Abstract: | BACKGROUND. Knowing parameters that provide timely information about the severity of the
patient is a useful prognostic tool to determine the evolution of the patient.
OBJECTIVE. To determine the prognostic variables of mortality in the patient admitted to the
Intensive Care Unit of the Vicente Corral Moscoso Regional Hospital in the city of Cuenca,
Ecuador during the period January to December 2019.
METHODOLOGY. Quantitative, analytical study; the universe was constituted by the patients
admitted to the Pediatric Intensive Care Unit during 2019. The information was tabulated and
analyzed in the SPSS15 program; a logistic regression analysis was performed taking as a
dependent variable the mortality recorded and as independent variables the PM2 score, sex, age
and provenance. Standardized mortality was used to determine the prognostic usefulness of PIM2
and the ROC curve was used to determine the discriminatory mortality capacity.
RESULTS. 222 patients were studied; 53.6% were women, the age group that predominated was
that of infants (37.8%). The 56.3% suffered from comorbidity, with the most frequent heart
diseases (40%). The mortality expected by PIM2 was the same as that observed, which was
10.4%. a standardized mortality of 1.00 was established. The area under the ROC curve was 0.827
(95% CI 0.728-0.925).
CONCLUSIONS. The PIM2 scale is a good predictor of mortality; however, socio-demographic
(age, sex, origin) variables do not interfere with or predict mortality. |
Description: | ANTECEDENTES. Conocer parámetros que proporcionen información oportuna sobre la
gravedad del paciente constituye una herramienta pronóstica útil para determinar la evolución del
paciente crítico.
OBJETIVO. Determinar las variables pronósticas de mortalidad en el paciente ingresado en la
Unidad de Cuidados Intensivos del Hospital Regional Vicente Corral Moscoso de la ciudad de
Cuenca, Ecuador durante el periodo enero a diciembre 2019.
METODOLOGÍA. Estudio cuantitativo, analítico; el universo estuvo constituido por los
pacientes ingresados en la Unidad de Cuidados Intensivos Pediátricos durante el año 2019. La
información se tabuló y analizó en el programa SPSS15; se realizó un análisis de regresión
logística tomando como variable dependiente la mortalidad registrada y como variables
independientes el puntaje PIM2, sexo, edad y procedencia. Para determinar la utilidad pronóstica
del PIM2 se realizó la mortalidad estandarizada y para determinar la capacidad discriminatoria
de mortalidad la curva ROC.
RESULTADOS. Se estudiaron 222 pacientes; el 53.6% fueron mujeres, el grupo etario que
predominó fue el de lactantes (37.8%). El 56.3% padecía comorbilidad siendo las cardiopatías las
más frecuentes (40%). La mortalidad esperada por el PIM2 fue la misma que la observada,
(10.4%) con una mortalidad estandarizada de 1.00. El área bajo la curva ROC fue de 0.827 (IC
95% 0.728-0.925). La edad, sexo, procedencia, comorbilidad no se identificaron como variables
pronósticas de mortalidad.
CONCLUSIONES. La escala PIM2 es un buen predictor de mortalidad; sin embargo, las
variables socio-demográficas (edad, sexo, procedencia) y comorbilidad no interfieren ni
pronostican la misma. |
