| dc.description | OBJETIVO: determinar la prevalencia de disfonía transitoria-permanente y su relación con los factores asociados, en pacientes postiroidectomías en el servicio de cirugía general y oncológica del Hospital José Carrasco Arteaga, Cuenca 2014 – 2016. MATERIAL Y METODOS: estudio retrospectivo, analítico y de corte transversal con 813 historias clínicas de pacientes que cumplieron los criterios de inclusión. Se estudió la disfonía postoperatoria respecto de: el sexo, tipo de histología, tipo de tiroidectomía, técnica quirúrgica, variante anatómica, antecedente irradiación cervical, cirugía cervical previa, antecedente lesión del nervio laríngeo recurrente y la reintervención quirúrgica. Se elaboró una base de datos y se analizaron las variables mediante el estadístico de asociación Chi2 y OR para determinar la existencia de riesgo con su significancia estadística (p< 0.05). RESULTADOS: la prevalencia de disfonía general postiroidectomía fue del 4,9% (n=40), del cual 82,5% (n=33) fue transitoria y 17,5% (n=7) permanente; como factores de riesgo para producir disfonía: el antecedente de irradiación cervical OR= 5,62 (IC 95%:1,50-20,99), el antecedente quirúrgico cervical OR= 9,89 (IC 95%:3,26-30,03), la tiroidectomía total más disección ganglionar OR= 6,66 (IC 95%:2,58-17,19), y la reintervención OR= 15,39 (IC 95%:7,35-32,26); siendo este último el mayor factor de riesgo y más importante para esta población de estudio. No se encontró asociación estadísticamente significativa (p> 0.05) con el resto de factores estudiados para esta población de estudio CONCLUSION: los factores asociados a la presencia de disfonía transitoria o permanente postiroidectomía son: la irradiación cervical, el antecedente quirúrgico cervical, la tiroidectomía total más disección ganglionar y la reintervención. | es_ES |
| dc.description.abstract | OBJECTIVE: to determine the prevalence of transitory-permanent dysphonia and its relationship with associated factors in posthyroidectomy patients that were asisted by the general and oncological surgery service at José Carrasco Arteaga Hospital, Cuenca 2014 - 2016. METHODS AND MATERIALS: a cross-sectional retrospective analytical study with 813 clinical histories of patients who reached the inclusion criteria. We studied the postoperative dysphonia with respect to sex, histology type, thyroidectomy type, surgical technique, anatomical variant, cervical irradiation antecedent, previous cervical surgery, recurrent laryngeal nerve injury antecedent and surgical reintervention. A database was developed and variables were analyzed through the association statistic Chi2 and OR to determine the existence of risk with its statistical significance (p< 0.05). RESULTS: the prevalence of general post-thyroidectomy dysphonia was 4.9% (n=40), of which 82,5% (n=33) was transient and 17,5% (n=7) permanent; as risk factors to produce dysphonia: cervical irradiation history OR= 5.62 (95% CI: 1.5020.99), cervical surgical history OR= 9.89 (95% CI: 3.26-30.03), total thyroidectomy and lymph node dissection OR= 6.66 (95% CI: 2.58-17.19), and reoperation OR 15.39 (95% CI: 7.35-32.26), being this last one the major risk factor and more important for this study population. No statistically significant association was found (p> 0.05) with the rest of the factors studied for this study population.
