Abstract: | BACKGROUND: The high prevalence and incidence of cholecystitis and these complications
make it a pathology with high socioeconomic impact, it ́s important to make a timely and
accurate diagnosis. Is the main complication of cholelithiasis and the treatment is laparoscopic
cholecystectomy.
OBJECTIVE: To validate the Tokyo 2018 criteria with the intraoperative finding and
histopathological examination of the surgical specimen for the diagnosis of acute cholecystitis
in patients treated at the General Surgery Service of the José Carrasco Arteaga Hospital from
January to December 2019
METHODOLOGY: A diagnostic test validation study was conducted (Tokyo 2018 criteria)
with the histopathology results of the surgical piece, from January to December 2019. Once the
data were collected, these were codified and entered into a database in the SPSS version 15
system.
RESULTS: The average age was 49.1, been more frequent in female sex with 54%. Murphy's
sign present in 94% being a sensitive sign, fever 36%; 74% presented leukocytosis, 62% elevated
C-reactive protein, 74% thickened wall. The Tokyo guideline for histopathology has sensitivity
and specificity of 74% and 63.1%, respectively. Positive and negative predictive value of 46.7%
and 83.3%.
CONCLUSIONS: Acute cholecystitis is more frequent in the female sex. The degree of mild
severity was the most significant, and the sensitivity and specificity was low. That happen when
we use pathologic simples as gold standard. |
Description: | ANTECEDENTES: La alta prevalencia e incidencia de colecistitis y sus complicaciones, la
convierten en una patología con alto impacto socioeconómico, esto hace importante realizar un
diagnóstico oportuno y certero. Es la principal complicación de colelitiasis y el tratamiento es la
colecistectomía laparoscópica.
OBJETIVO: Validar los criterios de Tokio 2018 con el hallazgo intraoperatorio y el examen
histopatológico de la pieza quirúrgica para el diagnóstico de colecistitis aguda en pacientes
atendidos en el servicio de cirugía General del hospital José carrasco Arteaga desde enero a
diciembre del 2019
METODOLOGÍA: Se realizó un estudio de validación de pruebas diagnósticas (criterios Tokio
2018) con resultados de histopatología de la pieza quirúrgica, periodo enero a diciembre 2019.
Recolectados los datos se procedió a la codificación de los mismos e ingresados en una base de
datos en el sistema SPSS versión 15.
RESULTADOS: La edad promedio fue 49.1, siendo la colecistitis aguda más frecuente en el sexo
femenino el 54%. El signo de Murphy estuvo presente en la mayoría de pacientes 94%, siendo un
hallazgo sensible, fiebre 36%; 74% presentó leucocitosis, proteína C reactiva elevada 62%, 74%
pared engrosada. La guía de Tokio frente a histopatología tiene sensibilidad y especificad del 72%
y 63.1% respectivamente. Valor predictivo positivo y negativo de 46.7% y 83.3%.
CONCLUSIONES: la colecistitis aguda es más frecuente en el sexo femenino, siendo, el grado
de severidad leve el más significativo, la sensibilidad y especificidad fue baja, esto sucede cuando
se utilizan muestras patológicas como estándar de oro |
