Abstract: | Background: according to the World Health Organization, high hemoglobin is associated with gestational risk, being the active cause of maternal-fetal morbidity and mortality, no studies in the country and absent in state guidelines.
Objective: to determine prevalence of pregnant women with high hemoglobin and to identify its association with perinatal morbidity, at the José Carrasco Arteaga Specialty Hospital (HEJCA) period 2017 – 2019.
Methodology: an analytical, cross-sectional and retrospective study was carried out; with a universe of 3650 clinical records of pregnant seen in the HEJCA 2017 - 2019, the sample of 316 patients was calculated, tabulated and analyzed with SPSS19 and Excel2019, high maternal hemoglobin was associated with the development or not of complications, with odds ratio, 95% confidence index and p value, in 2 x 2 tables, sociodemographic variables, trimester of pregnancy and iron supplementation were also related.
Results: the population was 316, the majority age group was 22-32 years old, 95% were supplemented with iron. The prevalence of pregnant women with high hemoglobin was 60.44%. Perinatal morbidity was identified in 10.47% of patients with high hemoglobin, preeclampsia being the most frequent. There was a statistically significant relationship between high hemoglobin in the second and third trimesters with preeclampsia (OR: 2,120 IC al 95%: 1,609 – 2,792; p: 0,001); (OR: 1,502 IC al 95%: 1,143 – 2,107; p: 0,038). Gestational diabetes had a statistical association in the second trimester (OR: 2,504 IC al 95%: 2,186 – 2,868; p: 0,035).
Conclusions: There is a high prevalence of high hemoglobin and it is associated with maternal-fetal morbidity. |
Description: | Antecedentes: según la Organización Mundial de la Salud, hemoglobina alta, se asocia con riesgo gestacional. Siendo causa activa de morbimortalidad materno fetal, no estudiado en el país y ausente en guías estatales.
Objetivo: determinar prevalencia de gestantes con hemoglobina alta e identificar su asociación con morbilidad perinatal, en el Hospital de Especialidades José Carrasco Arteaga (HEJCA) periodo 2017 – 2019.
Metodología: se realizó un estudio analítico, transversal y retrospectivo; con un universo de 3650 historias clínicas de gestantes atendidas en el HEJCA 2017 – 2019, se calculó la muestra de 316, tabulándose y analizándose con SPSS19 y Excel2019, la hemoglobina materna alta se asoció con el desarrollo o no de complicaciones, con razón de momios, índice de confianza 95% y valor p, en tablas de 2 x 2, también se relacionó las variables sociodemográficas, trimestre de embarazo y suplementación de hierro.
Resultados: la población fue 316 gestantes, el grupo etario mayoritario fue de 22 – 32 años, 95% fue suplementada con hierro. La prevalencia de pacientes con hemoglobina alta fue de 60,44%. Se identificó morbilidad perinatal en el 10,47% de gestantes con hemoglobina alta siendo la preeclampsia la más frecuente. Existió relación estadísticamente significativa entre hemoglobina alta en el segundo y tercer trimestre con preeclampsia (OR: 2,120 IC al 95%: 1,609 – 2,792; p: 0,001); (OR: 1,502 IC al 95%: 1,143 – 2,107; p: 0,038). Diabetes gestacional tuvo asociación estadística en el segundo trimestre (OR: 2,504 IC al 95%: 2,186 – 2,868; p: 0,035).
Conclusiones: Existe gran prevalencia de hemoglobina alta y se asocia a morbilidad materno-fetal. |
