Abstract: | Background: type 2 diabetes (T2DM) is considered a significant health problem
worldwide and in Ecuador; first-line treatment is oral antidiabetic drugs administered in
the absence of indications for insulinization. If oral therapy fails, insulin is administered,
which is the most potent anti-hyperglycemic agent available.
Objective: to determine the prevalence of therapeutic inertia at the time of initiating
insulinization in patients with type 2 diabetes, at the Hospital “José Carrasco Arteaga”,
in the city of Cuenca, in 2018.
Methodological design: observational, analytical, cross-sectional study. The universe
was made up of 3581 outpatients of Endocrinology and Internal Medicine of the Hospital
"José Carrasco Arteaga" in 2018, of which a total of 3,159 were excluded, due to non-
compliance with inclusion criteria, leaving 422 clinical records of patients with a diagnosis
of DM2, who have received two oral antidiabetic drugs for a period greater than three
months. Sociodemographic, clinical and therapeutic variables were measured. The data
collection was carried out from the records in medical records, in a form designed for the
study. The analysis was done in the SPSS v.25 program, applying descriptive statistics
and chi-square association, under a significance of 0.05; and the type of association was
calculated using Odds Ratio (OR), with 95% confidence intervals.
Results: the sample consisted of 422 diabetic patients with oral treatments; Of the total,
59.2% are 65 years or older, 54.7% were women, 35.8% had completed secondary
school, 46.7% were overweight, 96.7% had three or more years of evolution with the
disease, 63.7% performed glycemic control. 61.6% presented arterial hypertension as
the main comorbidity, 87% indicated Metformin plus Glibenclamide; 86.3% have
received treatment for more than three months. 25.8% presented therapeutic inertia and,
of these, 75.2% were related to health personnel. There was a significant association
between therapeutic inertia and glycemic control, treatment time and nephropathies (p
<0.05).
Conclusions: therapeutic inertia occurred in one of every four diabetic patients and in
most cases it was due to health personnel. Measures should be implemented to reduce
therapeutic inertia in patients with DM2. |
Description: | Antecedentes: la diabetes tipo 2 (DM2) es considerada un problema sanitario con
significancia a nivel mundial, y en Ecuador; el tratamiento de primera línea son los
antidiabéticos orales administrados en ausencia de indicaciones para insulinización. Si
la terapia oral fracasa, se procede a administrar insulina, que constituye el agente anti-
hiperglucémico más potente disponible.
Objetivo: determinar la prevalencia de inercia terapéutica al momento de iniciar la
insulinización en pacientes con diabetes tipo 2, en el Hospital “José Carrasco Arteaga”,
de la ciudad de Cuenca, en 2018.
Diseño metodológico: estudio observacional, analítico, transversal. El universo estuvo
conformado por 3581 pacientes de consulta externa de Endocrinología y Medicina
Interna del Hospital “José Carrasco Arteaga” en el año 2018, de los cuales se excluyó
un total de 3159, por incumplimiento de criterios de inclusión, quedando 422 historias
clínicas de pacientes con diagnóstico de DM2, que han recibido dos antidiabéticos
orales durante un período mayor a tres meses. Se midieron variables
sociodemográficas, clínicas y terapéuticas. La recolección de datos se realizó a partir
de los registros en historias clínicas, en un formulario diseñado para el estudio. El
análisis fue hecho en el programa SPSS v.25, aplicando estadística descriptiva y de
asociación chi-cuadrado, bajo una significancia de 0,05; y el tipo de asociación se
calculó el mediante Odds Ratio (OR), con intervalos de confianza al 95%.
Resultados: la muestra estuvo constituida por 422 pacientes con diabetes tipo 2, con
antidiabéticos orales como tratamiento; del total, el 59,2% tenía 65 años o más, 54,7%
fueron mujeres, 35,8% tuvo secundaria completa, 46,7% tenía sobrepeso, 96,7% tenía
tres o más años de evolución con la enfermedad, 63,7% presentaron un buen control
glucémico. El 61,6% presentó hipertensión arterial como principal comorbilidad, el 87%
tenía Metformina más Glibenclamida como tratamiento; 86,3% recibió tratamiento por
más de tres meses. El 25,8% presentó inercia terapéutica y, de estos, 75,2% se
relacionó con el personal de salud. Se observó asociación significativa entre inercia
terapéutica y control glucémico, tiempo de tratamiento y nefropatías (p<0.05).
Conclusiones: la inercia terapéutica se presentó en uno de cada cuatro pacientes
diabéticos y en la mayoría de los casos se debe al personal de salud. Se deben
implementar medidas para reducir la inercia terapéutica en pacientes con DM2. |
