Abstract: | Background: Atrial fibrillation (AF) is one of the most common arrhythmic disorders. The cardio embolic events are the most frequent complication, they are preventable by anticoagulation. Among the most widely used drugs are vitamin K antagonists that require periodic monitoring of prothrombin time using the International Normalized Ratio (INR), considered normal between 2 and 3.Objective: to evaluate the status of INR control in patients with non-valvular atrial fibrillation treated with vitamin K antagonists in the outpatient service of the José Carrasco Arteaga Hospital.Methodology: a descriptive, cross-sectional study of retrospective data was carried out; the data were taken out from the medical records of 108 patients with a diagnosis of non-valvular atrial fibrillation who attended an outpatient cardiology consultation during 2018 at the José Carrasco Arteaga Hospital. It was considered as inadequate control when the time in therapeutic range (TRT) is less than 60% by the direct method and less than 65% by the Rosendaal method.Results: the study showed that the INR control status was adequate in 47.2% of patients by the direct method, while by the Rosendaal method, it was 41.7%. The 100% used warfarin. In the group of patients with poor control, there was evidence of an older age and a higher score on the HAS-BLED scale (p <0.0001).Conclusions: The INR control status was poor in more than 50% of patients by both methods, so these patients did not benefit from anticoagulant therapy with vitamin K antagonists. |
Description: | Antecedentes: la fibrilación auricular (FA) es uno de los trastornos arrítmicos más comunes. Los eventos cardioembólicos son su complicación más frecuente los mismos que son prevenibles mediante anticoagulación. Entre los fármacos más utilizados están los antagonistas de la vitamina K que requieren monitorización periódica del tiempo de protrombina utilizando el Índice Internacional Normalizado (INR por sus siglas en inglés) considerándose normal entre 2 y 3.Objetivo: evaluar el estado de control del INR en pacientes con fibrilación auricular no valvular (FANV) tratados con antagonistas de la vitamina K en el servicio de consulta externa del Hospital José Carrasco Arteaga.Metodología: se realizó un estudio descriptivo, transversal de datos retrospectivos, de las historias clínicas de 108 pacientes con diagnóstico de fibrilación auricular no valvular que acudieron a consulta externa de cardiología durante el año 2018 en el Hospital José Carrasco Arteaga. Se consideró como control inadecuado cuando el tiempo en rango terapéutico (TRT) es inferior al 60% por el método directo y menor al 65% por el método Rosendaal.Resultados: el estudio demostró que el estado de control del INR era adecuado en el 47.2% de pacientes por el método directo, mientras que por el método de Rosendaal fue del 41.7%. El 100% utilizó warfarina. En el grupo de pacientes con mal control se evidenció una mayor edad y mayor puntuación en la escala HAS-BLED (p<0,0001).Conclusiones: El estado de control del INR fue deficiente en más del 50% de pacientes por ambos métodos, por lo que estos pacientes no se beneficiaron de la terapia anticoagulante con antagonistas de la vitamina K. |
