dc.description | Antecedentes: la hemorragia posparto (HPP) es una complicación obstétrica con alta morbimortalidad, problemática que se acentúa en países en vías de desarrollo, por sus repercusiones es prioritaria una oportuna identificación y manejo. Objetivo general: determinar la prevalencia, causas, factores de riesgo y efectividad del tratamiento médico y/o quirúrgico de la HPP. Metodología: revisión sistemática. Se incluyeron ensayos clínicos controlados, aleatorizados, estudios analíticos, descriptivos, publicados en los últimos 5 años, sin discriminación de idioma. La búsqueda se realizó en bases electrónicas como Cochrane Library Plus, Embase, Pubmed y Lilacs. La información se sintetizó en tablas personalizadas. Resultados: la prevalencia de HPP fluctúa entre 2,1% y 79,6%. La atonía uterina, fue la causa más común de HPP, seguida del trauma. La edad ≥ 35 años, nuliparidad, antecedentes de HPP, trastornos hipertensivos del embarazo, mayor duración de la tercera etapa de la labor de parto, partos instrumentales y desgarros perineales fueron considerados como factores de riesgo. La sutura de Hayman mostró efectividad entre el 87 y 95,1% y el taponamiento con balón hidrostático entre el 91 y 96% en el control HPP. Conclusiones: la prevalencia varía según la geografía, la atonía uterina fue la causa más frecuente, la nuliparidad fue el factor más comúnmente asociado a HPP. La sutura de Hayman y el taponamiento hidrostático mostraron alta efectividad en el manejo de HPP. Limitaciones: restricción en el acceso a información por rubros económicos, además reducidos estudios con muestras representativas que evaluaron la efectividad en el manejo de HPP. | en_US |
dc.description.abstract | Background: Postpartum hemorrhage (PPH) is an obstetric complication with high morbidity and mortality, a problem that is accentuated in developing countries, due to its repercussions, timely identification and management is a priority. General objective: to determine the prevalence, causes, risk factors and effectiveness of the medical and / or surgical treatment of PPH. Methodology: systematic review. Controlled, randomized clinical trials, analytical, descriptive studies, published in the last 5 years, without language discrimination, were included. The search was carried out in electronic databases such as Cochrane Library Plus, Embase, Pubmed and Lilacs. The information was synthesized in personalized tables. Results: the prevalence of PPH fluctuates between 2.1% and 79.6%. Uterine atony was the most common cause of PPH, followed by trauma. Age ≥ 35 years, nulliparity, history of PPH, hypertensive disorders of pregnancy, longer duration of the third stage of labor, instrumental deliveries, and perineal tears were considered as risk factors. Hayman suture showed effectiveness between 87 and 95.1% and hydrostatic balloon tamponade between 91 and 96% in the PPH control. Conclusions: the prevalence varies according to access to health, uterine atony was the most frequent cause, nulliparity was the factor most commonly associated with PPH. The Hayman suture and hydrostatic tamponade showed high effectiveness in the management of PPH. Limitations: restriction on access to information by economic category, in addition, limited studies with representative samples that evaluated the effectiveness in the management of PPH. | en_US |
