| Abstract: | Introduction: Difficult laparoscopic cholecystectomy refers to the surgical removal of the gallbladder, existing conditions of the same organ or of the vesicle members, which prevent an easy, rapid and comfortable dissection of the vesicle, and which determine a prolongation of time surgical.
Objective: To determine the Validation of the preoperative score for the diagnosis of difficult cholecystectomy in the trauma and emergency service of the Hospital Vicente Corral Moscoso, in the period 2017
Method and materials: A validation study of the Preoperative score for difficult cholecystectomy was performed, using transoperative parameters as a gold test, in the period between January and December 2017. The data were taken to all patients, at the same time, laparoscopic cholecystectomy in the HVCM trauma and emergency service, which met the inclusion criteria, the sample was made from 221 patients
Results: A prevalence of difficult cholecystectomy of 11.31% and 3.62% of very difficult cholecystectomy was found; 42.53% had a history of acute cholecystitis; 83.26% presented a surgical time greater than 60 minutes; The sensitivity and specificity were 60.61% and 77.66% respectively, a positive predictive value of 32.26%, a negative predictive value of 91.82%, positive likelihood ratio 2.71, negative likelihood ratio 0, 51.
Conclusions: The sensitivity of the test was low, however, the specificity was high as well as the negative predictive value, to determine a prediction of difficult cholecystectomy |
| Description: | Introducción: La colecistectomía laparoscópica difícil se refiere a la extracción quirúrgica de la vesícula, cuando existen condiciones asociadas del mismo órgano, de sus órganos aledaños o la presencia comorbilidades del paciente, que impiden una disección fácil, rápida y cómoda de la vesícula, y que determinan una prolongación del tiempo quirúrgico.
Objetivo: Determinar la Validez del score preoperatorio para diagnóstico de Colecistectomía difícil en el servicio de Trauma y Emergencia del Hospital Vicente Corral Moscoso, en el periodo 2017
Método y materiales: Se realizó un estudio de validación del score preoperatorio para colecistectomía difícil, mediante parámetros transoperatorios como prueba de oro, en el período comprendido entre enero a diciembre de 2017. Los datos se tomaron a todos los pacientes sometidos a colecistectomía laparoscópica en el servicio de trauma y emergencia del Hospital Vicente Corral Moscoso, que cumplieron los criterios de inclusión, la muestra seleccionada fue de 221 pacientes
Resultados: Se encontró una prevalencia de colecistectomía difícil de 11,31%, y 3,62% de colecistectomía muy difícil; el 42,53% presentó un antecedente de colecistitis aguda; el 83,26% presentó un tiempo quirúrgico mayor a 60 minutos; la sensibilidad y especificidad fue del 60,61% y 77,66% respectivamente, un valor predictivo positivo de 32,26%, un valor predictivo negativo de 91,82%, razón de verosimilitud positiva 2,71, razón de verosimilitud negativa 0,51. Test de Youden 0,38.
Conclusiones: La sensibilidad de la prueba fue baja, sin embargo, la especificidad demostró ser alta, al igual que el valor predictivo negativo, para determinar una predicción de colecistectomía difícil |
