morfología de onda y tipo de flujo de la porta mostraron correlación positiva según incrementa la puntuación Child con r:0,583, p=0,001; r:0,684; p= 0,001 y r: 0,255, p:0,05 respectivamente. La onda de la vena hepática y el índice de resistencia de la arteria hepática fueron significativamente mayores en clase C (p=0,01).
Conclusión: De la población de estudio, el 68,75% fueron adultos, mayormente hombres y con puntuación Child C (52,50%). La velocidad de la porta disminuye, el diámetro de la porta incrementa, el flujo invierte y se pierde fasicidad con el empeoramiento de la clase de Child-Pugh. El cambio en la forma de la vena hepática y aumento del índice de resistencia de la arteria hepática sugiere una disfunción hepática progresiva. |
