Abstract: | Background: acute pancreatitis is one of the main complications after endoscopic retrograde cholangiopancreatography (ERCP), occurring in up to 40% of cases. The associated risk factors behave differently between one population and another.
Objective: to determine the prevalence of acute pancreatitis after ERCP and its associated factors in patients at the José Carrasco Arteaga Hospital in the city of Cuenca during the period 2012-2016.
Method and Materials: cross – sectional study with analysis of associated risk factors. The population comprised of patients undergoing ERCP in the study period, of which the EPIDAT 3.1 program was used to obtain the sample, generating a total of 218 patients.
Results: the prevalence of acute pancreatitis after ERCP was 21%; as risk factors we found age less than 39 years (RP: 1.6, 95% CI: 1.5 to 4.3, p = 0.000), choledocholithiasis as a previous medical condition (RP: 1.9, 95% CI: 1.7 - 4.8, p <0.05) and, the normal state of the papilla (RP: 3, 95% CI: 2.1 - 7.7, p <0.05). The absence of comorbidity (RP: 0; p value of 0.000; IC95%: - 1) and the one that made less than 2 attempts to cannulate the duct (RP: 0.3, IC95%: 0.1 - 0.6, p 0.00), They were protective factors.
Conclusion: as associated factors for acute post-ERCP pancreatitis in the patients of the José Carrasco Arteaga hospital were choledocholithiasis, age of 39 years and the normal state of the papilla |
Description: | Antecedentes: la pancreatitis aguda es una de las principales complicaciones posterior a una colangiopancreatografía retrógrada endoscópica (CPRE), presentándose hasta en un 40% de casos. Los factores de riesgo asociados, se comportan de diferente forma entre una población y otra.
Objetivo: determinar la prevalencia de pancreatitis aguda post CPRE y sus factores asociados en pacientes del hospital José Carrasco Arteaga de la ciudad de Cuenca durante el período 2012 – 2016.
Método y materiales: estudio transversal con análisis de factores de riesgo asociados. La población conformada por los pacientes sometidos a CPRE en el periodo de estudio, de la cual se utilizó el programa EPIDAT 3.1 para obtener la muestra generando un total de 218 pacientes.
Resultados: la prevalencia de pancreatitis aguda post CPRE fue de 21%; como factores de riesgo se encontraron edad menor a 39 años (RP: 1.6; IC95%: 1.5 a 4.3; p= 0.000), coledocolitiasis como condición médica previa (RP: 1.9; IC95%: 1.7 – 4.8; p< 0.05) y, el estado normal de la papila (RP: 3; IC95%: 2.1 – 7.7; p< 0.05). La ausencia de comorbilidad (RP: 0; p valor de 0.000; IC95%: - 1) y el que se hayan realizado menos de 2 intentos de canulación del ducto (RP: 0.3; IC95%: 0.1 – 0.6; p 0.00), fueron factores protectores.
Conclusión: como factores asociados para pancreatitis aguda post CPRE en los pacientes del hospital José Carrasco Arteaga fueron la coledocolitiasis, edad de 39 años y el estado normal de la papila. |
