| Abstract: | Background: diffuse large B-cell lymphoma (DLBCL) represents 1/3 of the lymphomas approximately. The regimen of rituximab, cyclophosphamide, doxorubicin, vincristine and prednisone (R-CHOP) constitutes as its first-line treatment.
Objective: to evaluate the results of R-CHOP treatment, in DLBCL in patients of The José Carrasco Arteaga Hospital in Cuenca, 2013-2018.
Methodology: a descriptive transversal study of 83 cases of DLBCL; the information obtained from the hospital database system (AS 400) was used, which was recollected at Excel 2016 and being processed at SPSS-v23.0 software. The analysis was organized by tables, using frequency, percentages and measures of key trends.
Results: 83 cases of DLBCL with R-CHOP treatment were studied. The female sex predominated with 53%. The average age was 61,41 years. Stage III and IV was observed in 36,1% and 31,3% of the cases. Low risk at the diagnosis was the main, corresponding to 34,9%. Nodal site was the most common (53%). Gastric affection was 46,2% of the extra-nodal involvement. 68.7% of the patients showed complete response, 6% partial response, 7,2% stable disease, 9,6% progressive disease and 8.4% died during treatment.
Conclusions: the DLBCL was more frequent in the female sex, with higher frequency in patients older than 70 years. Most of the patients had advanced stages and a low risk. The complete remission was presented in most cases |
| Description: | Antecedentes: el linfoma difuso de células B grandes (DLBCL) representa aproximadamente 1/3 de los linfomas. El esquema rituximab, ciclofosfamida, doxorrubicina, vincristina y prednisona (R-CHOP) constituye su tratamiento de primera línea.
Objetivo general: evaluar los resultados del tratamiento R-CHOP, en el DLBCL en los pacientes del Hospital José Carrasco Arteaga de Cuenca, 2013-2018.
Metodología: estudio descriptivo, transversal, de 83 casos de DLBCL; se utilizó la información obtenida del sistema de base de datos del hospital (AS 400), la que se recolectó en Excel 2016 y procesándose en el software SPSS-v23.0. El análisis se elaboró mediante tablas, utilizando frecuencia, porcentajes y medidas de tendencia central.
Resultados: se estudiaron 83 casos de DLBCL con tratamiento R-CHOP. El sexo femenino predominó con el 53%; la media de edad fue de 61,41 años. El estadio III y IV se observó en el 36,1% y 31,3% de los casos. El riesgo bajo al diagnóstico fue el principal, correspondiendo al 34,9%. La localización nodal fue la más común (53%); la afección gástrica fue el 46,2% del compromiso extra-nodal. 68,7% de los pacientes presentaron remisión completa, 6% remisión parcial, 7,2% enfermedad estable, 9,6% progresión de la enfermedad y 8,4% fallecieron durante el tratamiento.
Conclusiones: el DLBCL fue más frecuente en el sexo femenino, con mayor frecuencia en pacientes mayores a 70 años. La mayoría de los pacientes tuvieron estadios avanzados y un riesgo bajo. La remisión completa se presentó en la mayoría de los casos. |
